Dear Mr. McGrane:

Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program (WCP) to implement the FDEP approved Site Assessment Workplan (SAWP), dated August 30, 2024 and FDEP approval in an electronic mail correspondence from Patrick McGrane, dated September 5, 2024, for former Mitchell Cleaners (ERIC_5507) located at 309 N. Woodland Blvd, DeLand, Volusia County, Florida (Exhibit 1). The objective of this site assessment is to assess the horizontal and vertical extent of on-site chlorinated and petroleum solvent contamination.

A separate proposal (Task Assignment DC041B) was completed for the proposed work inside of the building including installing six (6) shallow soil borings, of which MAGS wells were installed inside five

(5) of those borings, one outside shallow soil boring and collecting soil and soil vapor samples for laboratory analysis due to pending improvements to be made by the property owner. This data will be included in the Site Assessment Report (SAR) that will be completed associated with TA DC041C.

SCOPE OF SERVICES

Subtask 1 – Project Management

Ralph F. Mongillo Jr P.G. will serve as the Project Manager during the field investigation. Donna Cline, P.E. will be involved throughout the project, serving as Authorized Project Reviewers.

Subtask 2 – Health and Safety Plan Update, Utility Clearance Outside Soil Borings/Sampling/MAGS Wells, Monitoring Well Installation, Lithological Profile Boring, DPT Groundwater Screening Profile and IDW Management

Pre-Task Planning will be performed to prepare for field investigations. Verify the property owner that signed the FDEP Permission to Enter Property form still owns the property. Terracon will revise existing site-specific health and safety plan. Terracon will continue to have communication with the property owner and his legal counsel prior to entering property, in accordance with the Terracon Property Access Agreement signed by both parties.

Utility Locate Prior to conducting the exterior soil sampling, lithological soil borings, MAGS and monitoring well installations, and DPT groundwater screening profile assessment, Terracon will subcontract the services of a subsurface geophysical survey company to identify potential subsurface utilities or obstructions. Geo Tek was the low bid and estimates that the work will take one day to complete. The geophysical survey bids are attached.

Soil Investigation Terracon personnel will complete five (5) soil borings to a depth of 20 feet below ground surface (bgs) using a GeoProbe Direct Push Technology (DPT) core sampler pushed from 5 to 20 feet bgs. The proposed soil boring locations are presented on Exhibit 2. Terracon personnel will document the lithology observed at each of these soil borings. Terracon personnel will document the lithology observed at each of these soils boring. Soil samples will be collected at each boring location at 0.5, 2, 4, 6, 8, 10, 15, and 20 feet bgs for laboratory analyses of the full list volatile organic compounds (VOCs) by EPA Method 8260 and total recoverable petroleum hydrocarbons (TRPH) by the FL-PRO Method (40 total soil samples). The TRPH sample aliquot will be enough to provide a sample to be

October 11, 2024 Terracon Proposal No. PEQ247452 analyzed for TRPH Fractions using the Total Petroleum Hydrocarbons Criteria Working Group (TPHCWG) Method should any results exceed the SCTLs. In addition, a 25-gram Encore Sampler will be used to collect a sample at each location (left on hold) for possible synthetic precipitation leaching procedure (SPLP) analysis by EPA Method 1311 in the samples where VOC parameter(s) exceed the SCTLs. Terracon will select samples for SPLP VOC analysis and have the laboratory complete the SPLP VOC analysis. This additional analysis SPLP VOCs and TPHCWG will be considered a Work Change Directive/Task Assignment Change Order. Each borehole, not used as a MAGS well, will be abandoned using a cement grout mixture and a similar surface replaced.

MAGS Well Installation As detailed in the SAWP, three (3) of the five (5) exterior deep soil borings will be completed as MAGS wells. The MAGS wells are proposed to a depth of 18 feet bgs, with 17 feet of 2-inch diameter 0.010-inch slot poly vinyl chloride (PVC) screen. Each of the interior MAGS wells will be completely flush to floor to avoid a tripping hazard. The MAGS wells will be completed flush to ground surface in an 8-inch diameter metal cover set within a 2-foot by 2-foot by 4-inch concrete pad. The MAGS wells may be used as a vapor extraction well for a future Soil Vapor Extraction (SVE) system, if necessary.

Lithologic Soil Profile Four (4) lithological profile borings will be advanced to an estimated depth of 35 feet bgs collecting continuous soil samples using a Geoprobe DPT to identify the top of clay. Terracon’s geologist will be on-site to describe the lithology. Upon completion and lithologic boring abandonment, a shallow zone monitoring well will be installed two feet from the boring location as described below.

