FROM: Patrick McGrane, Project Manager
SUBJECT: Negotiation Summary Task Assignment DC015B (HW687)
DATE: June 27, 2024
This Task Assignment authorizes Terracon Consultants, Inc. (Terracon) to conduct Supplemental Site
Assessment activities at the Markham Maytag Laundry site.
The total cost for this project is $113,850.67 with approximately 340 total labor hours. All work is to be completed in accordance with the June 26, 2024, Scope of Work, Cost Estimate, and Bid Documentation.
Terracon may invoice according to the included schedule.
Subtask 1:
Project Management is automatically calculated as a percentage of total labor in accordance with
Contract HW687.
Subtask 2:
Following authorization of this Task Assignment, Terracon will perform the work necessary to obtain the correct permits required for the proposed work and locate any underground utilities that may be present onsite. The current access agreements with both the source property and offsite property owners are over 25 years old. A portion of work in this Subtask involves costs for Terracons effort to obtain updated
Access Agreements. Seven existing monitoring wells (MW001, MW007, MW011, MW012, MW014, MW015, and MW020) are to be found and redeveloped, prior to installation of the new deep wells.
Subtask 3:
Terracon will install five additional permanent monitoring wells using a Sonic Rig to a depth of 95 feet bgs with a screened interval of 85-95 feet bgs. IDW produced during drilling activities and subsequent well development will be stored onsite and disposed of during work in Subtask 5.
Subtask 4:
Groundwater samples will be collected from the five new monitoring wells (MW021-MW025), the nine monitoring wells not sampled during the July 2022 sampling event, and source well MW018. These samples will be analyzed for VOHs via EPA Method 8260. For a description of each well ID, screened interval, and sample analysis, refer to the table included in the Scope of Work.
Subtask 5:
All costs related to PFAS sampling and the related IDW management are contained in this Subtask. In total, six wells will be analyzed for PFAS compounds (MW021, MW022, MW023, MW025, MW018, and
MW019). Due to PFAS compounds already screened onsite, IDW transport and disposal costs for
Subtasks 2, 3, and 4 are also included within this Subtask. It is estimated that 15 drums of soil and 10 drums of liquid will be produced between Subtasks 2 and 3. An estimated two drums of liquid waste will be produced between Subtasks 4 and 5.
Subtask 6:
Following the completion of all assessment activities, Terracon will submit a Supplemental Site
Assessment Report summarizing their findings with analytical results and tables as well as any
Conclusions and Recommendations based on the recent data and soil/groundwater conditions.

A total of five (5) of the six (6) proposed monitoring wells will be installed to a depth of approximately 95 feet bgs, except for the well proposed for behind the building because of difficult access for the sonic drill rig. The groundwater investigation is being conducted to evaluate the potential horizontal and vertical extent of chlorinates solvents at a screen elevation of 0 to -10 feet NAVD88. The permanent monitoring wells will be installed using a sonic drill rig by the selected drilling company Groundwater Protection, a division of DrillPro.
Permanent wells Five (5) permanent groundwater monitoring wells will be installed using the sonic drilling method for evaluation of groundwater quality in the deeper aquifer zone at the site vicinity. Soil cores will be collected at three of the five well locations for lithological logging prior to well installation. The deep groundwater monitoring wells will be screened from 85-95 feet bgs where the land surface elevation is approximately 85 feet North American Vertical Datum of 1988 (NAVD88), using 10 feet of 2-inch diameter 0.010-inch slotted screen. Each well will be completed by filling the borehole annulus with 20/30 sand to approximately 2 feet above the top of screen, followed by a 2-foot-thick fine sand seal consisting of 30/65 sand filter pack. The remaining annulus in each borehole will be completed to ground surface with neat cement grout. The wells will be completed with an 8-inch diameter traffic-rated metal cover set in a 2-foot by 2-foot concrete pad. Each well will be developed until the water is clear and free of sediment and provide a hydraulic connection with the aquifer.
Quality Assurance/Quality Control (QA/QC) Decontamination will be conducted between each boring, well installation and sample collection. QA/QC samples will be collected and analyzed for the PFAS constituents of concern as follows:
June 26, 2024 Terracon Proposal No. PHB237027
Field Equipment Blanks: one (1) sample per tooling on the sonic rig, for a total of one (1) sample.
Field Blanks: one (1) sample per drilling event for the sonic rig, for a total of one (1) sample.
The QA/QC samples will be packaged under proper chain-of-custody (COC) procedures and sent overnight delivery to the FDEP laboratory in Tallahassee, Florida for analysis.
Terracon will complete an email deliverable for Subtask 3 documenting the field event is completed by providing field notes, soil boring logs, Monitoring well construction logs, well development forms, and IDW characterization laboratory reports. The IDW disposal of these drums will be completed in Subtask 5 since the waste was previously identified to be impacted by PFAS.
Subtask 4 – Groundwater Investigation
Groundwater Well Sampling The last groundwater sampling event for the site was in July 2022; therefore, Terracon will use this effort as the 2024 annual groundwater monitoring event. Groundwater samples will be collected from the five newly installed monitoring wells (MW021 thru MW025), the nine (9) monitoring wells not sampled during the July 2022 groundwater sampling event, and the source well MW018, in accordance with the FDEP SOPs, dated January 2017 (effective April 16, 2018) for chlorinated solvents. Prior to monitoring well sampling, Terracon will collect depth-to-water measurements of the monitoring wells presented in the table below and MW002, MW003, MW004, MW005, MW006, MW008, MW010, and MW017. A summary table located below includes the monitoring wells, screen interval depths, and laboratory analyses for each monitoring well. The Table below identifies each monitoring well that will be sampled and the parameters to be analyzed.

