Proposal for SB & MAGS Well Installation and Sampling Markhams Maytag Laundry, ERIC_3989 Alachua, Alachua County, FL May 22, 2025 Terracon Proposal No. PEQ257250

SCOPE OF SERVICES

Subtask 1 Project Management

Ralph F. Mongillo Jr P.G. will serve as the Project Manager during the field investigation.

Donna Cline, P.E., will be involved throughout the project, serving as Authorized Project Reviewer.

Subtask 2 Health and Safety Plan Preparation, Utility Clearance, Communications with Owner, Soil Sampling/MAGS well installation, and IDW Management

Pre-Task Planning will be performed to prepare for field investigations. Verify with the Alachua County Property office web site that the property owner who signed the FDEP Permission to Enter Property form and Terracon property access document, still owns the property. Terracon will prepare a site-specific health and safety plan. Communication with the property owner will be maintained during field activities per access agreement requirements.

Utility Locate Prior to conducting the field work, Terracon will subcontract the services of a subsurface geophysical survey company to identify potential subsurface utilities or obstructions. Geo Tek was the low bid and estimates that the work will take one day to complete. The geophysical survey bids are attached.

Soil Investigation Terracon personnel will complete three (3) soil borings (SB-03R, SB-04R and MAGS02) inside the building and three (3) soil borings (SB-01R/MAG01, MAGS03 and MAGS04) on the outside. The proposed depth varies for each soil boring as detailed below:

Soil Boring / MAGS well ID

Total Depth MAGS Well

Screen Interval

Depth of confirmation soil sample

SB-01R/MAGS01 20 feet below ground surface (ft bgs) or top of cap rock

10 ft bgs to top of cap rock

1, 2, 4, 6, 8 and 10, 15, and 20 ft bgs

SB-03R 4 ft bgs n/a 1, 2 and 4 ft bgs

SB-04R 10 ft bgs n/a 1, 2, 4, 6, 8 and 10 ft bgs

MAGS02 20 ft bgs or top of cap rock

0.5, 2, 4, 6, 8 and 10, 15, and 20 ft bgs

May 22, 2025 Terracon Proposal No. PEQ257250

Soil Boring / MAGS well ID

Total Depth MAGS Well

Screen Interval

Depth of confirmation soil sample

MAGS03 20 ft bgs or top of cap rock

10 ft bgs to top of cap rock

0.5, 2, 4, 6, 8 and 10, 15, and 20 ft bgs

MAGS04 20 ft bgs or top of cap rock

10 ft bgs to top of cap rock

0.5, 2, 4, 6, 8 and 10, 15, and 20 ft bgs

The proposed soil boring locations are presented in Attachment 2A of the FDEP Response to Comment Letter on the Groundwater Monitoring Report (Terracon, 2023).

Terracon personnel will document the lithology observed at each of these soils boring. Soil confirmation samples will be collected at each boring location at the depth specified in the summary table and as outlined in Attachment 2B (Former Soil Analytical Results).

Additionally, at the new MAGS well locations, soil samples will be collected at 0.5, 2, 4, 6, 8, 10, 15 and 20 ft bgs. Soil samples will be collected from soil borings SB-03R and SB-04R to depths of 4 and 10 ft bgs, respectively. The collected samples will be submitted for laboratory analyses of volatile organic halocarbons (VOHs) by EPA Method 8260 (41 total soil samples). A 25-gram Encore Sampler will be used to collect a sample at each location (left on hold) for possible synthetic precipitation leaching procedure (SPLP) analysis by EPA Method 1311 in the samples where VOH parameter(s) exceed the SCTLs. Terracon will select samples for SPLP VOC analysis and have the laboratory complete the SPLP VOC analysis. This additional analysis SPLP will be considered a Work Change Directive/Task Assignment Change Order.

MAGS Well Installation Four (4) of the six (6) soil borings will be completed as MAGS wells. The MAGS wells are proposed to a depth of approximately 20 ft bgs or will be installed to the top of the cap rock. Each MAGS well will be installed by hand auger and constructed with 2-inch diameter 0.010-inch slot poly vinyl chloride (PVC) screen from 10 ft bgs to the top of the cap rock.

Each of the interior MAGS wells will be completely flush to floor to avoid a tripping hazard.

The MAGS wells will be completed at the concrete slab surface with a 3.5-inch

The MAGS wells may be used as a vapor extraction well for a future Soil Vapor Extraction (SVE) system, if necessary.

IDW Management and Disposal Soil and liquid IDW derived from soil cuttings and decontamination during the investigations will be contained in DOT approved 55-gallon drums and temporarily stored onsite until the waste characterization sample results are completed. Erwin Remediation was the low bid and has been selected to manifest, transport and dispose of the estimated 3 drums of IDW.

May 22, 2025 Terracon Proposal No. PEQ257250

Terracon will complete an electronic mail deliverable for Subtask 2 documenting the field event is completed by providing field notes, soil sampling forms, MAGS well construction logs, and laboratory reports.

Subtask 3 - MAGS Testing/Sampling and Soil Boring and MAGS Letter Report

Modified Active Gas Sampling (MAGS) The proposed MAGS well locations are presented on Attachment 2A. The assessment will use MAGS testing procedures to assist in identifying areas at the site requiring additional assessment. A portable blower and generator mounted on a utility trailer will be utilized to conduct the MAGS test. A well head assembly will be connected to each MAGS well to allow for vacuum, flow, temperature, and OVA measurements. Vacuum influence will be measured from other MAGS wells and shallow monitor wells during the test.

A vacuum step test will be conducted at the first MAGS point, beginning with a lower applied vacuum, then increasing vacuum to reach an optimal vacuum to flow rate. This test will assess the Radius of Influence (ROI) and provide maximum vacuum levels at wellheads without excessive water entrainment.

Terracon will complete a Soil and MAGS Assessment report for Subtasks 3 documenting the field event, to include MAGS ROI calculations, field reading forms, laboratory reports, and evaluation of soil analytical results, with conclusions and recommendations based on the results of the soil and soil vapor testing for remedial remedies.

SCHEDULE

Terracon will schedule the work as soon as possible since there is no drilling subcontractor needed for the MAGS well installation. MAGS testing will occur approximately 1-2 weeks after the MAGS wells are installed. Terracon anticipates having the Soil Boring and MAGS Summary Report within 20 business days of receipt of analytical results to be used for decisions on remedial action on soils below the floor slab.

May 22, 2025 Terracon Proposal No. PEQ257250

RE: Utility Locating Services – 15089 M L King Blvd, Alachua, Florida

Mr. Mongillo, GeoTek Services is pleased to respond to your request for Utility Locating services at 15089 M L King Blvd, Alachua, Florida. We will scan six (10’ x 10’) areas with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings. We will scan as close as reasonably possible to fixed objects such as walls, landscaping, fences, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Best Regards, Martin (Bud) Connor GeoTek Services, LLC
