Ms. Sarah Aduwa Florida Department of Environmental Protection Waste Site Cleanup Section 2600 Blair Stone Road, MS 4500 Tallahassee, Florida 32399-2400

Subject: Scope of Work and Level of Effort – Site Assessment Jack Sheehan’s Drive-In Cleaners #2 831 Belvedere Road

Fort Lauderdale, Florida 33405 FDEP Site ID No. ERIC_5221

Dear Ms. Aduwa, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work and Level of Effort (LOE) for site assessment activities at the Jack Sheehan’s Drive-In Cleaner’s #2 site (Site) located in West Palm Beach, Florida (ERIC_5221). Geosyntec prepared this LOE based on Geosyntec’s 31 May 2024 Site Assessment Work Plan (SAWP). A description of task breakdown for Site assessment field activities and report preparation is provided below.

SUBCONTRACTOR EVALUATION

Geosyntec obtained three bids from licensed and qualified utility locating, drilling, and investigation derived waste (IDW) transportation and disposal firms; a cost comparison is provided in Attachment A. Geosyntec recommends the following subcontractors based on lowest cost among the qualified bidders:

• Underground utility locating – GeoTek Services, LLC

• Soil sampling, Modified Active Gas Sampling (MAGS) installation, and direct push technology (DPT) groundwater sampling – JAEE Environmental Service, Inc. (6 total field days)

• Monitoring well (MW) installation – Earth Tech Drilling, Inc. (2 total field days)

• IDW transportation and disposal – Erwin Remediation, Inc.

• MW Surveying – Kugelmann Land Surveying, Inc.

Copies of the bids from each subcontractor are included in Attachment B.

Subtask 1 – Project Management

Tori Hood (Boca Raton) will be the Geosyntec Project Manager. Her labor hours are directly computed.

Subtask 2 – Multi-Media Sampling (Soil, DPT groundwater, soil vapor)

Geosyntec personnel will mobilize to the Site to observe the first phase of site assessment activities, which includes a private underground utility survey; lithology boring; soil sampling;

MAGS well installations, sampling, and testing; DPT groundwater sampling; and installation of the interior monitoring wells.

Health and Safety: Subtask 2 includes labor for field preparation and preparation of a Site Health and Safety Plan (HASP) and associated task hazard analyses.

Utility locating: Geosyntec will observe utility locating at the Site performed by Geotek Services.

Identified utilities will be mapped by Geosyntec staff using a Global Position System (GPS) unit following the private underground utility survey.

Lithology boring: One soil boring (LSB-01) will be advanced using a DPT drill rig to a depth of approximately 50-feet below land surface (ft BLS) or refusal, whichever is encountered first, to characterize the Site-specific lithology. This data will be used to assist in the selection of soil sampling and DPT groundwater sampling intervals. Based on an assumed depth-to-water (DTW) of 8 BLS, up to five (5) soil samples (0-0.5 ft BLS, 0.5-2 ft BLS, 2-4 ft BLS, 4-6, and 6-8 ft BLS or water table) will be collected (five [5] samples total) to assist in assessment.

Soil Sampling: Soil sampling will be performed at up to 11 soil boring locations (SB001 through SB011), as presented on Figure 3 in the submitted SAWP. Based on an assumed DTW of 8 ft BLS, up to five (5) soil samples (0-0.5 ft BLS, 0.5-2 ft BLS, 2-4 ft BLS, 4-6, and 6-8 ft BLS or water table) will be collected from each of the 11 proposed soil sample locations (55 samples total).

Soil samples will be collected by a DPT rig or hand auger and field screened with a photoionization detector (PID). A concrete core drill will be used to access soil under the building slab (concrete) and driveway/parking lot (asphalt). Samples (including samples from the lithology boring) will be analyzed for the presence of volatile organic compounds (VOCs) via Environmental Protection Agency (EPA) Method 8260 by a National Environmental Laboratory Accreditation Program (NELAP)-accredited, fixed-base laboratory. Supplemental soil samples will be collected from each location and depth interval (via Encore or Terra Core samplers) for analysis of VOCs by the synthetic precipitation leaching procedure (SPLP) should VOC concentrations exceed the leachability-based soil cleanup target levels. For budgetary purposes, it is assumed that approximately half of the proposed soil samples (30) will be analyzed by SPLP for VOCs.

MAGS well installation: Six, 2-inch (in.) diameter Schedule 40 polyvinyl chloride (PVC) MAGS wells (MAGS01 through MAGS06) will be installed by hand auger to a depth of approximately 8 ft BLS (or 1-ft above the static water level if the depth to groundwater is different than historically observed). Each MAGS well will be installed with one foot of riser from 7 to 8 ft BLS and 7-ft of 0.010-in. slotted well screen extending from approximately 1 to 7 ft BLS, unless the DTW dictates a modification is warranted. A 20/30 grade sand filter pack will be placed from the bottom of the boring to the top of the well screen. The remaining annular space will be grouted to just below the top of casing.

