Subject: Statement of Work – Annual Groundwater Monitoring Former First Class Cleaners 591 East Semoran Boulevard Casselberry, Seminole County, Florida

ERIC_5489

Dear Sarah:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this statement of work (SOW) and attached level of effort (LOE) cost estimate for the Florida Department of Environmental Protection (FDEP) to conduct semi-annual groundwater monitoring at the former First Class Dry Cleaner Site (FDEP Facility ID ERIC_5489). The Site is located at 591 East Semoran Boulevard, Casselberry, Seminole County, Florida. Proposed sampling activities are in accordance with the recommendations provided to FDEP from the Semi-Annual Groundwater Monitoring Report 2023. Groundwater samples were last collected at the Site in February 2023.

The purpose of the annual groundwater monitoring is to collect groundwater samples for analysis of volatile organic halocarbons (VOHs) to evaluate groundwater quality at the Site. The attached LOE consists of four subtasks and is summarized below.

Subtask 1 – Project Management

Subtask 2 – Site Access Agreements

1. The Site property, located at 591 SR 436, Casselberry, FL 32707 (Parcel ID 20-21-30- 300-0A1G-0000) is owned by 591 SR 436, LLC. The existing agreement for this property was authorized in July 2021 by Tracy K. King, representative of 591 SR 436, LLC, and is still valid. However, the existing agreement should be updated to the latest FDEP site access agreement language. Therefore, an updated Source-Property Access

April 4, 2024

FR0929#/First Class_2024 Annual GWM_SOW

Agreement will be pursued but will not be required to be in place in order to collect samples.

2. Several properties owned by the City of Casselberry contain monitoring wells requiring sampling and/or water level gauging. These properties are 92 Wilshire Dr. Casselberry FL 32707 (Parcel ID: 21-21-30-501-0200-0040), 720 SR 436 Casselberry FL 32707 (Parcel ID 21-21-30-501-0200-0000), and Easements. The existing agreement for these properties was authorized in July 2001 by Bruce A. Pronovost, , but is no longer valid as the original site access agreement had language voiding it ten years after its signing.

Therefore, an updated Non-Source Access Agreement will be required to be in place in order to collect samples.

3. MW021 and MW003 are located at 815 SR 436 Casselberry, FL 32707 (Parcel ID 20-21-30-5MJ-0000-0010), which is owned by Barracuda Stations LLC as of 2019. No Site access agreements could be located on OCULUS; therefore, an updated Non-Source Access Agreement will be required to be in place in order to collect samples.

4. MW027, MW028, and MW029 are located within the property at 830 Sr 436 Casselberry Fl 32707 (Parcel ID 21-21-30-501-0100-0000), owned by Floorsystems Inc. as of December 1977. No Site access agreements could be located on OCULUS; therefore, an updated Non-Source Access Agreement will be required to be in place in order to collect samples.

5. MW020 is located within the property at 750 SR 436 Casselberry FL 32707 (Parcel ID 21-21-30-501-0200-0020) owned by Om Pati LLC, as of April 1993. There is no existing site access agreement. Therefore, a Non-Source Access Agreement will be required prior to collecting samples.

6. MW030 is located within the property at E 436 SR Casselberry FL 32707 (Parcel ID 21-21-30-501-0100-00A0), owned by Zhi Wen Li as of September 2017.

There is no site access agreement for this property; therefore, a new Non-Source Access Agreement will be required prior to collecting samples.

Geosyntec will attempt to obtain each new or updated site access agreement specified above.

However, due to the large number of agreements needed, it is anticipated that several site access agreements will not be in place to allow for timely sampling. Therefore, Geosyntec will allow no more than two months after the acceptance of this SOW for site access agreements to be in place before initiating Subtask 3. If, at that time, an access agreement is not in place, Geosyntec will not collect a sample or water level from the affected location(s) within that property unless otherwise directed by FDEP.

Subtask 3 – Annual Groundwater Monitoring Geosyntec will mobilize to the Site to collect depth-to-groundwater measurements from each accessible monitoring well (up to 34 total wells) within a 24-hour period, collect 16 groundwater

FR0929#/First Class_2024 Annual GWM_SOW samples (12 via low-flow methods and 4 via passive diffusion bags [PDBs]), and deploy four PDBs for the March 2024 annual groundwater monitoring. Refer to the sampling plan provided in Attachment A.

Groundwater levels will be collected on the first day. Each of the 30 monitoring wells are flush-mounted, in high-traffic areas, and/or prone to collecting stormwater inside the manhole.

Geosyntec personnel will bail the manholes, measure and record groundwater levels, and resecure the manholes. Additional time has been included due to the site-specific challenges, including (i) long distances between wells necessitating driving time and traffic navigation and (ii) flush-mounted wells located in vegetated and overgrown areas necessitating the use of a metal detector to locate wells.

Geosyntec will collect 16 groundwater samples from the monitoring well network using a combination of low-flow sampling techniques and existing PDBs.

PDBs deployed in February 2023 are located in MW034, MW035, MW036, and RW005.

PDB samples will be collected from these wells, and new PDBs will be redeployed for sampling in February 2025.

Low-flow sampling techniques will be used to collect groundwater samples from 11 0.75-inch diameter monitoring wells that are too small for PDBs and from one recovery well that contains pumps and cables that block PDB deployment (Table 1). Geosyntec has assumed that these wells can be sampled using a peristaltic pump, consistent with previous sampling events at the site. Additional time has been allotted to allow travel between widely spaced wells, right-of-way access, and heavy traffic. Two field personnel will work separately for maximum effectiveness.

Additionally, low-flow groundwater samples will be collected from RW002 (sampled from approximately 95 ft bls).

The groundwater samples will be submitted for laboratory analysis of VOHs by Environmental Protection Agency (EPA) Method 8260B.

Geosyntec will use a metal detector and a GPS to locate several monitoring wells in overgrown vegetated areas. Geosyntec assumes the sampling event will take 2 people 3 days to complete and will require one truck per person.

Investigation derived waste (IDW) will be collected in two 55-gallon drums, which will be labeled, covered with plastic, and wrapped in orange construction fencing. Geosyntec obtained IDW transportation and disposal (T&D) estimates from Hulls, Erwin Remediation, LLC, and Enviroserve (Attachment B). Each company is a licensed waste hauler in Florida. Erwin

FR0929#/First Class_2024 Annual GWM_SOW

Remediation was selected due to the lowest overall cost. No IDW sample will be collected for waste characterization, as Erwin Remediation has a current waste profile for First Class Cleaners.

Sub-Contractor T&D Cost Selected Erwin Remediation $ 220.00 X Hulls $ 573.00 EnviroServe $ 366.30 Note: T&D Cost includes required IDW sampling costs

Subtask 4 – Annual Groundwater Monitoring Report Geosyntec will prepare a report summarizing sampling activities and results and will provide conclusions and recommendations based on data evaluation. The report will contain text, tables and figures summarizing field/laboratory data.

A total of three tables will be included: (i) well construction/groundwater elevation data,

(ii) groundwater analytical results, and (iii) natural attenuation data.

A total of eight figures will be included: (i) site location, (ii) site features, (iii) groundwater elevation contours (one for each of three depth intervals), and (iv) VOH concentrations (one for each of three depth intervals).

The report will be signed/sealed by a Florida-licensed geologist/engineer.
