Prior to the completion of supplemental assessment activities, Geosyntec will obtain a signed access agreement with the current property owner, Shreeji 1630, LLC, who purchased the source property associated with the Site from Minal Patel on 19 December 2023 for $350,000. Subtask 2 includes effort to coordinate and obtain a current, signed access agreement for the subject Site.
Conversations with the new property owner indicate that the Site will be redeveloped as a convenient store and the new tenants are scheduled to commence interior redevelopment activities on Saturday, 11 March 2023.
Subtask 1 – Project Management
Subtask 2 – Soil Sampling and Modified Active Gas Sampling (MAGS)
Subtask 2 includes costs for Geosyntec personnel to mobilize to the Site to observe a private underground utility survey as well as locate, uncover, and evaluate historical soil vapor extraction wells for use during the proposed MAGS testing. Geosyntec personnel will also conduct soil sampling at up to 11 locations (SB-14 through SB-23 and MAGS009) as presented on Figure 1.
Based on an assumed depth-to-water (DTW) of 7 feet below land surface (ft BLS), up to five soil samples (0.5, 2, 4, 6, and 8 ft BLS or water table) will be collected from each of the 11 proposed soil sample locations (55 total samples). Soil samples will be collected by hand auger and a concrete core drill will be used to access soil under the building slab (concrete) and driveway/parking lot (asphalt). Samples will be analyzed on a 1-week turnaround time (TAT) for the presence of volatile organic halocarbons (VOHs) via Environmental Protection Agency (EPA) Method 8260 by a National Environmental Laboratory Accreditation Program (NELAP) accredited, fixed-base laboratory. Soil samples will be screened with a photo-ionization detector (PID) organic vapor analyzer. Supplemental soil samples will be collected from each location and depth interval (via Encore samplers) for analysis of VOHs by the synthetic precipitation leaching procedure (SPLP) should VOH concentrations exceed the leachability-based soil cleanup target levels. For budgetary purposes, it is assumed that half (27) of the proposed soil samples will be analyzed by SPLP for VOHs. Exterior soil samples will be surveyed with a global positioning system; interior sampling locations will be transcribed using as-builts based on Site features that will be retained following interior renovations.
Upon completion of soil sampling activities at the boring located near the northern wall of the former boiler room, the boring will be backfilled with a MAGS well. The proposed MAGS well will be constructed of 2-inch (in.) diameter Schedule 40 polyvinyl chloride (PVC), installed by hand auger to a depth of approximately 6 ft BLS (or 1-ft above the static water level if the depth to groundwater is different than historically observed). The MAGS well will be installed with 1-ft of riser and 5-ft of 0.01-in. slotted well screen extending from approximately 1 to 6 ft BLS. A 20/30 grade environmental sand filter pack will be placed from the bottom of the boring (top of water table at approximately 7 ft BLS) to the top of the well screen. The remaining annular space will be grouted to just below the top of casing. A temporary grout patch will be placed on top of a PVC cap that will be placed on each well to match existing grade and protect the well between installation and testing. The MAGS location will be transcribed using as-builts based on Site features that will be retained following interior renovations.
Each of the eight (8) existing historical MAGS wells and one proposed MAGS well (MAGS009) will be tested for a period calculated to remove one, 15-ft radius of soil vapor pore volume using a blower (Figure 2). Each test is expected to take approximately 1 hour. The vacuum and flow rates used at each MAGS well will be based on the results of a step test to be conducted at the new proposed interior MAGS well (MAGS009). The step test will take approximately 1 hour and include a minimum of three steps at increasing vacuums (-15, -25, and -35 inches of water [in- H2O]) to evaluate to evaluate optimum air flow rates. Vacuum will be monitored at adjacent MAGS wells during each individual test at approximate 15-minute intervals, as practical, during MAGS testing.
During testing, soil vapor will be monitored with an organic vapor analyzer equipped with a PID utilizing a 10.6 electronic volt lamp. Soil vapor samples will be collected at the end of each MAGS test using a batch-certified “clean” Summa canister provided by the NELAP accredited analytical laboratory. The soil vapor samples will be sent to a fixed-based laboratory for analysis of VOCs by the EPA TO-15 method on a 1-week TAT.
Geosyntec has included costs to generate one draft figure and draft table that summarize VOH laboratory analytical results from soil samples and one draft figure and draft table that summarize VOH laboratory analytical results from MAGS samples. These data will be provided to FDEP for evaluation and review of the final SVE piping layout within the building interior associated with a future interim source removal design.
Subtask 3 – Groundwater Sampling
Subtask 3 includes costs for Geosyntec personnel to mobilize to the Site to conduct groundwater sampling activities of Site monitoring wells. Prior to sampling, DTW measurements will be collected from Site monitoring wells to evaluate groundwater flow direction. The monitoring wells will be sampled using a peristaltic pump and passive diffusion bags (PDBs) in general accordance with FDEP Standard Operating Procedures (SOPs), effective April 2018. Groundwater samples will be collected as part of natural attenuation monitoring and evaluation of the previously observed per- and polyfluoroalkyl substances (PFAS) groundwater impacts.
NAM Sampling
Groundwater samples will be collected from 18 monitoring wells in the monitoring well network to evaluate the current extent of the contaminant plume and source concentrations. Monitoring wells to be sampled include eight (8) shallow monitoring wells (PZ01 through PZ03, MW01, SMW01 through SMW03, and SMW05), six (6) intermediate monitoring wells (IMW01 through IMW06), and four (4) deep monitoring wells (DMW01 through DMW04) using conventional low-flow purging techniques. The samples will be stored on ice and shipped to a NELAP approved, fixed-based analytical laboratory under chain of custody for analysis of VOHs by EPA Method 8260 under a 1-week TAT.
PFAS Sampling
PFAS screening activities were conducted at the Site in October 2021. Exceedances of the provisional groundwater cleanup target level was observed at monitoring wells MW-01 and IMW-
03. Due to the limited nature of the initial screening efforts (three wells sampled), an expanded PFAS sampling plan has been included in Subtask 3 that includes sample collection from five (5) additional monitoring wells located downgradient and side-gradient of the observed groundwater impacts as outlined in Table 1 below and illustrated on Figure 3.
Table 1. PFAS Screening Sample Locations

IMW-05 2 38-43 Side gradient off-Site, intermediate zone
IMW-06 2 35-40 Downgradient toe off-Site, intermediate zone
PFAS samples will be collected using low-flow purging via a peristaltic pump in accordance with draft SOPs for PFAS sampling developed by FDEP and the SOPs for PFAS sampling prepared by Geosyntec for use at FDEP’s Site Investigation Section firefighting training facilities. PFAS free water will used to decontaminate the water level meter between sampling wells.
Geosyntec understands that the FDEP laboratory will analyze the five (5) groundwater samples for PFAS constituents, and the FDEP will ship the sampling kits to Geosyntec. Based on the location of the Site, Geosyntec anticipates shipping the samples to the FDEP laboratory. Geosyntec assumes that the FDEP laboratory will provide return shipping labels with the sampling kits.
Subtask 4 – IDW Management
Geosyntec will containerize soil and groundwater investigation derived waste (IDW) in two 55-gallon steel drums pending off-Site transportation and disposal. Bid costs prepared by the waste disposal companies are attached (Attachment A). Based on the bids, Geosyntec recommends selecting Hull’s Environmental Services (Hulls) to haul and dispose of IDW under the existing non-hazardous waste profile for VOHs. It is anticipated that the previous provided IDW composite sample results for VOHs by EPA Method 8260B is sufficient for purposes of verifying the existing profile with Hulls. In addition to the VOHs sampled for, the PFAS results will be used to develop a separate PFAS profile. Recent experience with Hulls indicates low-level PFAS detections in groundwater can still warrant disposal as non-hazardous waste under the current profile. If PFAS are detected at significantly higher concentrations, then a work change directive will be prepared to increase disposal costs.
Subtask 5 – Supplemental Site Assessment Report
Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a supplemental site assessment report (SSAR) documenting assessment activities and summarizing analytical results. This subtask includes costs to generate a SSAR that summarizes the analytical results of the assessment activities outlined above and complies with the requirements of Chapter 62-780.600, Florida Administrative Code.
Subtask 6 – PFAS Deliverable
Geosyntec will prepare an email deliverable with attachments including one figure with groundwater analytical results for PFAS (showing PFOA, PFOS, and PFOA + PFOS concentrations for each well sampled and no isoconcentration lines), certified analytical laboratory report, and field forms including equipment calibration logs, groundwater sampling forms, and an IDW drum log. The email deliverable will be submitted to the FDEP within two working days of receipt of the final certified analytical laboratory report from the FDEP laboratory. Geosyntec assumes the final analytical laboratory report will be emailed by the FDEP laboratory to Ann Sava (asava@geosyntec.com) after the report is finalized. The email deliverable will be submitted to Evan Miller, and additional FDEP staff will be copied including Kelly Crain, Aaron Cohen, Jen Farrell, and Breck Dalton.

Senior Geophysicist Florida Professional Geologist Number 2470 Enclosure
Proposal This document shall serve as a proposal for work to be performed by
GeoView Associates, Inc. (GeoView) for GeoSyntec Consultants.
Description of Site
The project site is referred to as the Art Craft Cleaners Site and is located at 1630 University Blvd S in Jacksonville, Florida. The purpose of the geophysical survey will be to determine the location of underground utilities within a 10 feet radius of 11 proposed borings. The location of the study area is provided on Figure 1 (page #6).
Scope of Work The geophysical survey will be conducted using ground penetrating radar
(GPR) and conventional electromagnetic (EM) pipe and cable locators. The GPR survey will be conducted using either a Mala and/or a GSSI GPR system. The appropriate GPR antenna configuration and time range and processing parameters will be determined in the field by the project manager.
The EM survey will be performed using a Subsite 95R and/or a Radiodetection RD (8000, 8100 or 8200) system. As appropriate, the EM survey will be conducted using both passive and active modes. The passive mode is used to find cables and conduits carrying live power (50-60 Hz) and communication (29-33Hz) signals. For the active mode survey, a current is transmitted directly or broadcast into using electromagnetic induction a metal pipe or conduit and the path of the pipe/conduit is identified by following the signal with a portable receiver.
The active mode is used to trace out cast iron pipes, metal water and sewer lines, utilities that have trace wires, and inactive power and communication lines.
Presentation of Results The location and estimated depths of underground utilities or any other identified structures shall be indicated on the ground surface using spray paint or other methods as specified by GeoSyntec Consultants. No report shall be prepared.
Compensation Cost to complete the survey will be $900. If a scaled AutoCAD site map is required to document the study results, an additional fee of $200 will be added.
This price is inclusive of all charges associated with the project. Unless otherwise agreed upon, GeoView shall be compensated for all services within 30 days of invoice date. GeoView shall be paid in full regardless of whether the results of the geophysical survey are what GeoSyntec Consultants anticipated. If it is determined during the survey that the geophysical survey will not achieve the objectives of the project, GeoSyntec Consultants will immediately be notified. If a decision is made to discontinue the survey, only charges for time and materials costs to that point will be submitted.
These prices are based upon GeoView being able to access the site during normal business hours, Monday through Friday. It is also based upon the assumption that no site-specific training or drug testing will be required. If there are any such requirements, additional costs will be incurred.
Requirements of Client GeoSyntec Consultants will provide a scaled map and digital file, if available, of the project site showing the boundaries of the project site, areas of specific concern and pertinent landmarks. GeoSyntec Consultants will also supply all available information regarding the presence and history of underground utilities at the project site (see limitations for further discussion). Unless otherwise requested, GeoSyntec Consultants will be responsible for the coordination of site access, traffic control, clearing of onsite obstructions or any other logistical consideration necessary to conduct the survey. If any test drilling, monitor well installation or other excavation activities are planned it will be the responsibility of GeoSyntec Consultants to submit a notification (dig ticket) to the appropriate 811 One-Call agency. It is recommended that such notification be filed sufficiently in advance so that GeoView will have the opportunity incorporate the 811 findings into their results.
Limitations The objective of the geophysical survey is to determine the presence and configuration of underground utilities. However, the geophysical response of these features may be highly complex depending upon on the physical characteristics of the underground utilities, the electrical properties of the surrounding soils, the depth of burial of the utilities, and the proximity of the utilities to each other.
GeoView shall conduct the geophysical survey using the most “up-to-date” geophysical equipment in a manner consistent with the level of care and skill ordinarily exercised by members of the geophysical profession practicing in the same locality under similar conditions. GeoView’s utility locating personnel are all certified GPR and conventional pipe and cable locator operators with extensive in-house training.
It is acknowledged that GeoView is not an 811 Sunshine One-Call or equivalent utility mark-out service. GeoView does not have access to any utility-related records for either public or private properties. It is the responsibility of GeoSyntec Consultants to obtain such records and provide to GeoView prior to the site investigation. GeoView will use these records during the site investigation and report to GeoSyntec Consultants any utilities that are shown on the site records but cannot be confirmed by GeoView. GeoView will not mark on the ground surface any utilities which it cannot confirm.
GeoView shall only be liable for the successful locating of underground utilities or any other underground structures whose presence is both known and indicated on site maps to be provided prior to the performance of the geophysical survey. Every attempt will be made to locate both known and unknown utilities within the survey area. However, GeoView will only be liable for utilities whose presence is known prior to the survey. GeoSyntec Consultants accepts that due to the inherent limitations of non-intrusive geophysical technologies GeoView cannot be held liable for damages to utilities that cannot identified using non-intrusive technologies.
