This is a Task Assignment Change Order (TACO #1) issued by the Florida Department of Environmental Protection (FDEP) to Terracon Consultants, Inc. under Contract HW687. The change order amends Task Assignment BIL03A for supplemental site assessment activities at the Former 3D Oil Distribution Facility (Site ID ERIC_5549) located at 1744 Seagrave Street, South Daytona, Volusia County, Florida. The change order extends the task assignment completion date from June 28, 2024, to August 2, 2024, a 35-day extension requested by the contractor due to delays in scheduling the drilling subcontractor. The total contract value remains unchanged at $71,080.66 on a fixed unit rate basis. The deliverable due dates were extended as follows: soil delineation and baseline groundwater sampling deliverables from March 29, 2024, to April 29, 2024; and the Supplemental Site Assessment Report from May 17, 2024, to June 28, 2024. Invoice payments are structured in three phases: $33,075.59 upon department approval of soil delineation field notes and sampling logs; $23,009.78 for baseline groundwater sampling documentation; and $14,995.29 upon approval of project management and the final assessment report, with no change to compensation terms.