Based on review of the groundwater results, Terracon will determine if an Initial Notice of Contamination (INOC) beyond property boundaries package needs to be completed and prepare a WCD/TACO to complete the package so the 10-day notification deadline can be met by the FDEP.
Survey Top of casing (TOC) elevations and horizontal control will be surveyed by SurvTech Solutions Inc., a professional land survey company, of the newly installed and existing monitoring wells. Project controls will be relative to the North American Datum 1983/2011 Realization (NAD83/11) for horizontal criteria and the North American Vertical Datum of 1988 (NAVD88) for vertical criteria.
Terracon will complete an email deliverable for Proposal Subtask 4 documenting the field event is completed by providing field notes, MAGS preliminary data, groundwater sampling forms, laboratory reports, and management of IDW associated with Subtask 4.
June 26, 2024 Terracon Proposal No. PHB237027
Subtask 5 – PFAS Screening and IDW Management
Terracon proposes to collect groundwater samples during this mobilization to address the recommendations Terracon made in the November 30, 2023, Markhams Maytag Laundry PFAS Notification Deliverable. Terracon recommends collecting groundwater samples from four of the five newly installed wells (MW021, MW022, MW023, and MW025), MW018, and MW019 for PFAS analysis for the onsite monitoring wells (Exhibit 2). Once the selected monitoring wells are approved by the FDEP, Terracon will complete the PFAS sampling in general accordance with the Terracon State of Florida Sampling Standard Operating Procedure PFAS Version 2.0, dated January 30, 2024. The groundwater samples will be submitted overnight delivery to the FDEP Central Laboratory (DEP SOP LC-001-3).
This includes the electronic mail deliverable due to the FDEP in 48-hours after Terracon receives the PFAS analytical results. This will include preparation of a figure with the well locations and PFAS results, photograph documentation, laboratory data, PFAS checklist, chain-of custody, and signed waste manifest for VOC and PFAS IDW associated with Subtasks 4 and 5.
Quality Assurance/Quality Control (QA/QC) Decontamination will be conducted between each boring, well installation and sample collection. QA/QC samples will be collected as follows:
Equipment Blanks (EQB): One (1) EQB during groundwater sampling activities for a total of one
(1) sample.
Field Blanks: one (1) sample per groundwater sampling event for a total of one (1) sample.
The QA/QC samples will be packaged under proper chain-of-custody (COC) procedures and sent overnight delivery to the FDEP laboratory in Tallahassee, Florida for analysis.
Subtask 2 and 3 IDW Management and Disposal Soil and liquid IDW derived from soil cuttings, well development and installation, and decontamination during the Subtask 2 and 3 investigations will be contained in DOT approved 55-gallon drums and temporarily stored onsite until the waste characterization sample results are completed. The liquid IDW will be analyzed for the full list Volatile Organic Compounds (VOCs) and PFAS while the soil IDW will be analyzed for toxicity characteristic leaching procedure (TCLP) extraction for the full list VOCs, TCLP eight
(8) RCRA metals. Terracon has PFAS analytical data for the groundwater sampled at the site in November 2023. Assuming the waste is non-hazardous, the drums will be transported to an approved facility for disposal or treatment. Terracon estimates fifteen (15) drums of soil and ten (10) drums of liquid for disposal. Terracon will provide oversight of the IDW pickup, and sign waste manifests as an agent of FDEP. Transport will be conducted by Republic Services and disposal at a Subtitle C landfill.
Subtask 4 and 5 IDW Management and Disposal Liquid IDW derived from purge water during the Subtask 4 and 5 investigations will be contained in DOT approved 55-gallon drums and temporarily stored onsite until the waste characterization sample results for the full list VOCs and PFAS can be evaluated. Assuming the waste is non-hazardous, the drum(s) will be transported to an approved facility for disposal or treatment. Estimate two (2) drums of liquid.
Since the EPA recently came out with the Final Rule for five individual PFAS COCs (GenX chemicals) and is setting a Hazard Index for two or more of four PFAS as a mixture, many of the disposal facilities were hesitant in giving out a cost for disposal of PFAS impacted media due to the current uncertainty. Terracon will ask for three bids from IDW disposal subcontractors, while the Subtask 5 field work is being completed, using previous groundwater PFAS analytical data from the site and complete a WCD/Task Assignment Change Order (TACO) for the low bid.
June 26, 2024 Terracon Proposal No. PHB237027
Subtask 6 – Supplemental Site Assessment Report (SSAR)
Terracon will prepare a Supplemental Site Assessment Report that includes a summary of the sampling methodology and analytical results. The SSAR will include tables and figures summarizing the groundwater results in comparison to FDEP’s Cleanup Target Levels (groundwater). Exhibits will be provided to illustrate the site layout, lithology, sample locations and results by matrix.
SCHEDULE
The goal of the report deliverable is to provide any data obtained by the end of February 2025 and is dependent of driller availability and FDEP laboratory analytical turn around.
CLOSING REMARKS
Terracon services will be conducted in accordance with the terms and conditions, contained within DEP Waste Cleanup Program (WCP) Contract No. HW687, executed January 5, 2022. It is understood that a Task Assignment will be issued to authorize us to perform this work.
