From: Sarah Aduwa, Project Manager Date: November 15, 2024 Subject: Negotiation Summary

HW683_DC031C

This Task Assignment authorizes Geosyntec to conduct Well Abandonment activities at Pelican Dry Cleaners in accordance with Geosyntec scope of work, cost estimate and schedule received on November 14, 2024. The total cost is $6,517.62.

Nick Barnes is the project manager, her hours are directly computed.

Subtask 2, 3 – Monitoring Well Abandonment and Reporting Geosyntec personnel will mobilize to the Site to observe the abandonment of four groundwater monitoring wells, five MAGS wells, and one vacuum monitoring point (VMP). Well Abandonment permit applications for the monitoring wells will be completed and submitted to the Southwest Florida Water Management District (SWFWMD) by a State licensed water well driller.

As part of well abandonment activities, Geosyntec will observe the recommended subcontracted driller(Environmental Drilling Service) conduct well abandonment activities in accordance with SWFWMD guidelines. The driller will tremie grout the wells in place from the bottom of the well, within the well screen interval, until grout displaces and infills the annular space up to land surface. The driller will remove the concrete pads and the well vaults. The ground surface will then be patched with neat cement grout flush to surrounding grade if the well was located in concrete or asphalt patch if the well was located in asphalt. Concrete, asphalt, and other debris (e.g. well vault, flush-mount manhole cover) generated during well abandonment will be disposed of by the subcontractor driller offsite in general trash. Photographs of abandoned well locations will be collected in the field.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task assigner. The final cost is $6,517.62 with 24 labor hours.

Subject: Scope of Work and Level of Effort for Well Abandonments Pelican Dry Cleaners 2141 Drew Street Clearwater, Pinellas County, Florida FDEP Facility ID No. ERIC_5292

Dear Ms. Aduwa, Please find attached Geosyntec’s Scope of Work and proposed Level of Effort (LOE) for well abandonment activities at the Pelican Dry Cleaners (Site) located in Clearwater, Florida. A summary of the costs associated with the scope of work described below is included with the attached LOE and subcontractor evaluation which compares the associated driller costs from three quotations.

Subtask 1 – Project Management

Nick Barnes will be the Geosyntec Project Manager. His labor hours are directly computed.

Subtask 2 –Monitoring Well Abandonment and Reporting

Geosyntec personnel will mobilize to the Site to observe the abandonment of four groundwater monitoring wells, five MAGS wells, and one vacuum monitoring point (VMP). Well abandonment permit applications for the monitoring wells will be completed and submitted to the Southwest Florida Water Management District (SWFWMD) by a State licensed water well driller.

Geosyntec requested quotes from three drillers to complete well abandonment activities. The quotes are included in this proposal and an evaluation of the received quotes is attached. As part of well abandonment activities, Geosyntec will observe the recommended subcontracted driller (Environmental Drilling Service) conduct well abandonment activities in accordance with SWFWMD guidelines. The driller will tremie grout the wells in place from the bottom of the well, within the well screen interval, until grout displaces and infills the annular space up to land surface.

The driller will remove the concrete pads and the well vaults. The ground surface will then be patched with neat cement grout flush to surrounding grade if the well was located in concrete or

Concrete, asphalt, and other debris (e.g. well vault, flush-mount manhole cover) generated during well abandonment will be disposed of by the subcontractor driller offsite in general trash. Photographs of abandoned well locations will be collected in the field.

Upon completion of all site activities, Geosyntec will prepare a Well Abandonment Report for submittal to the Florida Department of Environmental Protection (FDEP). The report will summarize completed well abandonment activities and include an updated well location figure, well abandonment logs, SWFWMD well abandonment permits, and a photographic log.

Invoice and Deliverable Schedule:

Geosyntec proposes the following revised Invoice and Deliverable schedule. The schedule below assumes two weeks for approval of the Task Assignment (TA) approval by the FDEP:
