About this file
This document is a Task Assignment (TA) issued by the State of Florida Department of Environmental Protection (FDEP) to Terracon Consultants, Inc. for environmental remediation services at the Village Laundry site (ERIC_4743) located at 10325 Lake Carroll Way, Tampa, Florida. The Task Assignment DC014C is issued under the master contract HW687 and is effective through September 30, 2023. The scope of work authorizes costs for groundwater sampling and off-site access contacts and authorization. Specifically, the services include collection of a second round of groundwater samples from three deep monitoring wells (MW003V, MW004V, and MW007V) for volatile organic hydrocarbon (VOH) analysis, evaluation of right-of-way access for a downgradient monitoring well including a geophysical survey (GPR) to locate accessible drilling areas, submission of a Federal Department of Transportation (FDOT) right-of-way permit request, and delivery of a comprehensive email report with analytical results, updated groundwater tables, certified laboratory reports, field notes, and forms. The total Task Assignment value is $4,767.36 on a fixed unit rate basis, representing a negotiated reduction of $1,726.40 (27 percent) from the original proposal. The deliverable is due by June 30, 2023, with invoice payment due upon FDEP approval of the submitted materials. The cost estimate includes labor ($2,953.97), travel ($120.00), utilities geophysical survey subcontractor services ($1,072.50), laboratory analytical costs ($241.00), field equipment and expendables ($84.50), and a completion fee ($295.39).

TO: HW687 Contract File FROM: Dean Cox, Contract Manager SUBJECT: DC014C Groundwater Sampling – Village Laundry ERIC_4743
DATE: 5/16/23
This TA authorizes costs for groundwater sampling and offsite access contacts & authorization. The total cost of this TA is $4,767.36 and this entire amount is fixed unit rate. This TA was negotiated - the total cost is $1,726.40 or 27% less than originally proposed. Site access was confirmed.
The purpose of this TA is to collect a second round of groundwater samples from the deep monitoring wells. This data will help confirm deep contamination persistence atop the limestone directly above the Floridan and inform the planned installation of additional monitoring well(s) northwest offsite to complete horizontal (and possible vertical) plume delineation. Areas of DNAPL remain and concentrations remain moderate to high in general.

Task Assignment HW687 DC021B Terracon Proposal No. PH4237101
Dear Mr. Cox:
Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program to conduct deep well groundwater sampling for evaluate right-of-way access for Village Laundry (ERIC_4743) located at 10325 Lake Carroll Way, Tampa, Hillsborough County, Florida.
Scope of Services
Field preparation, mobilize to site, perform groundwater sampling and laboratory analysis of three (3) deep monitoring wells for VOH analysis;
Evaluate ROW access for a downgradient monitoring to include a GPR survey of the ROW to locate an accessible area for drilling;
Submit to FDOT a request for ROW access permit;
Submit a deliverable as an email to include an exhibit, certified lab report, field notes and forms, updated groundwater tables, and a summary of groundwater analytical results; and a summary of the GPR survey and ROW permit with FDOT.
See attached LOE for details
Schedule Deliverable submitted by April 30, 2023.
Compensation
See attached CEF for details.
The groundwater sampling is combined with a Village Laundry TA for PFAS sampling, which provides cost savings in IDW disposal, IDW drum, IDW transportation and disposal, mobilization, load/unload, vehicle, expendable, gw sampling kit.

Proposal for Deep Well Groundwater Sampling, ROW Access and Email Reporting Village Laundry, ERIC_4743 ■ Tampa, FL March 16, 2023 ■ Terracon Proposal No. PH4237101
SCOPE OF SERVICE DETAILS
In July and August 2022, Terracon performed a supplement site assessment event and reported the concentration of chlorinated solvent contaminants of concern at concentrations exceeding the applicable GCTLs in the samples collected from the on-site and off-site monitoring wells. These results were reported in a Supplementary Groundwater Assessment Report dated October 13, 2022. Vinyl chloride was reported at concentrations exceeding the GCTL in the newly installed deep wells. Before moving forward with the installation of additional deep well, FDEP recommended collecting an additional groundwater sample for laboratory analysis.
The downgradient well was not installed during the Supplemental Site Assessment due to utilities within the ROW and an inability to negotiate an off-site access agreement. Terracon plans to contact Sunshine 811 and complete a private utility geophysical survey in a 50 feet wide area of the ROW to identify a location for the downgradient well. Additionally, Terracon will submit an FDEP ROW Use Permit for installation of the off-site well as backup if private property access cannot be obtained.
CLOSING REMARKS
Terracon services will be conducted in accordance with the terms and conditions, contained within DEP Waste Cleanup Program (WCP) Contract No. HW687, executed January 5, 2022. It is understood that a Task Assignment will be issued to authorize us to perform this work.
We appreciate the opportunity to provide this proposal and look forward to working with you on this project. If you should have any questions or comments regarding this proposal, please contact either of the undersigned at 813-221-0050.
Sincerely, Terracon Consultants, Inc.
