Task Assignment Summary

This Task Assignment (TA) DC041C is executed between the State of Florida Department of Environmental Protection (FDEP) and Geosyntec Consultants, Inc. under Contract Number HW683. The TA authorizes Geosyntec to conduct annual groundwater monitoring and PFAS screening activities at the J.T.J. Laundromat facility located at 9784 66th Street North, Pinellas Park, Pinellas County, Florida (FDEP Site ID ERIC_5346). The project is managed by Sarah Aduwa for FDEP and Nick Barnes for Geosyntec, with a completion date of December 15, 2025.

The scope of work is divided into four subtasks: (1) Project Management, calculated at 13% of direct labor; (2) Groundwater Sample Collection involving volatile organic halocarbon (VOH) analysis from nine monitoring wells using EPA Method 8260; (3) preparation of an Annual Groundwater Monitoring Report with conclusions and recommendations; and (4) PFAS Screening Activities including sampling from four specified monitoring wells (MW001, MW004, MW005, MW007) with analysis by FDEP Central Laboratory. The total TA value is $11,913.63 on a fixed unit rate basis, with labor hours totaling 62 hours. Invoicing is structured in two phases: $10,183.40 upon FDEP receipt and review of the Groundwater Monitoring Report and IDW Manifest (due October 15, 2025), and $1,730.23 upon receipt and review of the PFAS Screening Deliverable (due September 15, 2025). Purge water generated during monitoring is anticipated to require disposal as PFAS-containing waste through Erwin Remediation at $350 profiling fee plus $195 per drum for disposal.

Ms. Sarah Aduwa Florida Department of Environmental Protection Waste Site Cleanup Section 2600 Blair Stone Road, MS 4520 Tallahassee, Florida 32399-2400

Subject: Scope of Work and Level of Effort Annual Groundwater Monitoring and PFAS Screening JTJ Laundromat

9784 66th Street North Pinellas Park, Florida 33782 FDEP Site ID No. ERIC_5346

Dear Ms. Aduwa, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW), Cost Estimating Form (CEF), and Level of Effort (LOE) to conduct annual groundwater monitoring concurrent with PFAS screening activities at the JTJ Laundromat facility (herein, the “Site”) located in Pinellas Park, Florida. The existing Site Access agreement remains accurate, with no change in property ownership. A description of task breakdown for groundwater sampling activities and report preparation is provided below. A copy of the CEF/LOE is included in Attachment A.

Subtask 1 – Project Management

Costs associated with this subtask are directly computed.

Subtask 2 – Groundwater Sample Collection (Volatile Organic Halogens)

Geosyntec will collect a sample from each of the 9 groundwater samples monitoring wells previously installed at the Site. Prior to groundwater sampling, Geosyntec will gauge and record the depth-to-water (DTW) from these monitoring wells within a 24-hour period.

Geosyntec assumes that a peristaltic pump will be used to collect groundwater samples from the monitoring wells, based on the previous assessment activities. Field activities will be conducted in accordance with the FDEP Standard Operating Procedures. Groundwater samples will be submitted under chain of custody protocol for laboratory analysis of volatile organic halocarbons (VOHs) by Environmental Protection Agency (EPA) Method 8260.

Ms. Sarah Aduwa June 2, 2025

Purge water generated during the groundwater monitoring activities will be stored in a 55-gallon drum on-Site prior to off-Site disposal. For planning purposes, it is expected that the IDW will need to be disposed of as PFAS-containing waste; if PFAS constituents are not detected, then a change order will be submitted to reduce the disposal cost. Geosyntec recommends selecting Erwin Remediation to haul and dispose of IDW generated during the groundwater monitoring activities based on the transportation and disposal quotes in Attachment A.

Subtask 3 – Annual Groundwater Monitoring Report (VOHs)

Geosyntec will prepare a Groundwater Monitoring Report documenting the VOH results of the groundwater monitoring event. The report will contain a summary of the field activities completed, tables summarizing field and laboratory groundwater data, figures showing groundwater flow direction(s) and groundwater analytical results, conclusions, and recommendations. Note, there is not yet sufficient data to perform non-parametric statistical analysis of VOH concentration stability. A final signed waste manifest for IDW transport and disposal will also be included in the Groundwater Monitoring Report. The report will be signed and sealed by a Florida-licensed professional geologist or engineer.

Subtask 4 –PFAS Screening Activities

Subtask 4 includes costs associated with PFAS groundwater screening. During the Subtask 2 sampling activities, groundwater samples are proposed to be collected from four surficial aquifer monitoring wells including MW001, MW004, MW005, and MW007 based on the VOH analytical results and groundwater flow direction to the north (see attached Figure) for PFAS screening.

Please see the table below for additional details on the proposed wells.

Well ID Diameter (inches)

Screen Interval (ft BLS)

Rationale/Location

MW001 1 3.5 – 8.5 Interior MW, closest to former drycleaning machine

MW004 1 5-15 North of building, in VOH groundwater plume

MW005 1 5-15

North of building, in VOH groundwater plume with highest VOH results

MW007 1 5-15 West of building, just outside rear of former facility

Groundwater samples will be collected in accordance with draft Standard Operating Procedures (SOPs) for PFAS sampling that were developed by FDEP and the SOPs for PFAS sampling that

Ms. Sarah Aduwa June 2, 2025 were prepared by Geosyntec for use at FDEP’s Site Investigation Section firefighting training facilities.

Geosyntec assumes that the PFAS samples will be analyzed by the FDEP Central Laboratory.

Geosyntec understands that the FDEP laboratory will analyze up to four groundwater samples for PFAS, provide 1 liter of PFAS-free water for an equipment blank, and ship the sampling kits to Geosyntec. Geosyntec assumes that the FDEP laboratory will provide return shipping labels with the sampling kits.

Within 48-hours of receipt and review of the analytical data, Geosyntec will prepare a PFAS Screening Deliverable documenting the results of the groundwater monitoring event. The deliverable will include:

An email summary of the field activities and results;

Groundwater sampling field forms;

A figure illustrating the analytical results for PFOA, PFOS, and the sum of PFOA &PFOS;

The FDEP analytical laboratory report.

Deliverable and Invoicing Schedule:

Subtask Deliverable Due Date Invoice Amount

4 PFAS Screening Deliverable September 15, 2025* $1,730.23

1, 2, 3 Groundwater Monitoring Report and IDW Manifest

October 15, 2025 $10,183.40

Total $11,913.63

The above schedule assumes the Task Assignment is received by July 2025.

*Due date will be adjusted based on actual turnaround time from the DEP Central Lab.

Geosyntec appreciates the opportunity to provide this proposed SOW/LOE. Please contact us if you have any questions or require additional information.

Sincerely, Nicholas S. Barnes P.E. (FL) Todd K. Kafka, P.G. (FL, TN)
