This Task Assignment authorizes Geosyntec to conduct Annual Groundwater/soil gas monitoring and PFAs sampling activities at Former Cole Drycleaner site in accordance with Geosyntec’s scope of work, cost estimate and schedule received on February 26, 2026.

Blaine Dawson is the Project Manager.

Subtask 2 and 3 Monitoring wells MW014, MW015, MW021-MW029, MW031, and MW034 will be sampled and analyzed for volatile organic halocarbons with monitoring wells MW014, MW015, and MW021-MW029 also being analyzed for dissolved gases.

Soil gas monitoring will also be conducted to assess potential methane and hydrogen sulfide generation beneath the former dry-cleaning building. Air samples will be collected from SGP-1, SGP-2, and SV-1 (soil vent fan) and screened for methane, oxygen, and hydrogen sulfide using a multi-RAE.

Geosyntec will prepare an Annual Groundwater Monitoring Report documenting the results of the depth to groundwater measurements and groundwater analytical results.

Subtask 4 Geosyntec will conduct PFAS groundwater monitoring activities concurrently.

Exceedances of the provisional groundwater cleanup target levels were observed at monitoring wells MW001, MW028, MW029, and MW034, with the summation of PFOA and PFOS exceeding 300 nanograms per liter (ng/L) at monitoring wells MW001 (348 ng/L) and MW028 (392 (ng/L).

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $19,881.92 with 100 labor hours.

Subject: Limited Annual Groundwater and Soil Gas Monitoring

Scope of Work and Level of Effort Former Coles Drycleaning

791 3rd Street North, Jacksonville Beach, Florida FDEP Facility ID ERIC_4608

Dear Ms. Aduwa, Geosyntec Consultants, Inc. (Geosyntec) has prepared this statement of work (SOW), Cost Estimate Form (CEF), and Level of Effort (LOE) for conducting limited annual groundwater and soil gas monitoring activities at the Former Coles Drycleaning Site (Site) located in Jacksonville Beach, Florida. A copy of the CEF and LOE is provided as Attachment A. The monitoring activities in this SOW were recommended in the March 2025 Annual Groundwater Monitoring Report and discussed during Geosyntec’s annual site review meeting with FDEP on April 29, 2025.

On 4 February 2026, Geosyntec confirmed that a Site Access Agreement (SAA) was executed with No. 3 Commercial, LLC on 31 October 2022, as the real property owner of the Former Coles Cleaners Site (ERIC_4608) located at the above address. The Duval County property appraiser database indicates the property owner since 5 November 2005 is “No. 3 Commercial, LLC,” and that the property has not changed ownership since then. As a result, Geosyntec believes the current SAA remains in force.

Subtask 1 – Project Management

Blaine Dawson, Senior Geologist, will be the project manager (PM), and his PM hours are directly computed.

Subtask 2 –Groundwater Monitoring

Annual groundwater monitoring is tentatively scheduled for March 2026 and will be completed in accordance with Table 1. Monitoring wells MW014, MW015, MW021-MW029, MW031, and MW034 will be sampled and analyzed for volatile organic halocarbons with monitoring wells

ERIC_4608 - Former Coles Drycleaning - Limited Annual Monitoring SOW_20266

MW014, MW015, and MW021-MW029 also being analyzed for dissolved gases. Groundwater samples will be stored on ice and shipped to the analytical laboratory under chain of custody for analysis under standard turnaround times.

Investigation derived waste (IDW) purge water generated during sampling activities will be placed in a 55-gallon drum and manifested for off-Site disposal. The IDW will be staged in the rear of the facility. Based on the historical detections of per- and polyfluoroalkyl substances (PFAS) in groundwater, IDW purge water generated during monitoring activities will need to be treated as PFAS impacted waste. Bid costs prepared by the waste disposal companies are included in Attachment A.

