Document Summary

This document package contains a Task Assignment (TA) issued by the State of Florida Department of Environmental Protection (FDEP) under Contract No. HW687 to Terracon Consultants, Inc. for groundwater monitoring services at the Fabricare Cleaners and Laundry site (ERIC_4741) located at 13541 North Florida Avenue, Tampa, Hillsborough County, Florida. The TA is numbered DC008E with a completion date of September 11, 2026. The assignment is executed between the FDEP's Waste Cleanup Program and Terracon, with Gabriel Sostre serving as the FDEP project manager and Donna Cline as Terracon's project manager. The package includes the executed task assignment, cost estimates, detailed level of effort worksheets, a negotiation summary memorandum, the original proposal, competitive bid worksheets for subcontracted services, and a groundwater sampling summary table.

Terracon will conduct annual groundwater monitoring for volatile organic halides (VOHs) and per- and polyfluorinated substances (PFAS) across 17 monitoring wells, with VOH sampling from 8 wells and PFAS sampling from 14 wells, as well as prepare a comprehensive Groundwater Monitoring Report. The total task assignment value is $11,968.38 on a fixed unit rate basis, representing a negotiated decrease of $1,736.57 (12.67%) from the initial proposal of $13,704.95. The scope consists of three subtasks: Subtask 1 (Project Management—$1,038.09), Subtask 2 (VOH Groundwater Sampling and Annual Monitoring Report—$8,733.20), and Subtask 3 (PFAS Groundwater Sampling—$2,197.09). Deliverables are due by August 7, 2026, with invoicing contingent upon FDEP approval. The cost estimate encompasses 84.61 total labor hours distributed across six staff categories, travel costs of $360.00, laboratory analytical costs of $657.00, subcontractor costs for investigative-derived waste disposal of $341.00, and field equipment and expendables totaling $1,587.00. Additional vendors quoted for disposal services include Petrotech Southeast, Inc. (selected for IDW disposal at $310.00), Clean Earth Environmental Solutions, and Republic Services.

Below is a summary of each of the subtasks related to this TA:

Subtask 1: Reserved for Project Management.

Subtask 2: Terracon shall collect Groundwater Samples for VOHs from eight Monitoring Wells.

Additionally, Terracon shall also prepare an Annual Groundwater Monitoring Report that details and summarizes the findings obtained from the Annual Groundwater Monitoring Event.

Subtask 3: Terracon shall collect Groundwater Samples for PFAS from nine Monitoring Wells. Terracon will conduct PFAS sampling during the same mobilization as VOH sampling and will include the results obtained from the Annual PFAS event in the Annual Groundwater Monitoring Report.

The previous total cost of this TA was $13,704.95. After negotiations with the FDEP, the new total is $11,968.38. This represents a decrease of $1,736.57 or 12.67%. The total amount of labor hours dedicated to this TA is 84.61. Terracon may invoice according to included schedule.

Waste Cleanup Contract No. HW687 Terracon Proposal No. PH4267086

Dear Mr. Sostre:

Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program (WCP) to conduct groundwater monitoring for volatile organic halogenated (VOHs) and per-and polyfluorinated substances (PFAS) analysis, and prepare a Groundwater Monitoring Report for the Fabricare Cleaners and Laundry (ERIC_4741) site located in Tampa, Florida.

Scope of Services

Field preparation, mobilization to the site, collection of water levels from 17 existing monitoring wells, groundwater sampling and VOHs laboratory analysis from 8 selected monitoring wells (see attached Table 1).

Include PFAS analysis for 14 groundwater monitoring wells to evaluate horizontal and vertical PFAS groundwater impacts (see attached Table 1).

Properly manifest the groundwater drum of investigative-derived waste (IDW).

Submit PFAS samples in cooler preserved on ice by overnight courier to FDEP’s laboratory for sample analysis;

Review data and prepare a Groundwater Monitoring Report including updated tables, groundwater flow, and concentration exhibits;

Proposal for Groundwater Sampling and Reporting Fabricare Cleaners and Laundry, Inc. ERIC_4741 Tampa, FL April 17, 2026 Terracon Proposal No. PH4267086

See attached Groundwater Sampling Summary Table and LOE for details.

Schedule Deliverable Annual Groundwater Monitoring Report submitted within 30 business days following receipt of laboratory results.

Compensation See attached CEF for details.

SITE RECOMMENDATIONS

Terracon collected groundwater samples from 9 groundwater monitoring wells for VOHs analysis during the August 2025 sampling event. The prior scope of work also included an additional 7 monitoring wells sampling as part of the expanded PFAS assessment. The VOH contaminant plume has been vertically defined pursuant to Chapter 62-780, Florida Administrative Code (FAC). Horizontal delineation of the VOH contaminant plume remains undefined to the east in the intermediate-deep aquifer interval (monitoring wells screened between 36 and 46 feet below the ground surface);

however, the horizontal delineation has been established onsite to the north, offsite to the west, and offsite to the south.

Based on assessment data, PFAS concentrations have been reported above the FDEP Provisional Groundwater Cleanup Target Level of 70 nanograms per liter (ng/L) for Perfluorooctanoic acid (PFOA), Perfluorooctane sulfonic acid (PFOS), and their sum (PFOA + PFOS) in the shallow, intermediate, and intermediate-deep monitoring wells. PFAS groundwater plume remains undefined to the south of the former drycleaning site.

Based on the historical assessment data, Terracon recommends sampling 8 monitoring wells to evaluate VOH concentrations and trends. Terracon also recommends PFAS sampling from 14 monitoring wells to evaluate PFAS levels in groundwater. See Table 1 for the proposed groundwater sampling summary.

DELIVERABLES

The Groundwater Monitoring Report will detail and summarize the groundwater sampling activities, field observations, and results of monitoring well analysis, IDW disposal documentation updates to the site’s historical data tables, and updated exhibits of groundwater flow and well concentrations. Conclusions and recommendations will include a summary of results and recommendations for future sampling, closure, or remedial activities. The PFAS evaluation will be provided in an additional section within this report.

SCHEDULE

It is estimated that the wells could be sampled within two weeks from TA authorization and report issued within 30 business days of receipt of analytical data.
