This Task Assignment authorizes Geosyntec the procurement, installation, and start-up for one month of operation of a Soil Vapor Extraction (SVE) remediation system at the Vick’s Cleaner’s Inc. in accordance with the Geosyntec scope of work, cost estimate, and schedule received on 03 October 2022.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2 – Site Visits and Field Preparation:

This subtask includes installation construction, permitting, and site visits with utility companies.

In addition, it includes the necessary evaluation required for submittals from the Contractor listed in the bid documents such as Contractor’s Health and Safety Plan, construction schedule, certificates of insurance, services agreement/work order, and copy of utility locate ticket.

Geosyntec will prepare and evaluate permit drawings for the SVE system installation and electrical service with Florida Power and Light (FP&L), the City of Gulf Breeze, and Santa Rosa County and assistance to Contractor with permit applications. Geosyntec will participate in one technical planning call with the FDEP prior to construction and prepare a Health and Safety Plan for SVE system construction activities. Geosyntec will also verify property ownership. Last, Geosyntec will participate in an onsite kick-off meeting to discuss the installation plans with the landowner, tenants, and Contractor (to occur prior to the first day of SVE system construction activities).

Interior work will be conducted after business hours as requested by the tenants. Management of the business also requested third-party overnight security be present to monitor interior work to ensure merchandise is not damaged.

Geosyntec will prepare an equipment component list and the invoice to DEP following receipt of the system at the Site. Once the property assignment letter is received from DEP, Geosyntec will

Subtask 3 – SVE Installation and Startup Costs associated with this subtask include the installation of conveyance piping inside and outside of the building to the SVE container, delivery of the SVE system from the manufacturer to the Site, off-loading the SVE system, setting up the system at the Site, providing electrical power, and system testing and shakedown. 10 onsite field day at 9 hours per day is assumed in order to observe the installation, troubleshooting, and startup activities based on the LMS’s bid estimate. The construction considers routing the system piping through the concrete surface of the facility. Up to 10 drums of Investigative Derived Waste (IDW) are anticipated to be generated during trenching activities.

Geosyntec and LMS will perform the following activities: Conduct private underground utility locating for the installation of the SVE wells and temporary electrical power pole, contract overnight security to accommodate concerns by the business, facilitate the transport and delivery of the SVE system, install 5 wellheads, install SVE conveyance piping as well as connect piping from wells to SVE system and vacuum test piping. Geosyntec will provide and install a 200-pound vapor phase granular activated carbon (GAC) cannister outside the SVE system trailer, install a 6 ft high chain link fence with a 6 ft wide gate for access with a lock, install a no trespassing sign and install a temporary power pole, main electrical disconnect, and meter can near the SVE trailer for three phase, 240-volt electricity provided by FP&L. Last, Geosyntec and LMS will obtain final approval of electrical system from FP&L and City of Gulf Breeze inspector, program the onboard (assumed) telemetry system and default alarms, and troubleshoot the system and blower with the system manufacture (LMS) and the electrician (contracted by LMS) for three days (10 hours each).

During startup, Geosyntec will perform the operation of the blower, operation of all valves, gauges, and sample ports, hour meter, vacuum relief valve, high-level alarm, and telemetry and remote alarm notifications.

Geosyntec will monitor air emissions by collecting air samples from the five SVE wells, GAC influent and effluent, 4 hours after system startup. Air samples will be analyzed by a Florida Department of Health (FDOH) National Environmental Laboratory Accreditation Conference (NELAC) certified laboratory by United States Environmental Protection Agency (USEPA) Method TO15. Geosyntec will also collect one composite IDW sample from the drums of solids for analysis of TCLP metals and TCLP VOCs to develop a waste characterization profile. Ten drums have been assumed for disposal based on non-hazardous waste characterization.\

Subtask 4 – Week 1 O&M and Month 1 O&M:

Week one and month one O&M events are included with this subtask. Each O&M visit will include air sampling performance, vacuum recording as well as air pressure and temperature readings, air flow rate recording at vapor extraction wells, draining fluids accumulated in the moisture separator and disposing of fluids in a 55-gallon drum as investigation derived waste (IDW), collecting groundwater level measurements in monitoring wells MW003, MW004, and MW005, inspecting and cleaning the equipment compound; and performing periodic equipment maintenance activities recommended by equipment manufacturers.

Vacuum measurements will be collected at each of the system vacuum gauges for each VEW) and at monitoring wells MW003, MW004, and MW005 during each O&M visit. Geosyntec will monitor air emissions by collecting samples from VEW wells VEW01 through VEW05, the GAC influent and effluent at the end of the first week and at the end of the first month of SVE system startup.

Subtask 5 –Construction Completion Report A construction completion report is included with this subtask upon the completion of the first month of O&M. this report will document the results of the construction activities, provide the results of the first month of SVE performance monitoring; include as-built drawings for the installed SVE system; and include the O&M manual for the SVE system.

Subtask 6 – Utilities This subtask includes estimated costs associated with utility connection fees and electrical utility fees for one month of system operation. Cellular connection is not included in this subtask during the first month.