Subtask 3 – Groundwater Monitoring Report

Geosyntec will prepare an Annual Groundwater Monitoring Report documenting the results of the depth to groundwater measurements and groundwater analytical results. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the groundwater monitoring event. The report will be signed and sealed by a Florida-licensed professional geologist. A final signed waste manifest for IDW transport and disposal will also be included in the Groundwater Monitoring Report.

Subtask 4 – PFAS Sampling and Reporting

Geosyntec will conduct PFAS groundwater monitoring activities concurrently with those described in Subtask 2. These sampling activities are proposed to evaluate the PFAS groundwater impacts observed during initial PFAS screening activities in November 2022. Due to the limited nature of the initial screening efforts (five wells sampled), an expanded PFAS sampling plan has been included in Subtask 2 that includes sample collection from each of the 13 monitoring wells sampled as part of Subtask 2 (MW014, MW015, MW021-MW029, MW031, and MW034) to evaluate delineation of the observed groundwater impacts.

PFAS samples will be collected using low-flow purging via a peristaltic pump in accordance with draft SOPs for PFAS sampling developed by FDEP and the SOPs for PFAS sampling prepared by Geosyntec for use at FDEP’s Site Investigation Section firefighting training facilities. PFAS free

ERIC_4608 - Former Coles Drycleaning - Limited Annual Monitoring SOW_20266 water will used to decontaminate the water level meter between sampling wells. Supplemental labor for PFAS field handling and sampling that will be performed in concert with the groundwater sampling in Subtask 2 has been included in the LOE.

Geosyntec understands that the FDEP laboratory will analyze the 13 groundwater samples for PFAS constituents, and the FDEP will ship the sampling kits to Geosyntec. Based on the location of the Site, Geosyntec anticipates shipping the samples to the FDEP laboratory. Geosyntec assumes that the FDEP laboratory will provide return shipping labels with the sampling kits.

The PFAS analytical results will be presented within the annual groundwater monitoring report included in Subtask 3.

Yes- if it is sampled for and has any hit it will have to go to a hazardous waste facility.

Good morning Larry, In light of the update you recently provided about LES, I was hoping you might be able to prepare an updated IDW disposal quote (previous version attached) for use as part of future proposals. Please see the below list of scenarios I need updated waste disposal costs for through the remainder of 2025. We will reach out again at the beginning of 2026 for updated rates.

Scenario 1

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has not been investigated for at the site. $260.00 per drum solid or liquid less than 50 miles- Add $95 per additional 50 miles

Scenario 2

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the laboratory's method detection limit but is below the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums- $380.00

Scenario 3

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums -$380.00

Scenario 4

FDEP PRP site where VOA concentrations are below hazardous levels for both soils and groundwater. $260.00 per drum solid or liquid less than 50 miles- Add $95 per each additional 50 miles

Please let me know if you have any questions or if you need anything else from me to support your efforts on this. If any of the above scenarios requires additional analytical results beyond those described, please let me know so I can include those in the overall cost evaluation. I really appreciate your help with this!

Re: Non-Hazardous Drum Disposal of PFOS/PFAS Impacted Materials

Mr. Barnes:

The price for the transportation and disposal (T&D) of 55-gallon drums of non-hazardous soil, mud, and water is listed below:

Impacted with Non-Hazardous Petroleum or Chlorinated Solvents:

Description Rate Unit

T&D - Non-Haz Soil $ 110.00 Per Drum

T&D - Non-Haz Mud/Water $ 125.00 Per Drum

Impacted with Non-Hazardous PFAS/PFOS:

Description Rate Unit

Profile Fee (Charged once per site) $ 350.00 Per Site

T&D - Non-Haz PFOS/PFAS Impacted Soil $ 195.00 Per Drum

T&D - Non-Haz PFOS/PFAS Impacted Mud/Water $ 195.00 Per Drum

Dry cleaning sites will require 8260-VOH lab analysis and PFAS sites will require the full FDEP lab analysis for IDW waste characterization.

This quote is valid for 120 days.

Respectfully, Robert Donado Erwin Remediation, Inc.
