Ms. Sarah Aduwa Florida Department of Environmental Protection Waste Site Cleanup Section 2600 Blair Stone Road Tallahassee, FL 32399-2400

Subject: Scope of Work and Level of Effort Annual Monitoring Former Bonos Cleaners (ERIC_4621) 1105 San Marco Blvd Jacksonville, Duval County, Florida

Dear Ms. Aduwa, Geosyntec Consultants, Inc. has prepared the following Scope of Work (SOW) and Level of Effort (LOE) for annual groundwater monitoring activities at the Former Bonos Cleaners (ERIC_4621, the Site) located in Jacksonville. A copy of the CEF and LOE is provided as Attachment A.

Annual VOH monitoring proposed in this SOW is consistent with the recommendations provided in the January 2025 Groundwater Monitoring Report and PFAS Desktop Evaluation. Additional groundwater monitoring for per- and polyfluoroalkyl substances (PFAS) has also been proposed in this SOW in accordance with the September 2024 guidance provide by the Florida Department of Environmental Protection.

On August 2, 2024, Geosyntec confirmed the Site Access Agreement (SAA) was executed with SLG Investment Partnership, LLP, on 10 September 2015 as the real property owner of the Former Bonos Cleaners Site (ERIC_4621) located at the above address. The Duval County Property Appraiser database indicates the property owner since 19 December 2007 is SLG Investment Partnership, LLP, and that the property has not changed ownership since then. As a result, Geosyntec believes the current SAA remains in force.

Subtask 1 Project Management

Blaine Dawson, Senior Geologist, will be the project manager (PM), and his PM hours are directly computed.

Ms. Sarah Aduwa 1 August 2025

ERIC_4621 - Bonos Cleaners - Annual Monitoring SOW_2025

Subtask 2 Groundwater Sampling

Geosyntec will conduct underground injection control (UIC) and VOH groundwater monitoring in accordance with the schedule in Table 1. Depth-to-groundwater (DTW) measurements will be collected from accessible Site monitoring wells (24) within a 24-hour period prior to sampling.

VOH groundwater samples (24) will be collected from MW001 through MW019 and injection wells IW001 through IW005. UIC groundwater samples will be collected from MW001, MW002, and MW007 and submitted to a fixed-base laboratory for analysis of iron. Additionally, monitoring wells MW001, MW016, and IW005 will be sampled for dissolved gases to evaluate the degradation progress of the remaining VOH groundwater contamination. The monitoring wells will be sampled using a peristaltic pump in general accordance with the Florida Department of Environmental Protection Standard Operating Procedures. The samples will be stored on ice and shipped to the analytical laboratory under chain of custody for analysis under standard turnaround times.

Sampling of MW011-MW014 and MW016-MW017 will require maintenance of traffic (MOT) equipment in the roadway south of the Site building. MOT equipment will be provided by Acme Barricades who has previously performed this service at the Site and shown to provide best value to FDEP. The Acme estimate is attached to this SOW (Attachment A).

Purge water generated during sampling activities will be placed in a 55-gallon drum and manifested for off-Site disposal. Investigation derived waste (IDW) will be staged in the rear of the facility. Historical analytical results (VOHs and PFAS) indicate IDW has been non-hazardous.

Geosyntec has assumed disposal of the IDW drum as non-hazardous; however, if concentrations in the waste profiling sample indicate exceedance of hazardous limits, a change order will be submitted for the additional disposal costs. Bid costs prepared by the waste disposal companies are included in Attachment A.

Subtask 3 - Groundwater Monitoring Report

Geosyntec will prepare an Annual Groundwater Monitoring Report documenting the results of the depth to groundwater measurements and chlorinated solvent concentrations. The report will contain tables summarizing field/laboratory groundwater data, two figures showing groundwater flow direction(s), one groundwater results figure, and a summary of the groundwater monitoring event. Geosyntec will evaluate plume stability using Mann-Kendall statistical analysis and the report will be signed and sealed by a Florida-licensed professional geologist. A final signed waste manifest for IDW transport and disposal will also be included in the Groundwater Monitoring Report.

ERIC_4621 - Bonos Cleaners - Annual Monitoring SOW_2025

Subtask 4 PFAS Sampling and Reporting

Geosyntec will conduct PFAS groundwater monitoring activities concurrently with those described in Subtask 2. These sampling activities are proposed to evaluate the stability of the PFAS groundwater impacts observed during the annual groundwater monitoring activities completed in August 2024.

PFAS samples will be collected using low-flow purging via a peristaltic pump in accordance with draft SOPs for PFAS sampling developed by FDEP and the SOPs for PFAS sampling prepared by Geosyntec for use at FDEPs Site Investigation Section firefighting training facilities. PFAS free water will used to decontaminate the water level meter between sampling wells.

Geosyntec understands that the FDEP laboratory will analyze the 24 groundwater samples for PFAS constituents, and the FDEP will ship the sampling kits to Geosyntec. Based on the location of the Site, Geosyntec anticipates shipping the samples to the FDEP laboratory. Geosyntec assumes that the FDEP laboratory will provide return shipping labels with the sampling kits.

The PFAS analytical results will be presented within the annual groundwater monitoring report included in Subtask 3.

Good evening John, I was hoping you could provide a quote for a day rate inclusive of setup/take down of approximately 200 ft of MOT 602 along Gary St in Jacksonville (southern side of property located at 1105 San Marco Blvd, Jacksonville, FL)? We will need the lane closest to the site building on Gary Street temporarily shut down while we do work within the roadway. If possible, I would really appreciate an updated quote back by COB Friday, 30 May 2025. We are tentatively scheduled to complete the referenced effort the last week of August 2025.

Let me know if you have any questions or if you need anything else from me. I appreciate the time and support on this matter!

Thank you, Blaine Dawson, PG (FL,TN) Senior Geologist

AWP Safety Equipment Rental Terms and Conditions

1. The rental of all equipment by you (the “Lessee”) from AWP, Inc. DBA Area Wide Protective, Inc. (the “Lessor”) is subject to the terms and conditions of this Lease Agreement (this “Agreement” or this “Lease Agreement”).

2. The Lessee shall not remove, alter, disfigure or cover up any numbering, lettering or insignia displayed upon any equipment and shall ensure that no equipment is subjected to careless or needlessly rough usage, and shall, at the Lessee’s own expense, maintain and ultimately return to the Lessor the equipment and its appurtenances, together with any tools and accessories pertaining thereto, the whole in good repair and running order. The Lessee is responsible for all routine maintenance and repair. Without limiting the generality of the foregoing, the Lessee shall, at the Lessee’s own expense, during the term of this Lease, pay the cost of; (a) all lubricants required to operate the equipment; (b) all repairs required to be made to the equipment to maintain the equipment in good and proper operating condition in accordance with the operator’s manual; and (c) replacing broken or worn-out parts.

The Lessee shall indemnify and hold harmless the Lessor against all loss and damage to the equipment during the term of this Lease. Without limiting the foregoing, the Lessee shall indemnify and hold harmless the Lessor against all loss, expenses, penalties, damages, condemnations and legal costs which the Lessor may suffer or may be required or condemned to pay for personal injuries (including death) and/or property damages suffered by any person because of the equipment or the operation, handling, transportation or use of the equipment by or while in the hands of the Lessee or the Lessee’s employees, agents, representatives or carriers. The Lessee shall remain responsible for the equipment until it has been returned to the Lessor (or in the case of loss or theft of the equipment until the total replacement value of the equipment has been provided to the Lessor, as calculated in accordance with the other provisions of these terms and conditions). The Lessee shall maintain at the Lessee’s own expense, public liability and all risk property insurance in adequate amounts to fully cover this indemnity. The Lessee hereby renounces all claims against the Lessor for any loss or damage it may suffer, either directly or indirectly, by reason of the condition of the equipment or its suitability for the work Lessee performs.

4. The Lessee agrees to pay, upon delivery, all monies as provided for in this Agreement and that in the event of non-payment by Lessee, Lessee agrees to pay all collection fees, including reasonable attorneys fees and costs incurred in the collection of unpaid monies.

5. Without limiting the other provisions of this Agreement, the Lessee shall insure the equipment against all losses or damage of any kind whatsoever in: (a) an amount to be calculated in this Agreement; or (b) the amount of the equipment’s full replacement cost, and hereby assigns to the Lessor all such policies of insurance and the proceeds therefrom. Replacement cost shall, for the purposes of this Agreement, be deemed to include disassembly, replacement and inspection of the equipment and/or loss of rental income and all other associated costs, excluding a deduction for depreciation.

6. The Lessee agrees that in the event of damages, loss or theft of the equipment, and whether or not through the negligence of the Lessee and/or its servants, agents, employees, or other users and whether or not acting within the scope of their employment, the Lessee will pay the Lessor the fair market value cost of the equipment and rental charges shall continue until the equipment is returned to the Lessor, except in the case of loss or theft of the equipment, rental charges shall only continue until the Lessor receives the fair market value cost.

It is agreed by the parties that the rental period shall include the duration of time when the equipment is being transported to and from the Lessor’s premises. AWP Safety - Page 2 of 3

1. The provisions of this Agreement shall be governed, construed and enforced in accordance with the laws of the State of Ohio. The parties consent to the exclusive jurisdiction of the Stark County Court of Common Pleas for any actions, suits or proceedings arising out of or relating to this quote or the transactions contemplated by this Agreement. The parties agree that any action arising from or related to this quote shall not be subject to removal to federal court for any reason and each party irrevocably waives any such right. Both parties agree that any such solicitation, offer or employment of any employee who performed services under this Agreement would cause great or irreparable harm to each party and that each party would be damaged in an amount difficult to ascertain, but which would likely exceed double the annual compensation of the employee solicited (or former employee as the case may be) representing the cost of training a new employee. Accordingly, each party agrees to pay the other, as liquidated damages, an amount equal to double the solicited employee’s (or former employee’s) annual compensation.
