HW683_DC004E

This Task Assignment authorizes Geosyntec to conduct Groundwater Monitoring and PFAS sampling at Classic Cleaners site in accordance Geosyntec’s scope of work, cost estimate and schedule received on October 31, 2025.

Melissa Shook is the Project Manager.

Subtask 2 and 3 – Geosyntec personnel will mobilize to the Site to conduct annual Natural Attenuation Monitoring (NAM) of groundwater. Groundwater samples will be collected from MW002, MW006, and MW007. The samples will be sent to an analytical laboratory for volatile organic hydrocarbon analysis via EPA Method 8260. Geosyntec will prepare a comprehensive Natural Attenuation Monitoring (NAM) Report that details the findings and outcomes of the recent NAM event. The report will include well-organized tables presenting field and laboratory groundwater data, and figures illustrating groundwater flow directions and VOH results. A concise summary of all field activities conducted will be provided.

Subtask 4 - During Subtask 2 mobilization, Geosyntec personnel will collect samples from monitoring wells MW002, MW006, and MW007 for PFAS analysis. Samples will be preserved on ice and sent to the FDEP Laboratory following proper chain of custody procedures. Geosyntec will prepare an email deliverable with attachments that include: a figure showing PFAS groundwater results (PFOA, PFOS, and their combined concentrations for each well, without isoconcentration lines), the certified analytical laboratory report, equipment calibration logs, and groundwater sampling forms.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $8,76 .88 with 55 labor hours.

Subject: Scope of Work and Level of Effort Biennial NAM Groundwater Sampling Classic Cleaners (ERIC_4929) 3307 SE Federal Highway Stuart, Martin County, Florida 34997

Dear Ms. Sarah Aduwa:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW) and proposed Level of Effort (LOE) for annual Natural Attenuation Monitoring (NAM) at the Classic Cleaners (Site) (ERIC_4929) located in Stuart, Florida.

SITE ACCESS

On 3 October 2025, Geosyntec confirmed the Site property (Parcel ID 38-38-41-002-069-00010-

5) was sold by Indian Street Shoppes, LLC, to Indian Plaza LLC. There is no existing site access agreement with Indian Plaza LLC, therefore Geosyntec will obtain a New Site Access Agreement prior to conducting work on the property.

SCOPE OF WORK

Subtask 1: Project Management

Subtask 2: Biennial NAM

Geosyntec will secure a site access agreement with Indian Plaza LLC before any field operations begin. Once site access is obtained, Geosyntec personnel will mobilize to the Site to conduct annual Natural Attenuation Monitoring (NAM) of groundwater. Geosyntec will complete a groundwater monitoring event consistent with the recommendations from the September 2024 NAM Report. Groundwater samples will be collected from MW002, MW006, and MW007, as illustrated in the attached figure. The samples will be sent to an analytical laboratory for volatile organic hydrocarbon analysis via EPA Method 8260. During the groundwater sampling event, Classic Cleaners – 2025 NAM 17 October 2025

2025 Annual NAM_Classic Cleaners SOW.docx water level measurements will be collected from the following wells: MW002, MW003, MW004, MW006, MW007, MW008 located throughout the Site.

Purge water generated during sampling activities will be collected and stored in a single 55-gallon drum. It is anticipated that analytical data obtained from the monitoring wells will be sufficient for waste profiling purposes, thereby eliminating the need for a separate investigation-derived waste (IDW) sample.

Based on the cost estimates detailed in Attachment A for IDW disposal, Geosyntec has identified Erwin Remediation as the most cost-effective option for transportation and disposal services. It is further assumed that oversight during drum pickup will not be necessary.

Company Non-Hazardous PFAS Impacted Water (per 55-gal drum)

York Environmental $1,290.00

Erwin Remediation $195.00

Hulls $1,295.20

Subtask 3: Biennial NAM Report

Geosyntec will prepare a comprehensive Natural Attenuation Monitoring (NAM) Report that details the findings and outcomes of the recent NAM event. The report will include well-organized tables presenting field and laboratory groundwater data, and figures illustrating groundwater flow directions and VOH results. A concise summary of all field activities conducted will be provided.

Additionally, the NAM Report will contain the final, signed waste manifest documenting the transportation and disposal of Investigation-Derived Waste (IDW). The report will be reviewed, signed, and sealed by a Florida-licensed professional geologist or engineer to ensure regulatory compliance and technical accuracy. Prior to submission of the report, Geosyntec will coordinate with the Site owner to discuss potential conditional closure options.

2025 Annual NAM_Classic Cleaners SOW.docx

Subtask 4: PFAS Sampling and Field Summary

During Subtask 2 mobilization, Geosyntec personnel will collect samples from monitoring wells MW002, MW006, and MW007 for PFAS analysis.

This email will be submitted to the FDEP within two working days after receiving the final certified laboratory report from the FDEP Laboratory.

Geosyntec expects the final laboratory report to be emailed to Melissa Shook (melissa.shook@geosyntec.com) by the FDEP Laboratory. The completed email deliverable will be submitted to Sarah Aduwa.

Additionally, the PFAS results will also be incorporated into Subtask 3. Costs for analysis and integration into the Subtask 3 deliverable are covered under Subtask 4.

Good morning Larry, In light of the update you recently provided about LES, I was hoping you might be able to prepare an updated IDW disposal quote (previous version attached) for use as part of future proposals. Please see the below list of scenarios I need updated waste disposal costs for through the remainder of 2025. We will reach out again at the beginning of 2026 for updated rates.

Scenario 1

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has not been investigated for at the site. $260.00 per drum solid or liquid less than 50 miles- Add $95 per additional 50 miles

Scenario 2

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the laboratory's method detection limit but is below the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums- $380.00

Scenario 3

FDEP drycleaner site where VOC concentrations are below hazardous levels for both soils and groundwater, and PFAS has been detected at concentrations above the current provisional groundwater cleanup target level of 70 ng/L. $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums -$380.00

Scenario 4

FDEP PRP site where VOA concentrations are below hazardous levels for both soils and groundwater. $260.00 per drum solid or liquid less than 50 miles- Add $95 per each additional 50 miles

Please let me know if you have any questions or if you need anything else from me to support your efforts on this. If any of the above scenarios requires additional analytical results beyond those described, please let me know so I can include those in the overall cost evaluation. I really appreciate your help with this!

Thank you, Blaine Dawson, PG (FL,TN) Senior Geologist p g p g $915.20 per drum solid or liquid- Transportation per drum/load up to 10 drums- $380.00
