About this file

This Task Assignment (TA) is entered into between the State of Florida Department of Environmental Protection (FDEP) and Geosyntec Consultants, Inc., pursuant to underlying Contract Number HW683. The TA, numbered DC005D, authorizes Geosyntec to conduct natural attenuation monitoring (NAM) at the Walker Plaza site (ERIC_4783) located at 603–617 North Maryland Avenue, Plant City, Hillsborough County, Florida. The assignment is effective through the completion date of May 31, 2024, with project management oversight provided by FDEP Project Manager Brittany Plyler and Geosyntec's Project Manager James Mills.

The contract authorizes Geosyntec to perform four subtasks for a total fixed unit rate value of $8,851.41: Subtask 1 comprises project management labor calculated at 13 percent of direct labor costs ($826.30); Subtask 2 includes NAM groundwater sampling activities involving depth-to-groundwater measurements from 19 wells and samples from 6 monitoring wells (MW002, MW006, MW010, MW011, MW013, and MW016) with analysis for volatile organic halocarbons ($4,130.62, due December 29, 2023); Subtask 3 covers the transportation and disposal of investigation-derived waste through Erwin Remediation at a cost of $265.22; and Subtask 4 covers preparation of the Annual NAM Report including data evaluation, Mann-Kendall statistical analysis, and evaluation of non-recorded institutional controls for potential site closure ($4,720.79, due March 29, 2024). The total TA value of $8,851.41 includes 57 labor hours and is invoiced upon completion of deliverables in two phases.

Ms. Brittany Plyler Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road Tallahassee, FL 32399-2400

Subject: Scope of Work and Cost Estimate Form/Level of Effort Natural Attenuation Monitoring - 2023

Walker Plaza 603 – 617 North Maryland Avenue Plant City, Hillsborough County, Florida

FDEP Facility ID ERIC_4783

Dear Ms. Plyler:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW) and Cost Estimate Form/Level of Effort (Attachment A) for natural attenuation monitoring (NAM) of volatile organic halocarbons (VOHs) at the Walker Plaza site (herein, the “Site”; Facility ID ERIC_4783) located in Plant City, Florida. NAM of VOHs proposed in this SOW is consistent with recommendations provided in the Post Active Remediation Monitoring Report (Report) dated March 2023 and approved by the Florida Department of Environmental Protection (FDEP) on 5 June 2023.

Geosyntec confirmed that Site Access Agreements were executed by Ms. Karen Davis on 27 April 2022 as the property owner of Walker Plaza (ERIC_4783) located at the above address and by David Ligon on 30 October 2018 as the property owner of the parcel adjacent to Walker Plaza located at 1502 East Baker Street, Plant City, FL.

Subtask 1 – Project Management

James Mills, E.I. will serve as Project Manager for the proposed SOW. Costs associated with this subtask are directly computed.

Subtask 2 – NAM Groundwater Sampling

This subtask includes costs associated with NAM activities tentatively scheduled for December 2023. As recommended in the 2023 Report, Geosyntec will collect depth-to-groundwater measurements from 19 accessible monitoring wells at the Site and groundwater samples from six monitoring wells. The monitoring wells include six surficial aquifer wells (MW002, MW006, MW010, MW011, MW013, and MW016). Groundwater samples will be collected in

WALKER PLAZA (ERIC_4783)_SOW_NAM SAMPLING_OCT 23, V23

general accordance with FDEP’s Standard Operating Procedures and will be submitted for laboratory analysis of VOHs by Environmental Protection Agency Method 8260B.

Geosyntec has assumed that this subtask can be completed by one field staff in two field days including round trip travel.

Subtask 3 – IDW Management

This subtask includes costs associated with the transportation and disposal of IDW generated during groundwater sampling activities. Geosyntec will containerize purge water in one 55-gallon steel drum, which will be labelled, covered with plastic, and wrapped in orange construction fencing. The drum will be transported off-Site and disposed of under the existing non-hazardous waste profile by Erwin Remediation, Inc. Quotes for the transportation and disposal of IDW are included as Attachment B. Erwin Remediation, Inc. remains the lowest cost provider.

Subtask 4 – Annual Groundwater Monitoring Report Preparation

This subtask includes effort for Geosyntec to prepare an Annual NAM Report summarizing sampling activities and results, and provide conclusions and recommendations based on data evaluation. The report will contain field forms, the final signed waste manifest for IDW transport and disposal, tables summarizing field and laboratory data, figures showing groundwater flow directions, and a textual summary of the groundwater monitoring event. As part of the report preparation and to evaluate the path forward, Geosyntec will evaluate plume stability and attenuation of the groundwater plume using Mann-Kendall statistical analysis. The report will be signed and sealed by a Florida-licensed geologist and one electronic copy of the final report will be submitted to the FDEP via email.

Additionally, this subtask includes effort for Geosyntec to evaluate the use of a non-recorded institutional control for groundwater with the Southwest Florida Water Management District to pursue conditional closure of the Site via Risk Management Option III. The Annual NAM Report will include the findings from this effort and recommend a path forward for site closure.

WALKER PLAZA (ERIC_4783)_SOW_NAM SAMPLING_OCT 23, V23

Invoice and Deliverable Schedule:

Note that this schedule reflects two weeks FDEP review of the SOW. Geosyntec requests the following schedule of deliverables and invoices:
