This Task Assignment authorizes Geosyntec

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2 – Annual Groundwater Monitoring Subtask 2 includes costs associated with annual groundwater monitoring activities. Geosyntec personnel will gauge the depth to water in Site-wide monitoring wells (total of 30 wells) to evaluate groundwater flow direction and will collect groundwater samples from three monitoring wells (MW021, MW025, and MW026). Groundwater samples will be collected using conventional low flow purging methods and will include collection of field parameters using a water quality meter during groundwater sampling activities.

Subtask 3 – Groundwater Monitoring Report Geosyntec will prepare a report documenting the results of the annual groundwater monitoring event. The report will contain, at a minimum, tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s) and analytical results, and a summary of conclusions and recommendations

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $8,332.56 with 62 labor hours.

Geosyntec may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Lipham Cleaners_Annual GW Sampling_SOW_2022-02

11 February 2022

Mr. Evan Miller Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road Tallahassee, FL 32399-2400

Subject: Scope of Work and Level of Effort 2022 Annual Groundwater Monitoring Lipham Cleaners 2492 W State Road 434, Longwood, Seminole County, Florida

ERIC_5500

Dear Mr. Miller:

Geosyntec Consultants, Inc. (Geosyntec) has prepared the attached proposed Scope of Work and Cost Estimate Form/Level of Effort (CEF/LOE) for annual groundwater monitoring at Lipham Cleaners (Site) located in Longwood, Orange County, Florida. A detailed explanation of the proposed scope of work is provided below.

Subtask 1 – Project Management

This subtask covers the general management of the project as specified in Contract No. HW683.

Subtask 2 – Annual Groundwater Monitoring

Subtask 2 includes costs associated with annual groundwater monitoring activities. Geosyntec personnel will gauge the depth to water in Site-wide monitoring wells (total of 30 wells) to evaluate groundwater flow direction and will collect groundwater samples from three monitoring wells (MW021, MW025, and MW026; Figure 1). This event was originally proposed to be conducted using passive diffusion bags (PDBs); however, further evaluation of the proposed monitoring well diameters indicated that this would not be a feasible option (smallest allowable diameter well is 1-inch; however, a proposed well is 0.75-inch diameter). Therefore, groundwater samples will be collected using conventional low flow purging methods and will include collection of field parameters (consisting of pH, specific conductance, temperature, dissolved oxygen [DO], and oxidation-reduction potential [ORP]) using a water quality meter during groundwater sampling activities. One duplicate groundwater sample will be collected for quality assurance purposes.

Groundwater samples will be submitted to a fixed-based laboratory for volatile organic hydrocarbon (VOH) analysis by Environmental Protection Agency (EPA) Method 8260.

Groundwater sampling activities are proposed to occur in April 2022.

Investigation Derived Waste (IDW) will consist of purge and decontamination water generated during sampling activities and will be placed in one 55-gallon drum for manifested off-site disposal. Necessary IDW characterization samples will be collected from the drum upon completion of the sampling event in order to develop a new waste profile and will include analysis of VOHs (8260B) and 8 RCRA Metals (6010C/7470A). Clark Environmental is the selected subcontractor for IDW transport and disposal. Subcontractor bids and evaluation form are included in Attachment A.

Subtask 3 – Groundwater Monitoring Report

Geosyntec will prepare a report documenting the results of the annual groundwater monitoring event. The report will contain, at a minimum, tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s) and analytical results, and a summary of conclusions and recommendations. Additionally, effort will be included to further evaluate analytical results for plume stability using Mann-Kendal analyses. The report will be signed and sealed by a Florida-licensed professional engineer or geologist in responsible charge of the work.

Effort included in this subtask includes the effort to update necessary ADaPT and GIS data tables consisting of groundwater elevation measurements and groundwater sampling laboratory results.

BID PROPOSAL FORM 2022

FDEP Drycleaner and Hazardous Waste Sites

Investigation Derived Waste (IDW) Transportation and Disposal

Contractor Company Name: HEPACO, LLC

Contact: Linda Basinger, 813-240-8020, lbasinger@hepaco.com

Region T&D of 1 Non-Hazardous IDW Drum (solid or liquid), Process Code STAB01

T&D of 1 Hazardous IDW Drum (solid or liquid), Process Code STAB02

Northwest $ 191.27 $ 367.36

Northeast $ 191.27 $ 367.36

Central $ 170.00 $ 346.10

Southwest $ 170.00 $ 346.10

Southeast* $ 170.00 $ 346.10

Assumptions and Clarifications:

• Unless otherwise specified within this proposal all terms and conditions will be governed by the most recent Master Services

Agreement between GEOSYNTEC and HEPACO, LLC. In the event that a current agreement is not in place all terms and conditions will be governed by HEPACO’s standard service agreement.

• Price is contingent upon profile approval and process code approval into HEPACO’s preferred disposal facilities.

• A minimum stop fee of $500.00 will apply to all shipments.

• The above rates are for less than truck load quantities (LTL). In the event that a higher volume of disposal is needed at any particular facility HEPACO will provide a case by case pricing scenario to help reduce overall cost.

• Above rates include a standard energy and insurance fee. In the event that market changes dramatically HEPACO reserves the right to increase or decrease these fees based on the appropriate energy index. In the event that this is required HEPACO will provide a

30 day written notice of the change.

* Excludes Monroe County (Case by case basis determined by volume of IDW)

• Bid costs above should be inclusive of transportation fees, fuel fees, environmental recovery fees, etc.

• No standby time should be included in the above bid costs.

• Assume that the drums are accessible.

• It is assumed that VOH totals results will be provided for profiling. If other analytical results are required, please indicated here:
