HW683_DC011C

This Task Assignment authorizes Geosyntec to conduct for the installation, start-up, and one month of operation of a Soil Vapor Extraction (SVE) remediation system at the former Scott’s Custom Cleaners Site in accordance with the Geosyntec scope of work, cost estimate, and schedule received on June 28, 2023. There is a $600.00 in cost reimbursement (utilities).

Geosyntec confirmed on June 28, 2023, that Subtask 1 consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.

Subtask 2: This subtask includes effort associated with preparing for obtaining a new site access agreement, construction activities and permitting. Geosyntec will submit the equipment component list and photo document the etching and labeling of the major system components of value.

Subtask 3: This subtask includes costs associated with the installation of 4 new SVE wells, installation of conveyance piping inside and outside of the building, pickup and delivery of the SVE system from the Walker Plaza Site to the Scott’s Cleaners Site.

Subtask 4: This subtask includes the week 1 and month 1 O&M events. Vacuum measurements will be collected from each VEW well, and selected monitoring wells with partially submerged screened intervals (e.g., MW002, MW-14, and MW-18) during each O&M visit. Geosyntec will monitor air emissions by collecting samples from SVE wells VEW01 through VEW04 and the GAC influent and effluent at the end of the first week and at the end of the first month of SVE system startup.

Subtask 5: This subtask includes costs associated with the preparation of the Construction Completion and Startup Report upon the completion of the initial one month of O&M. The Report will document the results of the construction activities, and provide the results of the first month of SVE performance monitoring.

Subtask 6: This subtask includes costs associated with utility connection fees and electrical utility fees for one month of system operation.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $199,421.76 with 599 labor hours.

Ms. Brittany Plyer Florida Department of Environmental Protection 2600 Blair Stone Road Tallahassee, FL 32399-2400

Subject: Scope of Work and CEF/LOE Soil Vapor Extraction System Installation and Startup Scott’s Custom Cleaners, Inc 2454 McMullen Booth Road, Suite 205 Safety Harbor, Pinellas County, Florida 33759 FDEP Site ID No. ERIC_5307

Dear Ms. Plyer:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this proposed Scope of Work (SOW) and Level of Effort (LOE) for the installation, start-up, and one month of operation of a Soil Vapor Extraction (SVE) remediation system at the former Scott’s Custom Cleaners Site located in Safety Harbor, Florida (ERIC_5307). As part of this implementation, the existing SVE system currently located at the Walker Plaza site (ERIC_4783) will be transport to the Site and re-used. Activities related to the SVE system decommissioning at Walker Plaza will be detailed in a separate

SOW/LOE.

Geosyntec submitted bid requests to three Contractors and conducted a Site walk with interested Contractors on May 10, 2023. We received two cost estimate and one no-bids. A bid comparison table along with copies of subcontractor quotes are included in Attachment A for reference.

Equipment with Experience (EWE) was recommended for SVE system fabrication, construction, and installation, while Erwin Remediation (Erwin) provided the most cost-competitive bid for IDW transport and disposal.

Subtask 1 – Project Management

Subtask 2 – Construction Preparation

This subtask includes effort associated with preparing for construction activities and permitting.

The subtask includes costs associated with the following activities:

• Obtain a new Site Access Agreement (SSA) with the current property owner;

http://www.geosyntec.com/

SVE SOW/LOE

Protection (FDEP) prior to construction;

• Prepare a Health and Safety Plan for SVE system construction and operation and maintenance (O&M) activities;

• Attend an onsite kick-off meeting to discuss the installation plans with the owner, tenants, and Contractor (to occur prior to the first day of SVE system construction activities).

Geosyntec will submit the equipment component list and photodocument the etching and labeling of the major system components of value (e.g., panel, blower, etc.) as a milestone deliverable.

Subtask 3 – SVE Installation and Startup

This subtask includes costs associated with the installation of 4 new SVE wells, installation of conveyance piping inside and outside of the building, pickup and delivery of the SVE system from the Walker Plaza Site to the Scott’s Cleaners Site, off-loading and leveling the SVE system, setting up the system at the Site, providing electrical power, and system testing and shakedown.

Based on EWE’s bid, Geosyntec assumes 18 onsite field days (10 hours per day) to observe the construction and installation activities and 2 onsite field day (10 hours per day) for shakedown and startup activities. Up to one soil and/or one water drum of investigative derived waste (IDW) are anticipated to be generated. Geosyntec and EWE shall perform the following activities:

• Conduct private underground utility locating for the installation of the SVE wells;

• Transport and delivery of the SVE system to the Site and off-loading/leveling;

• Install 4 new SVE wells (VEW01 and VEW04);

• Abandon 6 MAGS wells (MAGS001 through MAGS006);

• Install SVE system conveyance piping, connect conveyance piping from SVE wells to SVE system manifold, and vacuum test all conveyance piping to ensure adequate seals;

• Install wooden box structures to encase the VEW piping at VEW01 and VEW04;

• Provide and install a 140-pound vapor phase granular activated carbon (GAC) vessel outside the SVE system trailer (inside the fenced area).