MAGS well testing: Each MAGS well will be tested for a period calculated to remove one, 10 to 15-ft radius of soil vapor volume using a vacuum blower. The test duration is expected to be approximately 1 hour. The vacuum and flow rates used at each MAGS well will be based on the results of a step test at one interior location. The step test will include a minimum of three steps at increasing vacuums to evaluate optimum airflow rates. Vacuum will be monitored at adjacent MAGS wells at approximate 15-minute intervals, as practical, during MAGS testing. After the completion of the study, the MAGS wells will be left in place, capped, and covered with a flush-mount manhole so that they may be converted into soil vapor extraction (SVE) wells and sampled as necessary in the future.

During testing, soil vapor will be monitored with a PID. Soil vapor samples will be collected at the end of each MAGS test using a batch-certified “clean” Summa canister provided by the analytical laboratory. The soil vapor samples will be sent to a fixed-based laboratory for analysis of VOCs by the TO-15 method.

DPT groundwater sampling: Geosyntec personnel will observe the collection of DPT groundwater samples at up to 8 locations (DPT001 through DPT008), as shown on Figure 3 of the SAWP.

Groundwater samples will be collected at 10-ft intervals below the water table (i.e., 6 to 10 ft BLS, 16 to 20 ft BLS, 26 to 30 ft BLS, etc.) to an estimated 50 ft BLS (or refusal, whichever comes first), resulting in 5 samples x 8 locations (40 samples). Sampling intervals may be adjusted based on observed depth to water and Site lithology. Groundwater samples submitted to a NELAP-accredited fixed-based laboratory will be analyzed for the presence of VOCs by EPA Method 8260 (full list). No field duplicates will be collected.

Interior monitoring well installation: Two (2) monitoring wells (MW001 and MW002) will be installed inside the existing building. The monitoring wells will be advanced using either a hand auger or a DPT drill rig following core drilling of the building slab. The monitoring well target depths will be approximately 15 ft BLS to the extent practicable (e.g., hand augering, if necessary, may not achieve this target depth depending upon subsurface conditions). The wells will be constructed of Schedule 40 PVC with pre-packed screens at approximately 5 to 15 ft BLS.

Field GPS and Interior Mapping: Exterior sampling locations (e.g., DPT groundwater, soil samples) will be surveyed throughout the week with a field GPS by Geosyntec personnel. Interior sampling locations will be transcribed using as-builts based on Site features.

Field Deliverable and Draft Summary Figure: Geosyntec has included costs to manage the data generated during this subtask and to develop the following to facilitate discussion with the Florida Department of Environmental Protection (FDEP):

• One figure and table to summarize VOC laboratory analytical results from the DPT groundwater samples;

• One figure and table to summarize VOC laboratory analytical results from soil samples;

and

• One figure and table to summarize VOC laboratory analytical results from MAGS samples.

These data will be provided to the FDEP for evaluation to select the locations of proposed monitoring wells to be installed in Subtask 3 (current proposed locations are illustrated on Figure 3); data gaps will be addressed during Subtask 3, if practical. If fixed-based laboratory analytical results of soil and DPT groundwater samples indicate that additional sampling will be required to complete delineation, Geosyntec will propose additional sampling locations and corresponding depths. The data will be managed in a database.

Subtask 2 activities will be performed by two (2) Geosyntec staff members assuming one (1) day for utility locating and marking the boring locations (one Geosyntec staff), six (6) days for drilling oversight, one (1) day for MAGS well sampling and testing, and close-out tasks at the Site by two Geosyntec staff. We have assumed up to 10 hours per day not including local round trip mobilization.

Subtask 3 – Exterior Monitoring Well Installations, Monitoring Well Surveying and Groundwater Sampling

Geosyntec personnel will mobilize to the Site to conduct the following.

Exterior Monitoring well installations: Once the final monitoring well strategy has been established with FDEP, Geosyntec will re-mobilize to the Site to observe the installation of up to three (3) exterior water-table monitoring wells to evaluate groundwater quality. We have assumed the utility survey will be sufficient for siting the proposed monitoring well locations; however, should the spray paint be worn away or if there is any uncertainty in utility locations relative to drilling locations, Geosyntec will submit a WCD or TACO to the FDEP for focused utility locating. Water-table monitoring wells proposed in the SAWP (MW002, MW004, and MW005S) will be installed upgradient, cross-gradient, and downgradient of the facility using DPT. The wells will be constructed of Schedule 40 PVC pipe with 10 ft of 0.010-in. slotted well screen, filter pack consisting of 20/30 silica sand, and at least a 0.5-ft thick, fine sand (30/65 silica sand) seal. The remaining annular space will be grouted to land surface with Portland Cement Type I/II and monitoring wells will be completed at the surface with a flush-mount 8-in. manhole and a locking monitoring well cap.