GeoSyntec Consultants accepts that a non-intrusive utility designation study does not replace the need to employ widely accepted industry safety standards for subsurface work. Concerning the installation of well or test borings this standard of care includes post-holing or hand auguring at the planned test locations to the minimum depth at which underground utilities are anticipated at the project site. If absolute confirmation is needed to a specified depth that underground utilities are not present at a planned work location GeoView can provide vacuum excavation services. Pricing for such services is available upon request.
When GeoSyntec Consultants is working around indicated utilities, a minimum of 2 ft clearance from either side of the marked utility will be maintained. Unless otherwise indicated, a minimum vertical clearance of +/- 20 percent of the indicated depth shall also be maintained. GeoSyntec Consultants maintains the responsibility for manually exposing any indicated underground utility for absolute confirmation. Authorization and scheduling of the project is considered acceptance of these terms and conditions.
Other Terms and Conditions Additional Insured: If requested, GeoSyntec Consultants will be named as an additional insured with respect to the services to be performed under this agreement.
Confidentiality: GeoView shall not directly or indirectly disclose to any third person information regarding the results of the geophysical investigation prior to obtaining written permission from GeoSyntec Consultants.
Agreement: This agreement represents the entire agreement between the parties and may only be modified in writing signed by both parties.
Governing Law: This agreement shall be deemed to have been made in the place of performance of the Geophysical Services and shall be governed by, and construed in accordance with the laws of the state in which the geophysical services were provided. Any controversy or claim arising out of this agreement, or breach thereof, shall be settled by binding arbitration administered by the American Arbitration Association under its Construction Industry Arbitration rules. Judgment on the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof.
Indemnity: GeoView agrees to indemnify, protect and hold harmless GeoSyntec Consultants from and against all liabilities, claims or demands of every kind of injury, including death, or damages to any person or property related in any way to GeoView’s performance of this agreement, except to the extent such liabilities, claims or demands are caused by the negligence or willful misconduct of GeoSyntec Consultants.

General Description of Geophysical Methods Ground Penetrating Radar (GPR)
The GPR survey will be conducted using a Mala or GSSI GPR system. The survey will be conducted with either a 500, 400 or 250 mega-Hertz antenna. The appropriate antenna will be determined in the field by the project manager. GPR will be able to provide both the position and estimated depth of both metallic and non-metallic underground utilities.
The ability of GPR to resolve a particular utility will be controlled by the following factors: 1) Depth of penetration of the GPR signal, 2) Depth of burial and diameter of the underground utility and 3) Electrical contract between the underground utility and the surrounding soils and 4) The complexity of the utility layout at the site. Due to one or a combination of these factors the GPR may not be able to identify all the utilities at the site. The diameter or composition of utilities cannot be accurately determined using GPR, though can be estimated. Estimates of burial depth are typically accurate to within +/- 20 percent, more accurate depths are possible if invert elevations for some on-site utilities are known.
Conventional Electromagnetic Pipe and Cable Locators
A Radiodetection System is used to locate underground utilities using conventional means. The system consists of a dual-function receiver and transmitter. The receiver can be operated in two modes: active and passive. In the passive mode the receiver detects the presence of underground utilities that are energized by a 60-Hertz or other known frequency. These utilities can either be those actually carrying electrical power or those utilities that are both metallic and sufficiently close to the electrical lines to have an electrical field induced within them.
In the active mode, the transmitter deliberately induces an electrical current into the utility. The frequency of this field can be varied from 512 Hertz to 200 kilohertz. The electrical field is induced using a transmitter which is either directly attached to the utility, placed on or above the utility or attached to an induction coil which is placed around the utility. Depths of underground utilities can be determined when the EM equipment is being used in the active mode. Depths are typically accurate to within 10-15 percent of the total utility depth.

The attached figures illustrate the historically observed utilities and the 11 proposed sampling locations for utility locating. We will need a 20-ft by 20-ft area cleared at each location. The building has been gutted and all locations should be sufficiently clear for locating.
I apologize for the short notice but I would really appreciate your bids back by COB Today (31 January 2023). Please also provide your earliest availability to complete the work. The target date for completing the locate is next Monday, 6 February 2023. If unavailable that day, please let me know what days work the rest of this week, if available.
Feel free to contact me with any questions. Thank you for your time!
Thank you, Blaine Dawson, PG (FL,TN)