• Install a 6-ft wide swing gates for easy access to the SVE system and equipment, equipped with a combination lock.

June 29, 2023

• Install the FDEP-provided “Warning, No Trespassing, Hazardous Waste Treatment System” sign.

• Install either underground or aboveground electricity to the SVE skid-mounted equipment container, based on permitting requirements obtained by EWE as part of Subtask 2.

• Install a main electrical disconnect and meter can near the SVE trailer for single-phase, 240-volt electricity.

• Obtain final approval of the electrical system from Duke Energy and the City of Safety Harbor inspectors.

• Program the onboard telemetry system and default alarms.

• Troubleshoot the system and blower with EWE and the electrician (contracted by EWE).

During startup, Geosyntec will perform the following activities to demonstrate the mechanical elements of the system are operating within specified rates and capacities:

• Operation of the SVE blower;

• Operation of all valves, gauges, and sample ports;

• Establishment of design wellhead vacuum and radius of influence (ROI);

• Operation of the hour meter;

• Operation of the vacuum relief valve;

• Operation of the moisture vessel high-level alarm; and

• Operation of telemetry and remote alarm notifications.

Geosyntec will monitor air emissions by collecting air samples from the four (4) SVE wells and GAC influent and effluent after system startup. Air samples will be analyzed by a Florida Department of Health (FDOH) National Environmental Laboratory Accreditation Conference (NELAC) certified laboratory by United States Environmental Protection Agency (USEPA) Method TO-15.

This subtask also includes the following activities to be performed by the Geosyntec Project Manager (Fabio Fortes) and/or Engineer of Record (Joseph Bartlett) as specified below:

• Provide technical assistance to Site personnel daily and weekly site visits during installation activities;

• Complete final site inspection – to occur on day of SVE system startup.

Geosyntec will submit field documentation to FDEP as a deliverable milestone for this subtask.

The documentation will include field notes and representative photographs of Site conditions following installation of the SVE system.

June 29, 2023

Subtask 4 – Week 1 and Month 1 System O&M

This subtask includes the week 1 and month 1 O&M events. Each O&M visit will include the following:

• Performing required air sampling;

• Recording vacuum, pressure, and temperature readings at the SVE blower;

• Recording air flow rates at the blower and vapor extraction wells;

• Recording vacuum readings at the vapor extraction wells VEW01 through VEW04 and selected monitoring wells with partially submerged screened intervals (MW002, MW-14, and MW-18);

• Draining fluids accumulated in the moisture separator and disposing of fluids in a 55-gallon drum as IDW (1 drum assumed; transportation and disposal will be addressed under Year 1 O&M Task Assignment);

• Collecting groundwater level measurements in monitoring wells MW002, MW-14, and

MW-18.

• Inspecting and cleaning the equipment compound;

• Inspecting water entrainment and adjustment of VEW wellhead vacuums (if necessary) to minimize water entrainment; and

• Performing periodic equipment maintenance activities recommended by equipment manufacturers.

In accordance with the Interim Source Removal Work Plan – Revision 2 approved by FDEP on April 14, 2023, vacuum measurements will be collected from each VEW well, and selected monitoring wells with partially submerged screened intervals (e.g., MW002, MW-14, and MW-

18) during each O&M visit. The vacuum monitoring data will be used to assess the actual vacuum ROI at the Site. As necessary, additional monitoring wells may serve as vacuum monitoring locations. The system will be adjusted to optimize the mass recovery rates.

Geosyntec will monitor air emissions by collecting samples from SVE wells VEW01 through VEW04 and the GAC influent and effluent at the end of the first week and at the end of the first month of SVE system startup (6 samples per event). Air samples will be analyzed by a FDOH NELAC-certified laboratory by EPA Method TO-15. The analytical results associated with the first month of operation will be used to evaluate whether off-gas treatment can be terminated after 30 days of SVE operation (untreated individual HAP emissions of less than 2.3 pounds per day (lb/day), total Hazardous Air Pollutant (HAP) emissions of less than 6.8 lb/day, per the October 2008 FDEP Considerations for Air Emissions From Hazardous Waste Cleanup Sites Memorandum).

June 29, 2023

The results of each O&M event will be documented in a site-specific O&M log, which will be submitted to the FDEP with the Construction Completion and Startup Report.

Subtask 5 – Construction Completion Report

This subtask includes costs associated with the preparation of the Construction Completion and Startup Report upon the completion of the initial one month of O&M. The Construction Completion and Startup Report will: (i) document the results of the construction activities; (ii) provide the results of the first month of SVE performance monitoring; (iii) include as-built drawings for the installed SVE system; and (iv) include the O&M manual for the SVE system.

The Construction Completion and Startup Report will be signed and sealed by the Engineer of Record, a Florida Licensed Professional Engineer.
