Task Assignment Summary

This Task Assignment (TA) DC025B is a contract modification document entered into between the State of Florida Department of Environmental Protection (FDEP) and Geosyntec Consultants for biennial groundwater monitoring services at the Oak Hill Cleaners site located at 7628-1 103rd Street, Jacksonville, Florida 32210 (FDEP Facility ID ERIC_4544). The contract is executed under the parent Contract Number HW683, with Sarah Aduwa serving as the FDEP Project Manager and Blaine Dawson as the Contractor's Project Manager. The TA is effective through June 19, 2024.

The task authorizes Geosyntec to conduct biennial natural attenuation monitoring of volatile organic halocarbons (VOHs) consisting of three subtasks: (1) Project Management at a calculated rate of 13% of direct labor costs; (2) Groundwater Sampling, including collection of 25 groundwater samples from monitoring wells using both passive diffusion bags and conventional peristaltic pump methods, with depth-to-water measurements from 31 wells and industrial waste water (IDW) disposal; and (3) preparation of an Annual & Biennial Groundwater Monitoring Report with Mann-Kendall statistical analysis and Florida-licensed professional geologist certification. The total TA value is $16,816.92 on a fixed unit rate basis, with payment scheduled as $10,015.31 upon submittal of groundwater sampling field notes and IDW manifest by March 1, 2024, and $6,801.61 upon submittal of the Groundwater Monitoring Report by April 19, 2024. IDW disposal costs through Hull's Environmental Services are estimated at $200.00 for drum handling.

HW683_DC025B

This Task Assignment authorizes Geosyntec to conduct biennial groundwater monitoring at the Oak Hill Cleaners site in accordance with the Geosyntec scope of work, cost estimate and schedule received on 10 January 2024.

Subtask 1: Project Management An automatic calculation of project management labor based upon 13% of direct labor cost in this proposal and includes fringe, overhead, other directs costs (ODC) and completions fees.

Subtask 2: Groundwater sampling Geosyntec will collect 25 groundwater samples from the current monitoring well network per recommendations in the March 2023 Annual Ground Monitoring Report. Depth to water measurements will be collected within a 24-hour period from 31 monitoring wells. Fourteen monitoring well included in the current annual groundwater sampling plan (MW001, MW002, MW004, MW006, MW014 through MW018, MW026, MW027 and MW029 through MW031) and five monitoring wells sampled biennially (MW005, MW011, MW019, MW020R, and MW028) will be sampled using passive diffusion bags (PDBs). Conventional sampling will be performed in monitoring wells MW008, MW010, MW013, MW022, MW023, and MW025 using a peristaltic pump.

Purge water generated during sampling activities will be placed in a 55-gallon drum and manifested for off-site disposal. IDW will be staged in the rear of the facility.

Subtask 3: Groundwater Monitoring Report Geosyntec will prepare an Annual & Biennial Groundwater Monitoring Report documenting the results of the depth to groundwater measurements and VOH concentrations. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the groundwater monitoring event. A final signed waste manifest for IDW transport and disposal will also be included in the Groundwater Monitoring Report.

Subject: Biennial Groundwater Monitoring Scope of Work and Level of Effort Oak Hill Cleaners 7628-1 103rd Street, Jacksonville, Florida 32210 FDEP Facility ID ERIC_4544

Dear Ms. Aduwa, Geosyntec Consultants, Inc. (Geosyntec) has prepared this scope of work (SOW), Cost Estimate Form (CEF), and Level of Effort (LOE) for biennial natural attenuation monitoring (NAM) of volatile organic halocarbons (VOHs) at the Oak Hill Cleaners Site (Site) located in Jacksonville, Florida. A copy of the CEF and LOE is provided as Attachment A. NAM of VOHs proposed in this SOW are consistent with recommendations provided in the Annual Groundwater Monitoring Report approved by the Florida Department of Environmental Protection on 11 July 2023.

On 9 January 2024, Geosyntec confirmed that a Site Access Agreement (SAA) was executed with SUSO 2 Oak Hill LP on 15 December 2022 as the real property owner of the Oak Hill Cleaners Site (ERIC_4544) located at the above address. The Duval County property appraiser database indicates the property owner since 30 December 2013 is “SUSO 2 Oak Hill LP,” and that the property has not changed ownership since then. As a result, Geosyntec believes the current SAA remains in force.

Subtask 1 – Project Management

Blaine Dawson is the project manager; his hours are directly computed.

Subtask 2 – Groundwater Sampling

Geosyntec will collect 25 groundwater samples from the current monitoring well network per the recommendations in the March 2023 Annual Groundwater Monitoring Report. Depth to water measurements will be collected within a 24-hour period from 31 monitoring wells. Fourteen monitoring wells included in the current annual groundwater sampling plan (MW001, MW002, MW004, MW006, MW014 through MW018, MW026, MW027 and MW029 through MW031) and five monitoring wells (MW005, MW011, MW019, MW020R, and MW028) sampled biennially will be sampled using passive diffusion bags (PDBs). Geosyntec will mobilize at least two weeks in advance of sampling to deploy PDBs in the five monitoring wells sampled biennially so that the PDBs can equilibrate with groundwater. New PDBs will also be deployed in the 14 annually sampled monitoring wells following sample collection for the next annual sampling event. Conventional sampling will be performed on monitoring wells MW008, MW010, MW013, MW022, MW023, and MW025 using a peristaltic pump as PDB kits have not been acquired and deployed for these six monitoring wells. The 25 groundwater samples will be analyzed by a fixed-based lab for volatile organic halocarbons (VOHs) by Environmental Protection Agency (EPA) Method 8260B.

Purge water generated during sampling activities will be placed in a 55-gallon drum and manifested for off-site disposal. IDW will be staged in the rear of the facility. To confirm compliance with the existing non-hazardous IDW groundwater profile with our selected IDW waste hauler Hull’s Environmental Services, an IDW groundwater sample will be collected for VOHs via EPA Method 8260 from the purge water drum. Historical PFAS concentrations have indicated disposal as non-hazardous waste under the existing non-hazardous waste profile. Bid costs prepared by the waste disposal companies are summarized in the following table and included in Attachment A.

Subtask 3 – Groundwater Monitoring Report

Geosyntec will prepare an Annual & Biennial Groundwater Monitoring Report documenting the results of the depth to groundwater measurements and VOH concentrations. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the groundwater monitoring event. Geosyntec will evaluate plume stability using Mann-Kendall statistical analysis and the report will be signed and sealed by a Florida-licensed professional geologist. A final signed waste manifest for IDW transport and disposal will also be included in the Groundwater Monitoring Report.