Prior to field work, Geosyntec will obtain an updated Access Agreement from the current real property owner of the Vick’s Cleaners Inc. (ERIC_5421) site.

The DEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $175,679.61 with 393 labor hours.

Subject: Scope of Work and Level of Effort Soil Vapor Extraction System Installation and Startup Vick’s Cleaners Inc.

302 Gulf Breeze Parkway Gulf Breeze, Santa Rosa County, Florida FDEP Facility ID No. ERIC_5421

Dear Mr. Miller:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW) and Level of Effort (LOE) for the procurement, installation, start-up and one month of operation of a Soil Vapor Extraction (SVE) remediation system at the Vick’s Cleaner’s Inc. (referred to herein as “Site”).

The Site is located at 302 Gulf Breeze Parkway, Gulf Breeze, Santa Rosa County, Florida. A summary of the costs associated with the SOW described below is included with the attached LOE.

A bid comparison table along with copies of subcontractor quotes are attached for reference – LMS Manufacturing is recommended for system fabrication, construction, and installation.

GeoTek and GPR Services of Alabama both offered the lowest price of $1,500 for utility locating, and Geosyntec will contract with the firm that can best meet the project schedule. The lowest comprehensive hourly rate for overnight security was provided by Sceptre.

Subtask 1 – Project Management

Subtask 2 – Site Visits and Field Preparation:

This subtask includes the activities associated with preparing for construction activities including installation construction, permitting, and site visits with utility companies. The subtask includes costs associated with the following activities:

• evaluate required submittals from the Contractor listed in the bid documents such as

FR8793/Vick's Cleaners System Construction_SOW and LOE - Sept 2022 - Revised

Contractor’s Health and Safety Plan, construction schedule, certificates of insurance, services agreement/work order, and copy of utility locate ticket (Florida 811);

• participation in an onsite kick-off meeting to discuss the installation plans with the landowner, tenants, and Contractor (to occur prior to the first day of SVE system construction activities).

As requested by the tenants, interior work will be conducted during closed business hours for the Tuesday Morning store, thus requiring night work for the interior portions of the remediation system construction. Management of the Tuesday Morning store also requested third-party overnight security be present monitor interior work to ensure merchandise is not damaged.

During the annual meeting with the FDEP on 26 April 2022, Geosyntec recommended purchasing a new SVE system for the Site and requested proposals for new SVE systems from four remediation system contractors. The selected system manufacturer was LMS Manufacturing.

Geosyntec’s field preparation tasks will include support to LMS and site preparation activities for shipment and delivery of the system. Delivery of the system will be managed to coincide with the system installation schedule. Geosyntec will prepare an equipment component list and the invoice to DEP following receipt of the system at the Site. Once the property assignment letter is received from DEP, Geosyntec will etch and label the system components of value (e.g., panel, blower, etc.) and return the letter with photo-documentation to serve as a milestone deliverable.

Subtask 3 – SVE Installation and Startup

This subtask includes costs associated with the installation of conveyance piping inside and outside of the building to the planned location of the SVE container (northeast of the rear of the of the Tuesday Morning suite), delivery of the SVE system from the manufacturer to the Site, off-loading the SVE system, setting up the system at the Site, providing electrical power, and system testing and shakedown. Geosyntec assumes ten (10) onsite field days (9 hours per day) to observe the installation, troubleshooting, and startup activities based on the LMS’s bid estimate. The construction takes into account routing the system piping through the concrete surface of the facility. Up to 10 drums of Investigative Derived Waste (IDW) are anticipated to be generated

FR8793/Vick's Cleaners System Construction_SOW and LOE - Sept 2022 - Revised during trenching activities. As part of this subtask, Geosyntec and LMS shall perform the following activities:

• Conduct private underground utility locating for the installation of the SVE wells and temporary electrical power pole (GeoTek or GPRS will be selected based on availability).

• Contract overnight security by Sceptre (a private security company) to accommodate tenant concerns during overnight, indoor work activities within the Tuesday Morning store. Our LOE assumes 9-hour shifts for 10 days.

• Transport and delivery of the SVE system to the Site and off-loading;

• Install 5 SVE wellheads (VEW-1 through VEW-5) and associated manhole completions.

• Install SVE system conveyance piping, connect conveyance piping from SVE wells to SVE system container, and vacuum test all conveyance piping to ensure adequate seals.

• Provide and install a 200-pound vapor phase granular activated carbon (GAC) cannister outside the SVE system trailer (inside the fenced area).

• Install a 6-ft high galvanized double wide chain link fence (with privacy slats) around three sides of the SVE system, approximately 60 linear feet. The fourth side will be secured by the building wall.

• Install a 6-ft wide double hinged access gate, equipped with a lock.

• Install the FDEP provided “Warning, No Trespassing, Hazardous Waste Treatment System” Sign.

• Install a temporary power pole, main electrical disconnect, and meter can near the SVE trailer for three phase, 240-volt electricity provided by FP&L.

• Obtain final approval of electrical system from FP&L and City of Gulf Breeze inspector.

• Program the onboard (assumed) telemetry system and default alarms.

