About this file

This document package comprises a Task Assignment (TA) Change Order (TACO #1) and supporting cost documentation for Contract HW683 between the State of Florida Department of Environmental Protection (FDEP) and Geosyntec Consultants. The contract pertains to site assessment activities at the Former Ruck's Park brownfield site (Site ID ERIC_17896) located at 13760-13780 NE 5th Avenue, North Miami, Miami-Dade County. The change order was authorized by Work Change Directive #1 issued on July 7, 2025, and executed on August 19, 2025. Key personnel include Megan Johnson as FDEP's Contract Manager and Shekhar Melkote, P.G., as the contractor's Project Manager.

The original Task Assignment BF04B was valued at $99,981.91 with a completion date of October 10, 2025. TACO #1 increases the total contract value to $109,242.03, representing a cost increase of $9,260.12, with the completion date extended to November 21, 2025 (42 additional days). The change authorizes additional soil and groundwater sample collection at selected locations to confirm previous analytical results and measure groundwater levels. The scope adjustments include increased labor hours across multiple subtasks: Subtask 1 (Project Management) increased $1,125.72; Subtask 4 (Field Work) increased $4,575.57; and Subtask 5 (Site Assessment Report Addendum) increased $3,725.63. Subtask 6 (IDW Disposal) decreased by $166.80 due to reduced drum requirements. Supporting invoices from subcontractors include Earth Tech Drilling ($14,600.00), Engenuity Group surveying services ($3,870.00), and GeoTek Services utility locates ($1,500.00). Deliverables are scheduled with departmental approval requirements through October 13, 2025, with invoicing tied to completion of each subtask component.

Memorandum TO: Geosyntec Consultants, Inc. HW683 Contract File

FROM: Megan Johnson, Contract Manager

DATE: August 15, 2025

SUBJECT: Negotiation Summary – SRP28 Task Assignment (TA) Change Order #1 – TA BF04B C1 Former Ruck’s Park - Site ID #: ERIC_17896 Site Assessment Activities

This Task Assignment Change Order (TACO) settles Work Change Directive (WCD) #1 issued on July 7, 2025. This WCD authorized additional soil and groundwater to be collected at selected locations to confirm previous analytical results, and measure groundwater levels. Specific changes are detailed below:

Subtask 1: Project Management Under this subtask there was an increase in project management hours due to an increase in labor hours needed for the activities under Subtasks 4, 5, and 6. This change resulted in a cost actual increase of $1,125.72.

Subtask 2: Update QAPP/Work Plan and HASP There are no cost changes associated with this Subtask.

Subtask 3: Field Preparation and Off-Site Access There are no cost changes associated with this Subtask.

Subtask 4: Field Work Under this subtask there was an increase of 21 staff labor hours for Geosyntec to collect additional soil and groundwater samples. There was also one (1) additional day usage of a truck, a reduction of subcontractor costs, and an increase in laboratory costs. These changes resulted in an overall cost actual increase of $4,575.57.

Subtask 5: Site Assessment Report Addendum Under this subtask there was an increase of 22 staff labor hours for Geosyntec to update the Site Assessment Report Addendum language, tables, and figures with the additional data collected authorized under the WCD. These changes resulted in a cost actual increase of $3,725.63.

Negotiation Summary – Task Assignment Change Order #1 TA BF04B C1 Former Ruck’s Park – Site ID # ERIC_17896 August 15, 2025

Subtask 6: IDW Disposal Under this subtask there was an increase of three (3) staff labor hours and a reduction in the number of IDW drums needing disposal from the original estimate. These changes resulted in an overall cost actual decrease of ($166.80).

Summary The Department’s SRP Contract Manager and Geosyntec’s Contract Manager have both agreed to the above changes. All changes noted above represent a total overall cost increase of $9,260.12, as shown in the Final/Cost Actual LOE. Due to a rounding error in Excel, the Delta LOE states an overall cost increase of $9,260.11, however this was addressed in the Cost Actual LOE. The changes were noted in the TACO Request received from Geosyntec on August 5, 2025, and August 15, 2025. The final cost associated with this TA increased from $99,981.91 to $109,242.03. Documentation of the changes is included in the attached TACO Request, the Delta LOE spreadsheet and the Cost Actual LOE spreadsheet, and the TACO form for BF04B C1 prepared by the Department.

Hi Megan, Shekhar and I have drafted the TACO 1 files (TACO 1 original, TACO 1 Delta, and TACO 1 Final). There appears to be a 1 penny rounding difference, but I was pleased to get there .

I am attaching these, but was wondering what supporting documents you need. We haven’t gotten the final lab results from the July sampling conducted last week (from Eurofins), but we do have all of the sub invoices from April. I am attaching field notes from both the April and July from the field staff and can send the invoices that we do have, if you need them for your review. If you need the invoices from the July event, the main one we don’t have yet is the lab invoice from July (there will be one more IDW drum pickup for the July sampling, but that is a flat fee of $125/drum – we scoped a total of 15 IDW drums and only needed 9 total, so the math is pretty straightforward).

Other items you might need include the analytical results and COCs, I believe I sent these to you before, but if not, I can forward them. The most complicated part of this is TACO is the lab data changes, and it might be helpful if I run the Delta changes by you. The WCD amount for the July sampling and updated reporting was around $15K and the Delta TACO is about $9,260, so we are below that amount (mainly due to field staff hours for the April sampling was less than what was budgeted for).

Thanks and please let me know on supporting documents you need for TACO review.

Joe

Hi Megan, Please find attached the following invoices:

Earth Tech Drilling GeoTek Services – Utility Clearance Engenuity Group – Surveying Eurofins (April and July invoices)

We will get the last drum picked up as soon as we can so we can invoice Task 6 as well. We are working on the final SARA report and hope to wrap that up by the end of the month.

For the TACO deliverable dates for Task 5 and 6, you might want to bump those dates to 10/1/25, the report date is 9/1 and the IDW date is actually 6/1, so we should bump both of those tasks to 10/1 just in case there are comments to revise SARA, maybe even 11/1?

Thank you and please confirm receipt .

Joe