Geosyntec will install one (1) vertical extent monitoring well (MW005D) to a depth of up to 50 feet. The well will be constructed with a 5-ft or 10-ft screen depending upon the lithologic information.

The monitoring wells will be developed until the discharge is relatively sediment-free and sampled no sooner than 24 hours after development.

Monitoring Well Survey: This subtask includes costs for a Florida professional land surveyor to survey the six (6) newly installed monitoring wells. Cost has been included for Geosyntec to oversee the survey.

Groundwater sampling: Geosyntec personnel will collect groundwater samples from the six (6) Site monitoring wells. DTW measurements will be collected from the monitoring wells to evaluate groundwater flow direction. The monitoring wells will be sampled using a peristaltic pump in general accordance with the FDEP Standard Operating Procedures (SOPs), assuming the DTW and drawdown do not exceed the suction limit of the peristaltic pump. The samples will be stored on ice and shipped to a NELAP-accredited, fixed-based analytical laboratory under chain of custody for analysis under standard turnaround times. The groundwater samples will be analyzed for VOCs by EPA Method 8260.

IDW Sampling: One composite liquid and one composite soil sample will be collected and analyzed for VOCs and metals using the Toxic Characteristic Leaching Procedure (TCLP) for IDW characterization.

Subtask 3 activities will be performed by one (1) Geosyntec staff member assuming two (2) days for drilling oversight and two (2) days for well sampling, IDW sampling, and one (1) day for surveying oversight and close out tasks at the Site. We have assumed up to 10 hours per field day not including round trip mobilizations.

Field Deliverable and Draft Summary Figure: Geosyntec has included costs to generate 3 figures (final site map, groundwater results figure, groundwater elevation contour figure) and tables that summarize VOC laboratory analytical results from groundwater monitoring well samples, the water quality parameter data, monitoring well construction tables, and groundwater elevation data.

The deliverable will include the final surveyor report, monitoring well installation field forms, and groundwater sampling forms.

Subtask 4 – IDW Management Geosyntec will review the generation of a waste characterization profile as well as the preparation of IDW manifests. Geosyntec will coordinate disposal arrangements for up to 18 drums of non-hazardous IDW (based on driller’s estimate of drum quantities). Analytical costs are included to establish the soil and groundwater profiles required by the selected waste hauler. Due to the quantity of drums anticipated, we have assumed field oversight of the drum pickup is necessary.

For this cost estimate, we have assumed the IDW will not be impacted by PFAS (screening to be performed under separate TA) and Geosyntec will submit a WCD or TACO to the FDEP for any change of scope.

Subtask 5 – Site Assessment Report

Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a Site Assessment Report (SAR) documenting assessment activities and summarizing analytical results. This subtask includes costs to generate a SAR that summarizes the analytical results of the assessment activities outlined above and complies with the requirements of Chapter 62-780.600, FAC. LOE costing includes the following figures:

USGS Site Topographic Map (includes potable wells identified from online sources with addresses/coordinates that can be mapped within only a 0.5-mile radius of the Site)

• USGS Site Topographic Map

• Potable Well Survey

• Site Layout

• Site Utility Map

• Lithologic Cross Section (1 transect)

• Summary of Soil Analytical Results from 0-0.5, 0.5-2, 2-4, 4-6, and 6-8 ft BLS intervals until the water table (assuming DTW at 8 ft BLS, 5 figures)

• Summary of DPT Groundwater Sampling Analytical Results

• Summary of Groundwater Elevations

• Summary of Monitoring Well Analytical Results

The SAR will include recommendations for remediation, further assessment, or closure based on the data. The SAR will be signed/sealed by a FL professional geologist or engineer.

RE: Utility Locating Services – Jack Sheehan #2, WPB, Florida

Ms. Hood, GeoTek Services is pleased to respond to your request for Utility Locating services at Jack Sheehan #2 located in West Palm Beach, Florida. We will scan up to 8,000 sq.ft. with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as vehicles, structures, obstructions. We will scan as close as reasonably possible to fixed objects such as curbs, landscaping, or obstructions, etc. Our deliverable will be marking paint or flags indicating the horizontal location of potential underground utilities.

It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. We also request all grassy areas to be mowed prior to scanning. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Best Regards, Martin (Bud) Connor GeoTek Services, LLC

Customer has requested an estimate for the following scope of work:

• Client has requested an estimate to horizontally locate detectable underground utilities within a 5-10' radius of multiple proposed soil bores, both interior and exterior, in areas as shown on the provided exhibit and as directed on site. To also include time for Hand Held Concrete Scanning (GPR) to determine what may be within the slab of the interior locations.

· The estimate portion for concrete scanning includes horizontally locating detectable under surface utilities and anomalies including, conduit, post tension, rebar, and wire mesh.

· Real time adjustments will be made on site to maximize signal…
