On 10 March 2026, Geosyntec confirmed that a Site Access Agreement (SAA) was executed with AJS Realty LLC on 08 August 2023, as the real property owner of the Jack Sheehan’s Drive-in Cleaners #2 (ERIC_5521) located at the above address. The Palm Beach County property appraiser database indicates the property owner changed ownership on 12 August 2025 to 831 Belvedere Road LLC. As a result, Geosyntec believes that a new Site Access Agreement is required.
A description of task breakdown for Site assessment field activities and report preparation is provided below.
Subtask 1 – Project Management
Melissa Shook, Professional, will be the project manager (PM). Her labor hours are directly computed.
Subtask 2 – Monitoring Well Installations, Surveying and Groundwater Sampling
Ms. Sarah Aduwa 25 March 2026
Exterior monitoring well installations: Geosyntec personnel will mobilize to the Site to observe the installation of up to five exterior monitoring wells (MW001, M002, MW003, MW004, and MW005) screened from 10 to 20 ft BLS. A utility survey will be conducted to verify that monitoring well locations do not conflict with underground utility location. Additionally, due to the location of MW005 (located in the right of way), a temporary traffic plan will be required.
As recommended by FDEP in an email dated 11 August 2025, the monitoring wells will be installed upgradient, cross-gradient, and downgradient of the facility using DPT drilling methods.
The wells will be constructed of Schedule 40 PVC pipe with 10 ft of pre-packed 0.010-in. slotted well screen, filter pack consisting of 20/30 silica sand will be placed to 2-ft above the top of the screen, and at least a 2-ft thick, fine sand (30/65 silica sand) seal will be placed above the filter pack. The remaining annular space will be grouted to land surface with Portland Cement Type I/II and monitoring wells will be completed at the surface with a flush-mount 8-in. manhole and a locking monitoring well cap.
The monitoring wells will be developed until the discharge is relatively sediment-free. Geosyntec will demobilize to allow 24 hours between development and sampling.
Monitoring Well Survey: This subtask includes costs for a Florida professional land surveyor to survey the five newly installed monitoring wells.
Groundwater sampling: Geosyntec personnel will remobilize to the site to measure depth to groundwater and groundwater samples from the five Site monitoring wells (MW001, MW002, MW003, MW004, and MW005). The monitoring wells will be sampled using a peristaltic pump in general accordance with the FDEP Standard Operating Procedures (SOPs). The samples will be stored on ice and shipped to a NELAP-accredited, fixed-based analytical laboratory under chain of custody for analysis under standard turnaround times. The groundwater samples will be analyzed for volatile organic carbons by EPA Method 8260 (full list).
IDW Management: Investigation derived waste (IDW) purge water generated during well installation and sampling activities will be placed in a 55-gallon drum and manifested for off-Site disposal. For this cost estimate, we have assumed the IDW be impacted by PFAS and Geosyntec will submit a WCD or TACO to the FDEP for any change of scope. Bid costs prepared by the waste disposal companies are included in Attachment A. Due to the quantity of drums anticipated and Site logistics, we have assumed field oversight of the drum pickup is necessary.
Field Deliverable and Draft Summary Figure: Geosyntec has included costs to generate three figures (final site map, groundwater results figure, groundwater elevation contour figure). The deliverable will include the final surveyor report, monitoring well installation field forms, and groundwater sampling forms.
Subtask 2 activities will be performed by one Geosyntec staff member assuming two days for drilling oversight and one day for well sampling, and close out tasks at the Site. We have assumed up to 10 hours per field day not including round trip mobilizations.