Monitoring Well Installation Terracon will oversee the installation of six (6) shallow zone monitoring wells of which four (4) of the wells will be installed adjacent to the lithologic boring locations. The proposed monitoring well locations are presented in Exhibit 2. It is anticipated that the proposed shallow zone monitoring wells will be installed to a depth of approximately 30 feet bgs, with 10 feet pre-packed screen. Each well will be completed flush to ground surface in an 8-inch diameter metal cover set within a 2-foot by 2-foot by 4-inch concrete pad. Monitoring wells will be developed until the water is visibly free of sediment.

The monitoring wells will be surveyed using an arbitrary temporary benchmark (TBM) to assess groundwater flow direction. Presumed groundwater flow is believed to be towards the southeast.

DPT Groundwater Screening Profile The objective of the DPT groundwater profile assessment is to evaluate the extent and magnitude of drycleaning contaminants of interest within the on-site property boundary. Terracon anticipates advancing up to eight (8) DPT groundwater screening borings and collecting groundwater samples from four-foot vertical intervals and the following two (2) depths: 20 to 24 and 30 to 34 or until refusal at each location (16 groundwater samples). The proposed groundwater screening locations are presented in Exhibit 2. The groundwater screening samples will be collected by evacuating approximately one to two screen volumes of water to remove the initial fines from uncovering the screen at the desired depth. The groundwater samples will be submitted to the laboratory for analysis of full list VOCs and TRPH. Additional details are summarized in the SAWP (Terracon, 2024).

IDW Management and Disposal Soil and liquid IDW derived from soil cuttings, well development, decontamination, and purge water during the Subtask 3 investigation will be contained in DOT approved 55-gallon drums and temporarily stored onsite until the waste characterization sample results are completed. The liquid IDW will be analyzed for the full list VOCs and TRPH and the soil IDW will be analyzed for toxicity characteristic leaching procedure (TCLP) extraction for the full list VOCs, TCLP eight (8) RCRA metals, and TRPH.

Assuming the waste is non-hazardous, the drums will be transported to an approved facility for

October 11, 2024 Terracon Proposal No. PEQ247452

Work Change Directive/Task Assignment Change Order to complete the package so the 10-day notification deadline can be met by the FDEP.

As a cost saving measure, no additional mobilization costs will be incurred as the specific purpose survey by SurvTech Solutions, Inc. will begin the same day as groundwater sampling activities.

Terracon will complete an email deliverable for Proposal Subtask documenting the field event is completed by providing field notes, MAGS preliminary data, groundwater sampling forms, and laboratory reports.

Subtask 4 –PFAS Screening and IDW Management

Terracon will propose to sample groundwater in three (3) of the six (6) onsite monitoring wells for polyfluoroalkyl substances (PFAS). Terracon selected monitoring wells MW001, MW002, and MW004 to have samples collected for PFAS analysis. Terracon will complete the PFAS sampling in general accordance with the Terracon State of Florida Sampling Standard Operating Procedure PFAS Version 2.0, dated January 2024. The groundwater samples will be submitted overnight delivery to the FDEP Central Laboratory (DEP SOP LC-001-3).

IDW Management and Disposal Liquid IDW derived from purge water during the Subtask 4 and 5 investigations will be contained in DOT approved 55-gallon drums and temporarily stored onsite until the waste characterization sample results for TRPH, the full list VOCs, and PFAS can be evaluated. Assuming the waste is non-hazardous, the drums will be transported to an approved facility for disposal or treatment. Estimate one (1) drum of liquid.

This includes the electronic mail deliverable due to the FDEP in 48-hours after Terracon receives the PFAS analytical results. This will include preparation of a figure with the well locations and PFAS results, photograph documentation, laboratory data, PFAS checklist, and chain-of custody.

Terracon will complete an email deliverable for Subtask 4 documenting the field event is completed by providing field notes, groundwater sampling forms, laboratory reports and IDW manifest.

Terracon will prepare a Site Assessment Report that will detail and summarize the soil, groundwater, and soil vapor sampling activities, field observations, and results of soil and groundwater analysis and will include data tables and exhibits of groundwater flow and soil and groundwater concentrations.

Field data and laboratory reports collected under Task Assignment DC041B, work completed in the interior of the building, will be included in this report. Conclusions and recommendations will include a summary of results and recommendations for future sampling or remedial activities.

SCHEDULE

The availability of the low bid drilling subcontractor will determine when Terracon can schedule the work after receiving the signed Task Assignment. It is estimated that the drilling activities may be completed within a 1–2-month period based on subcontractor availability. MAGS testing and groundwater sampling will occur approximately 2-3 weeks after the wells are installed. Terracon anticipates the deliverable of site assessment activities within 30 business days of receipt of analytical results, pending the need for supplemental sampling activities.
