The Task Assignment (TA Number DC028G) is issued pursuant to underlying Contract Number HW687, executed January 5, 2022, and authorizes work at the Alpine Cleaners facility located at 5240 NW 24th Boulevard, Gainesville, Alachua County, Florida (ERIC_3987). The primary parties are FDEP, represented by Project Manager Gabriel Sostre, and contractor Terracon Consultants, Inc., represented by Project Manager Donna Cline. Key personnel include Senior Geologist Ralph F. Mongillo Jr. as field project manager and Senior Remediation Engineer Donna Cline as authorized project reviewer.
The Task Assignment authorizes Terracon to conduct Annual Groundwater Monitoring and Reporting at the Alpine Cleaners site on a fixed unit rate basis. The scope encompasses two subtasks: Subtask 1 (Project Management) and Subtask 2 (Groundwater Sampling and Report Preparation). Deliverables include field notes and groundwater sample documentation due May 8, 2026, and an Annual Groundwater Monitoring Report due by June 12, 2026, contingent upon FDEP approval. The work involves collecting groundwater samples from five shallow zone monitoring wells (MW001S, MW002S, MW004S, MW005S, and PZ003) for volatile organic halogen (VOH) analysis, depth-to-water measurements from eight wells, investigative derived waste management and disposal, and preparation of a comprehensive report with analytical results and conclusions. The Total TA Value is $8,249.86, representing 49.33 total labor hours. Invoice payment of the full amount is due upon FDEP approval of the Annual Groundwater Monitoring Report. Cost components include direct labor ($2,171.15), overhead at 116 percent ($2,518.53), fringe benefits at 56.263 percent ($1,221.55), vehicle costs ($240.00), subcontractor services for IDW disposal ($214.50), laboratory analytical costs ($475.00), and field equipment and expendables ($818.00). The TA is effective as of February 10, 2026, and shall be completed by June 12, 2026.

Dear Mr. Sostre:
Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program to conduct Annual Groundwater Monitoring (GWM) activities and prepare a report documenting the results for the former Alpine Cleaners facility (Site), located at the intersection of 5240 NW 34th Boulevard, Suite E, in Gainesville, Alachua County, Florida.
The purpose of this proposal is to collect groundwater samples from five shallow zone monitoring wells for analysis of volatile organic halogens (VOHs) constituents of concern as identified in the recommendations section of the Supplemental Site Assessment Report, dated February 14, 2025 and prior to system startup of the Soil Vapor Extraction system. In addition, no groundwater samples are proposed to be collected for analysis of per- and polyfluoroalkyl substances (PFAS) constituents of concern since the previous PFAS results at concentrations below the Practical Groundwater Cleanup Target Levels (PGCTL) during the last two sampling events.

SCOPE OF SERVICES
Subtask 1 – Project Management
Ralph F. Mongillo Jr P.G. will serve as the Project Manager during the field investigation.
Donna Cline, P.E., will be involved throughout the project, serving as Authorized Project Reviewer.
Terracon obtained quotes from subcontractors for IDW management services. Exhibits of proposed sample locations are provided in Attachment B. A summary of subcontractor quotes and selected contractor is provided for each service in Attachment C and includes a copy of the quotes.
Subtask 2 – Groundwater Monitoring
Pre-Task Planning will be performed to prepare for field investigations. Terracon will update the existing site-specific health and safety plan. Terracon will also pick up the IDW drum, order the sample kits, and meet internally to discuss the scope of work, and the sampling equipment required.
Site Access Verifications and Notifications Verify that the source and off-site property owners that signed the FDEP Permission to Enter Property form still own the properties. Terracon will also notify the source property owner South Financial Services Inc. (Land Parcel No. 06083-025-000), and the tenant, and non-source offsite property owner J & Z Properties Inc. (Land Parcel Nos. 06083-026-002 and 06083-026-003) of the schedule for the field work.
Groundwater Well Sampling Groundwater samples will be collected from five (5) shallow zone wells (MW001S, MW002S, MW004S, MW005S, and PZ003) for VOH analysis (Exhibit 5). The monitoring wells will be sampled in accordance with the FDEP SOPs, dated January 2017 (effective April 16, 2018) for chlorinated solvents. Prior to monitoring well sampling activities, Terracon will collect depth-to-water measurements of the five shallow zone monitoring wells to be sampled and MW003S, PZ001, and PZ002.
Terracon will complete an email deliverable for Subtask 2 documenting the field event is completed by providing field notes, groundwater sampling forms, laboratory chain-of-custodies, and nonhazardous waste manifest.
Terracon will prepare an Annual Groundwater Monitoring (GWM) report that includes a summary of the sampling methodology and analytical results. The GWM report will include tables and figures summarizing the groundwater results in comparison to FDEP’s Cleanup Target Levels (groundwater). Exhibits will be provided to illustrate the site layout, sample locations, and results by matrix.
IDW Management and Disposal Liquid IDW derived from purge water during the Subtask 2 will be contained in one DOT approved 55-gallon drum and temporarily stored onsite until the waste characterization sample results for the full list VOCs can be evaluated. Assuming the waste is non-hazardous, the drum will be transported to an approved facility for disposal or treatment.
Estimate one (1) drum of liquid.
SCHEDULE
The goal of the report deliverable is to provide any data obtained by the end of May 2026.