Subtask 3 – Site Assessment Report
Geosyntec will prepare and submit a Site Assessment Report (SAR) documenting assessment activities and summarizing analytical results. This subtask includes costs to generate a SAR that summarizes the analytical results of the assessment activities outlined above and complies with the requirements of Chapter 62-780.600, FAC. Our cost estimate includes the following figures:
USGS Site Topographic Map
Potable Well Survey
Site Layout
Site Utility Map
Lithologic Cross Section (1 transect)
Summary of Soil Analytical Results from 0-0.5, 0.5-2, 2-4, 4-6, and 6-8 ft BLS intervals until the water table (assuming DTW at 8 ft BLS, 5 figures)
Summary of DPT Groundwater Sampling Analytical Results
Summary of Groundwater Elevations
Summary of Monitoring Well Analytical Results
The SAR will include recommendations for remediation, further assessment, or closure based on the data. The SAR will be signed/sealed by a FL professional geologist or engineer.
Subtask 4 – PFAS Screening
Geosyntec will conduct PFAS screening activities concurrently with those described in Subtask 2.
Each well sampled as part of Subtask 2 (MW001, MW002, MW003, MW004, and MW005) is proposed to be sampled for PFAS to evaluate PFAS impacts. PFAS samples will be collected using low-flow purging via a peristaltic pump in accordance with draft SOPs for PFAS sampling developed by FDEP and the SOPs for PFAS sampling prepared by Geosyntec for use at FDEP’s Site Investigation Section firefighting training facilities. PFAS-free water will used to decontaminate the water level meter between sampling wells. Supplemental labor for PFAS field handling and sampling that will be performed in concert with the groundwater sampling in Subtask 2 has been included in the LOE.
Geosyntec understands that the FDEP laboratory will analyze the five groundwater samples for PFAS constituents, and the FDEP will ship the sampling kits to Geosyntec. Geosyntec anticipates shipping the samples to the FDEP laboratory using return shipping labels provided by the laboratory with the sampling kits. The PFAS analytical results will be presented within the annual groundwater monitoring report included in Subtask 3.
Geosyntec will prepare an email deliverable with attachments that include: a figure showing PFAS groundwater results (PFOA, PFOS, and their combined concentrations for each well, without isoconcentration lines), general groundwater flow direction, the certified analytical laboratory report, equipment calibration logs, and groundwater sampling forms. This email will be submitted to the FDEP within two working days after receiving the final certified laboratory report from the FDEP Laboratory. Geosyntec expects the final laboratory report to be emailed to Melissa Shook (melissa.shook@geosyntec.com) by the FDEP Laboratory. The completed email deliverable will be submitted to Sarah Aduwa, James Breck Dalton, Safa Shariat, Jenifer Farrel, Mubeen Darji, and Doris Brown.

RE: Utility Locating Services – Jack Sheehan #2, WPB, Florida
Ms. Shook, GeoTek Services is pleased to respond to your request for Utility Locating services at Jack Sheehan #2 located in West Palm Beach, Florida. We will scan the area shown in your PDF drawing with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as vehicles, structures, obstructions. We will scan as close as reasonably possible to fixed objects such as curbs, landscaping, or obstructions, etc. Our deliverable will be marking paint or flags indicating the horizontal location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. We also request all grassy areas to be mowed prior to scanning. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Good morning Larry, In light of the update you recently provided about LES, I was hoping you might be able to prepare an updated IDW disposal quote (previous version attached) for use as part of future proposals. Please see the below list of scenarios I need updated waste disposal costs for through the remainder of 2025. We will reach out again at the beginning of 2026 for updated rates.
Scenario 1
FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has not been investigated for at the site. $260.00 per drum solid or liquid less than 50 miles- Add $95 per additional 50 miles
Scenario 2
FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the laboratory's method detection limit but is below the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums- $380.00
Scenario 3
FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums -$380.00
Scenario 4
FDEP PRP site where VOA concentrations are below hazardous levels for both soils and groundwater. $260.00 per drum solid or liquid less than 50 miles- Add $95 per each additional 50 miles
Please let me know if you have any questions or if you need anything else from me to support your efforts on this. If any of the above scenarios requires additional analytical results beyond those described, please let me know so I can include those in the overall cost evaluation. I really appreciate your help with this!
