This Task Assignment authorizes Geosyntec to assess a presumptive remedy for soil in accordance with the Geosyntec scope of work, cost estimate, and schedule received on May 16, 2023.
Geosyntec confirmed on May 16, 2023, that Subtask 1 consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.
Subtask 2: This subtask includes costs associated with the preparing for and conducting an on- Site meeting with the Site owner to discuss presumptive remedies and Site closure preferences.
Previous interactions with the owner have shown that she is more responsive with in-person meetings than via telephone calls. A site meeting will also enable Geosyntec engineers to visually inspect the interior conditions at the site left after renovations were begun by the previous tenant. Geosyntec will discuss soil vapor extraction (SVE), including the installation of interior wells, and possible excavation of affected soil as presumptive remedies with the Site owner. Geosyntec will also schedule a teleconference with FDEP following the meeting with the Site owner.
Subtask 3: Geosyntec will schedule and conduct a 1-hour presumptive remedy teleconference with FDEP to agree upon a path forward for an Interim Source Removal (ISR). Following the teleconference, Geosyntec will submit a summary email with meeting minutes to FDEP.
The FDEP Project Manager confirmed site access on May 17, 2023.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $3,100.15 with 19 labor hours.

Subject: Scope of Work and CEF/LOE – Presumptive Remedy Assessment Former Trim-N-Tidy #1 1212 South Florida Avenue Lakeland, Florida 33803 FDEP Site ID No. ERIC_5391
Dear Ms. Plyler:
Geosyntec Consultants, Inc. (Geosyntec) has prepared this proposed Scope of Work (SOW), Level of Effort (LOE), and Cost Estimating Form (CEF) to assess a presumptive remedy for soil at the Former Trim-N-Tidy Site located at 1212 South Florida Avenue, Lakeland, Florida (ERIC_5391;
“Site”) based on input from FDEP during an 18 October 2022 teleconference. This SOW includes
(i) conducting a presumptive remedy assessment meeting with the Site owner, and (ii) conducting a presumptive remedy assessment and recommendation teleconference with the Florida Department of Environmental Protection (FDEP). A description of task breakdown for field activities and report preparation is provided below.
Subtask 1 – Project Management
Costs associated with this subtask are directly computed.
Subtask 2 – Presumptive Remedy Discussion with Site Owner
This subtask includes costs associated with the preparing for and conducting an on-Site meeting with the Site owner to discuss presumptive remedies and Site closure preferences. Based on input from the FDEP during an 18 October 2022 teleconference, Geosyntec will discuss soil vapor extraction (SVE), including the installation of interior wells, and possible excavation of affected soil as presumptive remedies with the Site owner.
This subtask includes effort to mobilize to the site and conduct a meeting with the Site owner to discuss the presumptive remedies and ask for her feedback. Previous interactions with the owner have shown that she is more responsive with in-person meetings than via telephone calls. A site meeting will also enable Geosyntec engineers to visually inspect the interior conditions at the site left after renovations were begun by the previous tenant. Effort has also been included to discuss
Ms. Brittany Plyler May 16, 2023
Trim-N-Tidy (ERIC_5391)_HW683 SOW-LOE_Presumptive Remedy_Rev1_2023-05 closure options for the Site which will be used to guide remediation goals for the Site. Geosyntec will schedule a teleconference with FDEP following the meeting with the Site owner.
Subtask 3 – Presumptive Remedy Discussion with FDEP
Following completion of activities under Subtask 2, Geosyntec will schedule and conduct a 1-hour presumptive remedy teleconference with FDEP to agree upon a path forward for an Interim Source Removal (ISR). Following the teleconference, Geosyntec will submit a summary email with meeting minutes to FDEP. Geosyntec anticipates the preparation of an ISR Work Plan will be performed under a separate Task Assignment.
Invoice and Deliverable Schedule Geosyntec proposes the following deliverables schedule.
Subtask Deliverable Deliverable Due Date Invoice Amount
Email Summary and Meeting Minutes from Owner Site Meeting
31 July 2023 $1,374.11
