HW683_DC032A
This Task Assignment authorizes Geosyntec to conduct for soil gas and groundwater monitoring activities at the Former Coles Cleaners Site in accordance with the Geosyntec scope of work, cost estimate, and schedule received on March 14, 2023.
Geosyntec confirmed on March 14, 2023, that Subtask 1 consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.
Subtask 2: Cost include obtaining a new site access agreement and Quarter 1 groundwater monitoring which is scheduled for March 2023. The samples will be stored on ice and shipped to the analytical laboratory.
Subtask 3: Quarter 2 groundwater and soil gas monitoring is scheduled for June 2023. Soil gas monitoring will also be conducted to assess potential methane and hydrogen sulfide generation beneath the former dry-cleaning building. Air samples will be collected from SGP01, SGP02, and the soil vent fan (SV1) and screened for methane, oxygen, and hydrogen sulfide using a multi-RAE. The samples will be stored on ice and shipped to the analytical laboratory.
Subtask 4: Quarter 3 groundwater monitoring is tentatively scheduled for September 2023. The samples will be stored on ice and shipped to the analytical laboratory.
Subtask 5: Quarter 4 groundwater monitoring is tentatively scheduled for December 2023. The samples will be stored on ice and shipped to the analytical laboratory.
Subtask 6: Geosyntec will prepare an Annual Groundwater Monitoring Report documenting the results of the depth to groundwater measurements and groundwater analytical results during the groundwater monitoring events (Q1 through Q4).
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $39,985.52 with 219 labor hours.

Subject: Scope of Work and CEF/LOE – Groundwater Monitoring Former Coles Cleaners 791 3rd Street N Jacksonville Beach, Duval County, Florida FDEP Facility ID ERIC_4608
Dear Ms. Plyler, Geosyntec Consultants, Inc. (Geosyntec) has prepared this statement of work (SOW), Cost Estimate Form (CEF), and Level of Effort (LOE) for soil gas and groundwater monitoring activities at the Former Coles Cleaners Site (Site) located in Jacksonville Beach, Florida. This below described assessment activities are in accordance with the recommendations presented in the January 2023 Annual Groundwater Monitoring Report (GMR).
On 7 October 2022, Geosyntec identified that a Site Access Agreement was executed with the real property owner (David W. Cole) on 29 November 2004, for the Former Coles Cleaners Site (ERIC_4608). On 5 November 2005, Mr. Cole and his wife, Emma I. Cole, sold the property for $1.00 to No. 3 Commercial, LLC; however, there is no access agreement with this entity, although Geosyntec has been interfacing with Mr. Cole regarding access and notifications since our first involvement in 2015. Subtask 2 includes effort to coordinate and obtain a current, signed access agreement for the subject Site.
Subtask 1 – Project Management
Subtask 2 – Q1 Quarterly Monitoring: Quarter 1 groundwater (UIC and performance) monitoring is tentatively scheduled for March 2023 and will be completed in accordance with Table 1.
The samples will be stored on ice and shipped to the analytical laboratory under chain of custody for analysis under standard turnaround times. Groundwater sampling logs, updated groundwater analytical data tables, and a one-page UIC sampling letter will be submitted to FDEP to document the field work for invoicing.
Coles Cleaners_HW683 SOW_2023 Monitoring
Purge water generated during sampling activities will be placed in a 55-gallon drum and manifested for off-site disposal. IDW will be staged in the rear of the facility. To confirm compliance with the existing non-hazardous IDW groundwater profile with the selected IDW waste hauler (Hull’s Environmental Services), an IDW groundwater sample will be collected for VOHs via EPA Method 8260 from the purge water drum.
Subtask 3 – Q2 Semi-Annual Monitoring: Quarter 2 groundwater (UIC and performance) and soil gas monitoring is scheduled for June 2023 and will be completed in accordance with Table 1.
Consistent with previous sampling events, groundwater level measurements will be collected within a 24-hour period from 22 monitoring wells on the first day of field activities.

Subtask 4 – Q3 Quarterly Monitoring: Quarter 1 groundwater (UIC and performance) monitoring is tentatively scheduled for September 2023 and will be completed in accordance with Table 1.

Subtask 5 – Quarter 4 Annual Monitoring: Quarter 4 groundwater (UIC and performance) monitoring is tentatively scheduled for December 2023 and will be completed in accordance with Table 1. Note that the annual monitoring event includes microbial analyses performed by a specialty analytical laboratory, SiREM; their quote is attached.

Subtask 6 – Groundwater Monitoring Report: Geosyntec will prepare an Annual Groundwater Monitoring Report documenting the results of the depth to groundwater measurements and groundwater analytical results. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the groundwater monitoring events (Q1 through Q4).
