Subject: Supplemental Site Assessment Scope of Work and Level of Effort Millionaires Dry Cleaners 6313 Miramar Parkway Miramar, Florida 33023 FDEP Facility ID ERIC_4233

Dear Ms. Plyler, Geosyntec has prepared this Scope of Work (SOW) and Cost Effort Form/Level of Effort (CEF/LOE) to further assess the contaminated groundwater within the shallow and intermediate aquifers in the vicinity of the Millionaires Dry Cleaners Site (the “Site”) to advance the Site to regulatory closure. The site is located at 6313 Miramar Parkway, Miramar, Florida (ERIC_4233).

The development of this scope of work was conceptualized with FDEP staff during a teleconference held on 2 June 2023.

Geosyntec obtained three bids from licensed and qualified contractors for drilling, surveying, utility location, and transportation and disposal of investigation-derived waste (IDW). A cost comparison is provided in Attachment A. Based on the lowest cost and/or earliest availability among the qualified bidders, Geosyntec recommends the following subcontractors:

• Drilling and Well Installation – Earth Tech Drilling, Inc

• Monitoring Well Survey – Pulice Land Surveyors, Inc

• Utility Locate – Blood Hound, LLC

• IDW transportation and disposal – Erwin Remediation

Copies of the bids from each subcontractor can be found in Attachment B.

Ms. Brittany Plyler 26 October 2023

Subtask 1 – Project Management

Labor hours have been computed directly for Tori Hood as Project Manager.

Subtask 2 – Monitoring Well Installation, Survey and Semi-Annual Groundwater Sampling (Event 1)

This supplemental site assessment includes the installation of three monitoring wells (2 shallow and one intermediate) and two (2) semi-annual groundwater sampling events. The installation and survey of the monitoring wells will be performed under this task. The semi-annual groundwater sampling will take place during both this task and Task 3.

Geosyntec will submit an 811 public utility locate ticket in advance of the scheduled visit. Figure 1 illustrates the approximate locations for the monitoring well installations. Geosyntec personnel will be on-site to observe a private underground utility survey before the drilling activities are initiated. This geophysical investigation will employ ground-penetrating radar (GPR) and electromagnetic resonance (EMR) equipment. Once utilities are identified, their locations will be marked. The final locations for installation of the monitoring wells will be determined based on determining a safe distance from identified utility locations, if necessary. Blood Hound, LLC provided the most competitive bid and was therefore chosen for this task.

Geosyntec will mobilize to the Site to install 2 shallow monitoring wells and 1 intermediate monitoring well. The shallow monitoring wells will be screened from 3 to 13 feet Below Land Surface (ft BLS), while the intermediate monitoring well will be screened from 24 to 29 ft BLS.

The wells will be fitted with 1-inch diameter schedule-40 polyvinyl chloride (PVC) pipe. The shallow wells will have a 10-foot-long 0.010-inch pre-packed screened section, and the intermediate well will have a 5-foot-long 0.010-inch pre-packed screened section. The annular space around each well will be packed with a sand filter (20/30 silica sand). Above this filter pack, a 1-foot-thick fine sand (30/65) transition seal will be placed. The remaining annulus up to the surface will be filled with neat cement grout. Each well will be developed until the purged water becomes clear. Both the soil cuttings and developed water will be stored in 55-gallon steel drums.

The soil will be logged, and the lithology will be recorded following ASTM Guidance D-2488-00.

Based on the bids received from vendors, Earth Tech Drilling, Inc. was selected to conduct the drilling and monitoring well installation.

This subtask also accounts for the costs of hiring a Florida professional land surveyor (PLS) to survey the 3 newly installed monitoring wells. Additionally, the elevation of the top of casing of existing monitoring well MW008 will be verified. Costs for Geosyntec to oversee the survey during a separate mobilization have also been included. Pulice Land Surveyors are recommended for this role.

Subtask 2 also includes the mobilization of Geosyntec personnel to the Site to conduct groundwater sampling of 14 monitoring wells, including the 3 newly-installed monitoring wells, as depicted in Figure 1. In general accordance with FDEP Standard Operating Procedures (SOPs), the wells will be sampled using a peristaltic pump. The samples will be stored on ice and sent to a NELAP-approved fixed-base analytical laboratory under a chain of custody for analysis, with standard turnaround times. The groundwater samples will be assessed for volatile organic compounds (VOCs) using EPA Method 8260B. Groundwater depths will be measured in the 14 monitoring wells prior to sampling to generate groundwater elevation contour maps.

The handling of IDW and its transportation to a disposal facility is incorporated in this task. Erwin Remediation will undertake this responsibility. As discussed with FDEP on 2 June 2023, this subtask will encompass the drilling IDW and purge water generated during semi-annual groundwater sampling.

Geosyntec will prepare a field deliverable as an approval milestone. The deliverable will include copies of relevant field forms such as lithologic logs, well construction logs, as-built markups, groundwater sampling forms, equipment calibration records, IDW logs and manifests, analytical laboratory report, and updated tables and figures (site layout, groundwater elevation contours, analytical results).

Subtask 3 – Semi-Annual Groundwater Sampling (Event 2)

Subtask 3 includes the mobilization of Geosyntec personnel to the Site to conduct groundwater sampling of up to 9 monitoring wells which may exceed groundwater cleanup target levels (GCTLs) for select VOCs. Based on the latest VOC laboratory results, the approximate number of wells to be sampled is between 7 to 9 monitoring wells which includes the 3 newly installed monitoring wells. In general accordance with FDEP SOPs, the wells will be sampled using a peristaltic pump. The samples will be stored on ice and sent to a NELAP-approved fixed-base analytical laboratory under a chain of custody for analysis, with standard turnaround times. The groundwater samples will be assessed for VOCs using EPA Method 8260B. Groundwater depths will be measured in the 14 monitoring wells prior to sampling to generate groundwater elevation contour maps.

The handling of IDW and its transportation to a disposal facility is incorporated in this task. Erwin Remediation will dispose of 2 drums of purge water generated during the second semi-annual groundwater sampling.

Following the completion of the second semi-annual groundwater sampling as described in this subtask, Geosyntec will compile and review the analytical data obtained during Subtask 2 and 3.

This subtask includes the costs associated with the preparation of GWMR summarizing both semi-annual monitoring events. The report will provide a summary of the analytical data obtained during the first and second groundwater sampling events, emphasizing key findings, trends, and pertinent observations. It will include VOC analyses using EPA Method 8260B. The GWMR report will be submitted to FDEP and certified by a Florida Professional Geologist or Engineer.

SCOPE OF WORK

Underground Utility: We understand the scope of work on this project is to search for underground utilities in a 50SF area where three (3) Monitoring Well locations will be. We will attempt to trace any utilities for which structures are visible from the work area. Utilities will be marked on the surface using paint, pin flags, or other appropriate means. The client will be responsible for providing drawings or notifying GPRS of any utilities entering the work area for which there are no apparent surface features or structures visible from the work area. To avoid additional charges, the areas should be laid out, marked, and cleared of obstructions before our arrival. Please visit www.simspec.org for an overview of our industryleading best practices that will be applied to this project.

The client will need to provide access to applicable site plans, asbuilts, drawings prior to the start of the field work and a site contact to direct our onsite Technician to the scan area and be able to answer sitespecific scope questions as needed.

GPRS is not responsible for housekeeping. Any debris, equipment, or other obstructions in the area at the time of scanning could potentially block out needed data.• A thorough utility search can only be completed if GPRS is given access to all utility structures, interior and exterior. This service is never a replacement for the use of the state One Call system (811).

All of our technicians have OSHA10 safety training or greater. Sitespecific safety training is not included in this quote. Please notify us if this project requires additional safety training.

EQUIPMENT

Underground Scanning GPR Antenna. This GPR Antenna uses frequencies ranging from 250 MHz to 450 MHz and is mounted in a stroller frame that rolls over the surface. Data is displayed on a screen and marked in the field in realtime. The surface needs to be reasonably smooth and unobstructed to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the efficacy of GPR.

The total effective scan depth can be as much as 8’ or more with this antenna but can vary widely depending on the soil conditions and composition. Some soil types, such as clay, may limit maximum depths to 3’ or less. As depth increases, targets must be larger to be detected, and nonmetallic targets can be challenging to locate. The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

Electromagnetic Pipe Locator. This receiver can passively detect the signals from live AC power or radio signals traveling along some conductive utilities. Operators can connect a transmitter directly to accessible metallic pipes, risers, or tracer wires to generate a current at a specific frequency. The receiver can then detect the resulting signal along the pipe or tracer wire. Various factors may impact this device's effectiveness, including (but not limited to) access to the utility, conductivity, grounding, and interference from other utilities.

The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

Traceable Rodder. The rodder consists of a copper wire encased in fiberglass. This device is pushed through a pipe with direct access, such as a sewer line at a cleanout or a storm drain catch basin. Operators then induce a current on the wire and trace the signal from the surface. The maximum traceable depth is 10’ depending on the soil conditions, and the maximum distance is 200’. Inserting the rodder into deeper pipes within manholes may not be feasible depending on site conditions. GPRS will not access electrical conduits. The signal is not detectable through metallic pipes. For more information, please visit: Link

GPS. This handheld unit offers accuracy down to 4 inches; however, the accuracy achieved will depend on the satellite environment at the time of collection and is not considered surveygrade. Features can be collected as points, lines, or areas and then exported as a KML/ KMZ or overlaid on a CAD drawing.
