About this file
This document package comprises a Task Assignment Change Order (TACO) Number 2 and supporting documentation for environmental site assessment services at the City of Marianna-Jackson Square property (Site ID ERIC_18453) in Florida. The parties involved are the State of Florida Department of Environmental Protection (FDEP), serving as the contracting agency, and Geosyntec Consultants, Inc., the contractor. The contract operates under indefinite delivery vehicle HW683, with Task Assignment BF02A as the primary work authorization. Megan Johnson represents FDEP as the project manager, while Kevin Warner serves as the contractor's project manager. The work is being performed for the City of Marianna under FDEP's oversight.
The contract authorizes a comprehensive soil and groundwater assessment and reporting project that includes five primary subtasks: project management, direct push technology (DPT) borings and monitoring well installation, groundwater sampling and analyses, site assessment report preparation, and contaminated investigation-derived waste (IDW) disposal. The original task assignment valued at $145,668.02 was increased to $166,119.83 through TACO 2, reflecting a net cost increase of $20,451.81. The change order encompasses additional labor hours for re-sampling of 14 site monitoring wells, revised report documentation incorporating second-round groundwater sampling data, and associated disposal costs for IDW drums generated during field activities. Invoicing is structured in three milestone payments: $122,803.35 upon department approval of field documentation and laboratory results, $4,330.67 for IDW disposal certification approval, and $38,985.81 for project management and site assessment report approval, with a total contract value of $166,119.83.

Memorandum TO: Geosyntec Consultants, Inc. HW683 Contract File
FROM: Megan Johnson, Contract Manager
DATE: October 15, 2024
SUBJECT: Negotiation Summary – SRP28 Task Assignment (TA) Change Order #2 – TA BF02A C2 Jackson Square, City of Marianna – Site ID # ERIC_18453 Site Assessment activities
This Task Assignment Change Order (TACO) settles the WCD authorizing the re-sampling of 14 site monitoring wells with other associated costs. Specific changes are detailed below:
Subtask 1: Project Management The increase of overall task labor hours produced a change associated with Project Management.
This change resulted in a cost actual increase of $2,022.75.
Subtask 2: DPT Borings and MW Installation The subcontractor’s actual invoice resulted in a cost actual decrease of ($353.10).
Subtask 3: Groundwater Sampling and Analyses This subtask authorized the increased costs for the second round of groundwater sampling including labor hours, transportation and equipment expenses, and laboratory costs. This change resulted in a cost actual increase of $8,094.52.
Subtask 4: Site Assessment Report This subtask was for the revising of the SAR to include the second round of groundwater samples.
This change resulted in a cost actual increase of $9,914.49.
Subtask 5: IDW Disposal This subtask includes additional costs for labor and expenses for disposing of IDW drums generated. This change resulted in a cost actual increase of $773.15.
Summary The Department’s SRP Contract Manager and Geosyntec’s Contract Manager have both agreed to the above changes. All changes noted above represent a total overall cost increase of $20,451.81, as shown in the Cost Actual LOE. Due to a rounding error in Excel, the Delta LOE shows a cost increase of $20,451.82, but this was corrected in the Cost Actual LOE. The changes were noted in the TACO Request received from Geosyntec on October 2, 2024. The final cost associated with this TA increased from $145,668.02 to $166,119.83. Documentation of the changes is included in the attached TACO Request, the Delta LOE spreadsheet and the Cost Actual LOE spreadsheet, and the TACO form for BF02A C2 prepared by the Department.
