Subject: Negotiation Summary
HW683 DC008D
This Task Assignment authorizes Geosyntec to conduct annual groundwater monitoring and PARM Well Installation.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2 – Monitoring Well Installations:
Subtask 2 includes costs for Geosyntec personnel to mobilize to the Site and oversee the installation of two (2) water-table monitoring wells (MW002R and MW012) to a depth of 42 feet below land surface (ft BLS) at the locations illustrated on Figure 1. This depth interval is consistent with historical monitoring wells at the Site and depth to water measurements collected in July 2021 indicated groundwater is approximately 34 to 37 ft BLS. The monitoring wells will be installed using hollow-stem auger drilling method and constructed of 2-inch
(in) diameter Schedule-40 polyvinyl chloride (PVC) pipe with 10-feet (ft) of 0.010-in standard factory-slotted screen. Filter packs will consist of ten (10) ft of 20/30 silica sand sealed with a minimum of 2-ft of 30/65 sand. The remaining borehole will be grouted to land surface. The monitoring wells will be completed with 8” diameter traffic rated steel manholes set in 2-ft by 2-ft by 6-inch thick concrete pads and developed until visually free of particulates by a combination of pumping and surging. To prevent damage during Site redevelopment activities, four (4) 4-in steel bollards will be installed at the corner of each monitoring well pad and painted yellow for increased visibility.
Liquid investigation-derived waste (IDW) generated during monitoring well installation and groundwater sampling activities will be containerized in new, 55-gallon steel drums, profiled, manifested, and disposed of off-site by a licensed waste hauler.
Printed on recycled paper.
MEMORANDUM
Concord Cleaners #045 August 22, 2022 Page Two
Subtask 3 – Groundwater Sampling:
Subtask 3 includes costs for Geosyntec personnel to mobilize to the Site to conduct groundwater sampling activities of Site monitoring wells. Groundwater sampling activities will be conducted at least three (3) months following completion of well installation activities to allow sufficient time for equilibration with the surrounding aquifer. The monitoring wells will be sampled using a submersible pump in general accordance with FDEP Standard Operating Procedures (SOPs).
The samples will be stored on ice and shipped to a NELAP approved, fixed-based analytical laboratory under chain of custody for analysis under standard turnaround times. The groundwater samples will be analyzed for VOHs by EPA Method 8260.
Subtask 4 – Groundwater Monitoring Report and Site Rehabilitation Completion Report:
Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a Groundwater Monitoring Report documenting the groundwater assessment activities and summarizing analytical results. Should groundwater analytical results indicate closure criteria have been met, Geosyntec will also proceed with the preparation and submittal of a Site Rehabilitation Completion Report.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $24,108.39 with 99 labor hours.
Geosyntec may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.
Geosyntec and FDEP confirmed on 17 August 2022, that an Access Agreement with the current real property owner of the Concord Cleaners #045 (ERIC_4870) site has been executed.

Mr. Evan Miller Florida Department of Environmental Protection 2600 Blair Stone Road Tallahassee, FL 32399-2400
Subject: 2022 PARM Well Installation and Sampling Scope of Work/Level of Effort Concord Cleaners #045 1940-77 North Monroe Street Tallahassee, Leon, Florida FDEP Facility ID ERIC_4870
Dear Mr. Miller, Geosyntec Consultants has prepared this scope of work (SOW) and proposed Level of Effort (LOE) for conducting post active remediation monitoring (PARM) well installation and groundwater sampling activities at the Concord Cleaners #045 (Site) (Facility ID ERIC_4870) located in Tallahassee, Florida. This SOW and LOE includes:
Subtask 1 – Project Management
Subtask 2 – Monitoring Well Installations: Subtask 2 includes costs for Geosyntec personnel to mobilize to the Site and oversee the installation of two (2) water-table monitoring wells (MW002R and MW012) to a depth of 42 feet below land surface (ft BLS) at the locations illustrated on Figure 1. This depth interval is consistent with historical monitoring wells at the Site and depth to water measurements collected in July 2021 indicated groundwater is approximately 34 to 37 ft BLS. The monitoring wells will be installed using hollow-stem auger drilling method and constructed of 2-inch (in) diameter Schedule-40 polyvinyl chloride (PVC) pipe with 10-feet (ft) of 0.010-in standard factory-slotted screen. Filter packs will consist of ten
(10) ft of 20/30 silica sand sealed with a minimum of 2-ft of 30/65 sand. The remaining borehole will be grouted to land surface. The monitoring wells will be completed with 8” diameter traffic rated steel manholes set in 2-ft by 2-ft by 6-inch thick concrete pads and developed until visually free of particulates by a combination of pumping and surging. To prevent damage during Site redevelopment activities, four (4) 4-in steel bollards will be installed at the corner of each monitoring well pad and painted yellow for increased visibility.
mailto:Evan.E.Miller@FloridaDEP.gov
Geosyntec will subcontract with Meridian Surveying and Mapping, Inc. (Meridian) to survey the horizontal locations and top-of-casing elevations for the newly installed monitoring wells. The horizontal datum will be North American Datum 1983 Florida East Zone State Plane Coordinates (latitude/longitude), U.S. survey feet, and the vertical datum will be North American Vertical Datum 88, U.S. survey feet. Geosyntec will coordinate with Meridian to complete the work immediately following well installation activities.
Liquid investigation-derived waste (IDW) generated during monitoring well installation and groundwater sampling activities will be containerized in new, 55-gallon steel drums, profiled, manifested, and disposed of off-site by a licensed waste hauler. A non-hazardous profile has already been established for both soil and groundwater at the site with Hulls. It is assumed that Geosyntec will not need to be present during pickup of the IDW drums generated during site investigation activities; costs have not been included for a site visit for IDW disposal. Costs for IDW disposal are included under Subtask 4.
Subtask 3 – Groundwater Sampling: Subtask 3 includes costs for Geosyntec personnel to mobilize to the Site to conduct groundwater sampling activities of Site monitoring wells.
Subtask 4 – Groundwater Monitoring Report and Site Rehabilitation Completion Report: Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a Groundwater Monitoring Report documenting the groundwater assessment activities and summarizing analytical results. Should groundwater analytical results indicate closure criteria have been met, Geosyntec will also proceed with the preparation and submittal of a Site Rehabilitation Completion Report.
Invoice and Deliverable Schedule:
Geosyntec and FDEP confirmed on 17 August 2022, that an Access Agreement with the current real property owner of the Concord Cleaners #045 (ERIC_4870) site has been executed.

Good afternoon, I’m interested in procuring an estimate to locate (EM and GPR) at the Concord Cleaners #045 site located at 1940-77 N Monroe St, Tallahassee, Florida. The attached figure illustrates the two (2) monitoring well locations for utility locating (MW002R and MW012 [blue text]). We will need a maximum of a 20’x20’ grid around each location to make sure we have enough room to install the wells. The area surrounding each well is dirt or asphalt. Work will need to be completed on or before Thursday, 25 August 2022. If possible, I’d appreciate your bids back by COB Thursday (11 August 2022). Let me know if you have any questions.
Thank you, Blaine Dawson, PG (FL,TN)

RE: Utility Locating Services – Concord Cleaners, Tallahassee, Florida
Mr. Dawson, GeoTek Services is pleased to respond to your request for Utility Locating services at the Concord Cleaners located in Tallahassee, Florida. We will scan a 20’ x 20’ area around the two proposed bore locations shown in your PDF drawing with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings, or vehicles. We will scan as close as reasonably possible to fixed objects such as brush, landscaping, debris, etc.