• Troubleshoot the system and blower with the system manufacture (LMS) and the electrician (contracted by LMS) for three days (10 hours each).

During startup, Geosyntec will perform the following activities to demonstrate the mechanical elements of the system are operating within specified rates and capacities:

FR8793/Vick's Cleaners System Construction_SOW and LOE - Sept 2022 - Revised

• operation of the SVE blower;

• operation of all valves, gauges, and sample ports;

• establishment of design well head vacuum and radius of influence (ROI);

• operation of the hour meter;

• operation of the vacuum relief valve;

• operation of the moisture vessel high-level alarm; and

• operation of telemetry and remote alarm notifications.

Ten drums have been assumed for disposal based on non-hazardous waste characterization; LMS will provide transportation and disposal of the IDW drums based on the $110/drum contingency provided in their cost estimate.

This subtask also includes the following activities to be performed by the Geosyntec Project Manager (K. Carlton) and/or Engineer of Record (K. Warner) as specified below:

• Provide technical assistance to Site personnel daily and weekly site visits during installation activities;

• Complete final site inspection – to occur on day of SVE system startup.

Geosyntec will submit field documentation to FDEP as a deliverable milestone for this subtask.

The documentation will include field notes and before/after photographs.

Subtask 4 – Week 1 O&M and Month 1 O&M:

This subtask includes the week 1 and month 1 operation and maintenance (O&M) events. Each O&M visit will include the following:

• recording vacuum readings at the vapor extraction wells VEW01 through VEW05 and vapor monitoring points VMP001 and VMP002;

• draining fluids accumulated in the moisture separator and disposing of fluids in a 55-gallon drum as investigation derived waste (IDW) (1 drum assumed; transportation and disposal will be addressed under Year 1 O&M task assignment);

• collecting groundwater level measurements in monitoring wells MW003, MW004, and

MW005;

• inspecting and cleaning the equipment compound; and

• performing periodic equipment maintenance activities recommended by equipment manufacturers.

Vacuum measurements will be collected at each of the system vacuum gauges (system trailer and wellhead) for each VEW (active or passive use) and at monitoring wells MW003, MW004, and MW005 during each O&M visit. The vacuum monitoring data will be used to assess the actual vacuum ROI at the Site. As necessary, additional monitoring wells may serve as vacuum monitoring locations. The system will be adjusted to optimize the mass recovery rates.

Geosyntec will monitor air emissions by collecting samples from VEW wells VEW01 through VEW05, the GAC influent and effluent at the end of the first week and at the end of the first month of SVE system startup (7 samples per event). Air samples will be analyzed by a FDOH NELAC certified laboratory by EPA Method TO15. The analytical results associated with the first month of operation will be used to evaluate whether off-gas treatment can be terminated after 30 days of SVE operation (untreated individual HAP emissions of less than 2.3 lb/day, total HAP emissions of less than 6.8 lb/day, per the October 2008 FDEP Considerations for Air Emissions From Hazardous Waste Cleanup Sites Memorandum).

The results of each O&M event will be documented in a site-specific O&M log, which will be submitted to the FDEP with the Construction Completion and Startup Report.

RE: Utility Locating Services – 302 Gulf Breeze Parkway, Gulf Breeze, Florida

Mr. Carlton GeoTek Services is pleased to respond to your request for Utility Locating services at 302 Gulf Breeze Parkway located in Gulf Breeze, Florida. We will scan the area shown in your PDF drawing with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as structures, buildings, or vehicles. We will scan as close as reasonably possible to fixed objects such as obstructions, vehicles, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed-price quote for this effort is $1,500.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Geosyntec Consultants 1120 N. 12th Avenue, Pensacola, FL 32501 Re: Proposal for GPR Services Located at 302 Gulf Breeze Pkwy, Gulf Breeze, FL GPR Proposal Number: P0021.22

Dear Mr. Carlton, In accordance with you request, GPR Services of Alabama, Inc. (GPR Services) is pleased to provide Geosyntec Consultants with this proposal to scan utilities/amomalies at the above referenced location as shown on the provided figure. Scanning techniques are presented below followed by cost information for this project.

GPR SCANNING METHOD

GPR Services will scan the proposed location as shown on the provided site Figure or as directed by on-site personnel. Any subsurface anomaly located by our GSSI Utiltyscan will be marked with pink spray paint. GPR Services will perform scanning methods with the following equipment;

GSSI Utilityscan- 350 MHz GPR Antenna. This antenna transmits a sequence of pulses and displays the images on a tablet, which records and saves each reading. The 350 MHz antenna allows for GPR penetration from depths of 0’-7’ in conductive (clayey) soil types and 0’-12’ in loose sandy soils.

Equipment Limitations: Typically, a target must be at least 1” in diameter per 1’ of depth in order for it to be located with GPR, i.e., a pipe 7’ in depth must be at least 7” in diameter.

Wet/saturated soil, standing water, and shallow groundwater can also limit the abilities of GPR.

In addition, certain types of pipes (clay and empty pvc type pipes) are very difficult to locate and GPR Services of Alabama cannot guarantee it will be able to locate all utilities on site.

The services described-above will be provided for the unit costs indicated below:
