HW683 DC017A

This Task Assignment authorizes Geosyntec to conduct additional site assessment activities.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2 –Well Installation and Groundwater Sampling Geosyntec will install one monitoring well at the adjacent property to the east at the approximate location shown on Figure 14 of the SAR to serve as a downgradient monitoring well and potential temporary point of compliance (TPOC). The well will be installed to a depth of approximately 50 feet (ft) below land surface (bls) and will have a 10 ft screened interval from 40 to 50 ft bls and will be completed with an expansion cap and an 8-inch diameter, bolt-down manhole. Following installation, the well will be developed with a submersible pump until the purged water is visibly clear. Following installation and development of the proposed monitoring well, Geosyntec will collect a round of depth-to-water (DTW) measurements at the 24 existing and 1 newly installed wells and sample wells MW- 38, MW-48, GMW- 003, GMW-004, GMW-005, and the newly installed well, GMW-010, for analysis of volatile organic halocarbons (VOHs) by EPA Method 8260B.

Subtask 3 –Vapor Monitoring Point Installations, Soil Sampling, and Modified Active Gas Sampling (MAGS) Testing Vapor Monitoring Point (VMP) Installation: Two (2) VMPs will be installed at the locations shown in Figure 7 to re-test MAGS006S. The VMPs will be installed in nearby locations via hand auger to a maximum depth of 12 to 13 ft bls, if possible, and constructed with approximately 2 to 3 ft of 1-inch diameter Sch. 40 PVC riser and 10 ft of 0.010-inch slotted PVC well screen. Soil samples will be collected for laboratory analysis for VOHs by EPA Method 8260B at intervals 0-0.5 ft, 0.5-2 ft, 2-4 ft, 4- 6 ft, 6 to 8 ft, 8-10 ft, and 12 -14 ft bls. Split samples for SPLP analysis will also be collected and stored at the laboratory pending review of the total VOH results.

MAGS Testing: Based on FDEP’s recommendation and to confirm the previous results, MAGS wells 002S, 003S, 003D, 005S, 005D, 006S and 001D will be re-tested in accordance with the approach outlined in the SAWP. Soil vapor will be monitored with an organic vapor analyzer, and vapor samples will be sent to a fixed-based laboratory for analysis by EPA Method TO-15. Following the completion of MAGS testing, the MAGS wells will be left in place, capped, and covered with a neat cement grout so that they can be converted into soil vapor extraction wells if needed.

MEMORANDUM

Maitland Plaza June 10, 2022 Page Two

Subtask 4 – Investigation Derived Waste Management Geosyntec will review the generation of a waste characterization profile as well as the preparation of IDW manifests. Geosyntec will coordinate disposal arrangements for up to five drums of non-hazardous IDW.

We estimate 3 soil and 2 drums of IDW will be generated. One solid IDW sample and one liquid IDW sample will be analyzed for VOHs to establish the IDW profiles for the Site based on the requirements of the lowest cost provider for transportation and disposal (Erwin Remediation, Inc.).

Subtask 5 – Data Evaluation, Management, and Supplemental Site Assessment Report (SSAR) Preparation Geosyntec will prepare a SSAR to document assessment activities and summarize analytical results. This subtask includes costs to generate a SSAR that summarizes the analytical results of the assessment activities outlined above and complies with the requirements of Chapter 62-780.600 FAC. As part of this subtask, Geosyntec will import and tabulate GIS data in the select electronic data deliverable format.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $40,746.50 with 216 labor hours.

Geosyntec may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Mr. Evan Miller Florida Department of Environmental Protection 2600 Blair Stone Road Tallahassee, FL 32399-2400

Subject: Supplemental Site Assessment Scope of Work and CEF/LOE Maitland Plaza (ERIC_5057) 110 North Orlando Avenue Maitland, Florida 32751

Dear Mr. Miller:

Geosyntec has prepared this proposed Scope of Work (SOW) and Level of Effort (LOE) for additional site assessment activities for the Maitland Plaza facility, located at 110 N. Orlando Avenue, Maitland, Florida 32751 (ERIC_5057). Geosyntec prepared this SOW/LOE in accordance with HW683 and based on recommendations from Florida Department of Environmental Protection (FDEP) following review of the Site Assessment Report (SAR); these comments focused on addressing data gaps in downgradient groundwater and re-testing of select MAGs wells. Figures 5 & 14 of the SAR with the proposed locations have been revised and attached for reference following our discussion. A description of task breakdown for site assessment field activities and report preparation is provided below.

Subtask 1 – Project Management

Subtask 2 –Well Installation and Groundwater Sampling

Geosyntec obtained competitive bids from drilling, private underground utility locators, and investigative derived waste (IDW) subcontractors to complete site assessment activities. Geosyntec personnel will mobilize to the Site to observe a private underground utility survey. We have assumed the work in subtask 2 can be completed during day-time hours (in contrast to Subtask 3).

Geosyntec will install one monitoring well at the adjacent property to the east at the approximate location shown on Figure 14 of the SAR to serve as a downgradient monitoring well and potential temporary point of compliance (TPOC). The well will be installed to a depth of approximately 50 feet (ft) below land surface (bls) and will have a 10 ft screened interval from 40 to 50 ft bls and will be completed with an expansion cap and an 8-inch diameter, bolt-down manhole. Because the well is offsite and no soil contamination is expected, Geosyntec recommends soil sampling only be performed in the final 15 ft of the boring to verify the well is installed above the Hawthorn Group clay, which is expected at approximately 50 ft in the area of the site. This will save both money and time during the installation. Following installation, the well will be developed with a submersible pump until the purged water is visibly clear. All http://www.geosyntec.com/

20 March 2022 soil cuttings, development water, and decontamination fluids will be placed in 55-gallon steel drums, labeled as IDW, and picked up by a licensed hazardous waste hauler for off-site disposal.

Following installation and development of the proposed monitoring well, Geosyntec will collect a round of depth-to-water (DTW) measurements at the 24 existing and 1 newly installed wells and sample wells MW- 38, MW-48, GMW-003, GMW-004, GMW-005, and the newly installed well, GMW-010, for analysis of volatile organic halocarbons (VOHs) by EPA Method 8260B.

Geosyntec has included costs to generate one figure and table that summarize VOH laboratory analytical results from the monitoring wells.

Subtask 3 –Vapor Monitoring Point Installations, Soil Sampling, and Modified Active Gas Sampling (MAGS) Testing

Vapor Monitoring Point (VMP) Installation: Previously installed MAGS well MAGS006S is located at the southern end of the plaza and was too distant from other MAGS wells to determine a radius of influence during the initial testing. Two (2) VMPs will be installed at the locations shown in Figure 7 to re-test MAGS006S. Soil samples will be collected for laboratory analysis for VOHs by EPA Method 8260B at intervals 0-0.5 ft, 0.5-2 ft, 2-4 ft, 4-6 ft, 6 to 8 ft, 8-10 ft, and 12 -14 ft bls.

Split samples for SPLP analysis will also be collected and stored at the laboratory pending review of the total VOH results; for cost estimate purposes, we have assumed analysis of the SPLP leachate from 50% of the soil samples collected (7 total).

Because a step test was previously performed at the Site, the MAGS testing will be completed using the flows recorded from the previous event.

Following the completion of MAGS testing, the MAGS wells will be left in place, capped, and covered with a neat cement grout so that they can be converted into soil vapor extraction wells if needed.

Based on previous communications with the Site owner’s representative, Geosyntec anticipates that the MAGS testing will need to be completed after the plaza is closed. We have assumed three nights for installation and re-testing of the MAGS.

Geosyntec has included costs to generate an updated sample location figure and tables summarizing the radius of influence, and VOH laboratory analytical results from soil samples and vapor samples.

Subtask 4 – Investigation Derived Waste Management

Geosyntec will review the generation of a waste characterization profile as well as the preparation of IDW manifests. Geosyntec will coordinate disposal arrangements for up to five drums of non-hazardous IDW.

Maitland Plaza_ERIC_5057, SARA SOW

We estimate 3 soil and 2 drums of IDW will be generated. One solid IDW sample and one liquid IDW sample will be analyzed for VOHs to establish the IDW profiles for the Site based on the requirements of the lowest cost provider for transportation and disposal (Erwin Remediation, Inc.).

Subtask 5 – Data Evaluation, Management, and Supplemental Site Assessment Report (SSAR) Preparation

Geosyntec will prepare a SSAR to document assessment activities and summarize analytical results. This subtask includes costs to generate a SSAR that summarizes the analytical results of the assessment activities outlined above and complies with the requirements of Chapter 62-780.600 FAC. As part of this subtask, Geosyntec will import and tabulate GIS data in the select electronic data deliverable format.

SUBCONTRACTOR EVALUATION

Geosyntec obtained bids from licensed and qualified drilling, utility locating, and IDW transportation and disposal firms; a cost comparison is provided in Attachment 1 with copies of each of the bids. Geosyntec recommends the following subcontractors based on lowest cost among the qualified bidders:

• Underground utility locating – SiteLines, LLC.

• DPT drilling (soil sampling, monitoring well installation) – Action Environmental provided the overall lowest total cost for the well installation.

• IDW transportation and disposal – Erwin Remediation, Inc.

SITE LINES LLC PROPOSAL

SITE LOCATION

Maitland Plaza Site – 110 N. Orlando Ave, Maitland, FL

PROPOSED SCOPE OF WORK

Site Lines (“Company”) will use Ground Penetrating Radar (GPR) and conventional electromagnetic (EM) pipe and cable locators to determine the location of underground utilities at the site identified by Geosyntec (“Client”).

RESULTS OF THE INVESTIGATION

The estimated depth and location of utilities identified during the investigation will be marked on the ground surface using spray paint or other method to be agreed upon by the parties. Designated utilities will also be documented on a field sketch.

The American Public Works Association Uniform Color Codes for the marking of underground utilities will be used.

PROJECT FEE

The fee to complete the project as described within this proposal is: Seven Hundred Dollars [$700.00].

Client shall pay Company in full within thirty (30) days of the invoice date, unless the parties have agreed to another payment arrangement in writing. If during the investigation, Company determines that the goals of the project cannot be achieved with the resources available to Company, Company will notify Client as soon as reasonably possible. Under such circumstances, Company will charge Client only for time expended and materials used to date.

CLIENT REQUIREMENTS

Client is responsible for all site access, maintenance of traffic (MOT), site clearing and/or any other logistical considerations necessary to conduct the utility locate.

Client will also provide all pertinent maps of the project site, including those that show the boundaries of the utility locate, pertinent landmarks, and known utilities (if available). Company is not a substitute for Chapter 556, Florida Statutes.

Accordingly, Client is responsible for calling Sunshine State One Call of Florida

(811) before undertaking any invasive activities, whether through Company, any other entity, or on its own.

DESCRIPTION OF COMPANY INVESTIGATION AND LIMITATIONS

A GSSI SIR-4000 GPR system will be used with a 900, 400, or 270 megahertz antenna configuration depending on site conditions and desired targets. The GPR will assist in locating nonmetallic and metallic utilities. Physical surface conditions, subsurface conditions and technical limitations effect the ability of GPR to resolve targeted utilities and/or subsurface structures. Depth and/or diameter of the utility, soil type, moisture/ionic content of the soil, and electrical properties of the soil compared to that of the utility can prevent the GPR from identifying utilities at the site. The diameter of a utility cannot be determined by GPR data. Estimated depths of utilities are directly affected by soil conditions and the material of the utility. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities.

An RD 8000 will be used for the EM survey. Electromagnetic induction which transmits a signal directly into the desired target and passive power detection which locates naturally occurring magnetic fields in energized lines will be identified by detecting and following the signals with a receiver. The EM survey will assist in finding only metallic utilities. The magnetic field and/or induced signal should emanate from the target in a circular geometry. When this occurs, the receiver will receive its peak signal directly above the target. However, when metallic utilities are running near each other, or there is environmental interference from fences, cars or overhead power lines, the signal can become distorted and the peak no longer can be directly over the utility. Care must be taken when doing any invasive work around the utilities. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities. Company’s personnel are highly experienced and certified operators of GPR and EM pipe and cable locators.

Due to the limitations described above, when Client is working around indicated utilities, a minimum of two (2)-feet clearance from either side of the marked utility will be maintained. For absolute confirmation, Client is responsible for exposing any indicated underground utility.

Company will conduct utility investigations in accordance with the level of care and generally accepted professional practices as ordinarily exercised by members of the same profession practicing in the same locality under similar conditions.

REPRESENTATIONS AND WARRANTIES

Neither Company nor its employees or agents are licensed surveyors under Florida law, nor do they represent themselves as such. Accordingly, the work Company is to perform is not a substitute for work typically completed by a licensed surveyor.

Limitations in technology, complexity of the physical characteristics of the subsurface conditions, and circumstances beyond Company’s control may limit the performance of Company’s utility investigation. For these reasons, Company expresses no guarantee that all utilities and underground structures will be identified and located during this project.

Company is responsible only for locating underground utilities, the presence of which is both known and indicated on site maps to be provided by Client to Company before Company performs the utility locate. Company will use its best efforts to locate both known and unknown utilities within the project boundaries. However, Company will only be responsible for locating utilities, the presence of which is known before Company undertakes its investigation.

Company expressly disclaims any and all warranties regarding specific or desired results.
