From: Sarah Aduwa, Project Manager Date: November 6, 2025 Subject: Negotiation Summary

HW683_DC037C

This Task Assignment authorizes Geosyntec to conduct Groundwater Monitoring and PFAS sampling at Gentle Touch Dry Cleaners site in accordance Geosyntec’s scope of work, cost estimate and schedule received on November 4, 2025.

Nick Barnes is the Project Manager.

Subtask 2 and 3 – Geosyntec will collect a sample from each of the 10 groundwater samples monitoring wells previously installed at the Site. Prior to groundwater sampling, Geosyntec will gauge and record the depth-to-water (DTW) from these 10 monitoring wells within a 24-hour period. The samples will be sent to an analytical laboratory for volatile organic hydrocarbon analysis via EPA Method 8260. Geosyntec will prepare a comprehensive Natural Attenuation Monitoring (NAM) Report that details the findings and outcomes of the recent NAM event. The report will include well-organized tables presenting field and laboratory groundwater data, and figures illustrating groundwater flow directions and VOH results. A concise summary of all field activities conducted will be provided.

Subtask 4 - Subtask 4 includes costs associated with PFAS groundwater screening.

During the Subtask 2 sampling activities, groundwater samples are proposed to be collected from three surficial aquifer monitoring wells including MW001, MW002, and MW004 based on the VOH analytical results and groundwater flow direction generally to the south for PFAS, screening.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $12,799.97 with 69 labor hours.

From: Goudarzi, Mohammad Sent: Friday, September 26, 2025 12:25 PM To: Aduwa, Sarah Cc: Darji, Mubeen Subject: RE: Presumptive Remedy Form for Gentle Touch Dry Cleaners ERIC 4712

Sarah, I have reviewed the MAGS data and previous documents/emails for Gentle Touch Dry Cleaners (ERIC 4712) and below are my comments:

1. Based on the DTW in MW001 (4.28 ft) and other monitoring wells, and considering the 35 IW vacuum applied, it appears that water entrainment may have occurred in most of the MAGS wells, since they are screened from 1-4 ft bgs. Additionally, the MAGS step test performed was a step-down and not a step-up test (from low to high vacuum). Geosyntec should consider conducting another round of MAGs testing on select wells and specify the vacuum level to be applied. Moving forward please make sure that the MAGS are screened about 1 ft above the water table and the MAGS step testing is a step-up test.

2. It would be bene cial to complete another round of GW sampling, as the last sampling event was conducted in 2023.

Let me know if you have any questions.

Subject: Scope of Work and Level of Effort Annual Groundwater Monitoring and PFAS Screening Gentle Touch Dry Cleaners

3611 W Hillsborough Avenue (formerly 3613 W Hillsborough Avenue) Tampa, Florida 33614 FDEP Site ID No. ERIC_4712

Dear Ms. Aduwa, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW), Cost Estimating Form (CEF), and Level of Effort (LOE) to conduct annual groundwater monitoring concurrent with PFAS screening activities at the Gentle Touch Dry Cleaners facility (herein, the “Site”) located in Tampa, Florida. A description of task breakdown for groundwater sampling activities and report preparation is provided below. A copy of the CEF/LOE is included in Attachment A.

The property ownership information on the Hillsborough County Property Appraiser’s website and the Permission to Enter Property Form have been reviewed, with the current Permission to Enter Property form remaining valid.

Subtask 1 – Project Management

Costs associated with this subtask are directly computed for Nick Barnes, Project Manager.

Subtask 2 – Groundwater Sample Collection (Volatile Organic Halogens)

Geosyntec will collect a sample from each of the 10 groundwater samples monitoring wells previously installed at the Site. Prior to groundwater sampling, Geosyntec will gauge and record the depth-to-water (DTW) from these 10 monitoring wells within a 24-hour period.

Geosyntec assumes that a peristaltic pump will be used to collect groundwater samples from the monitoring wells, based on the previous assessment activities. Field activities will be conducted in accordance with the FDEP Standard Operating Procedures. Groundwater samples will be submitted under chain of custody protocol for laboratory analysis of volatile organic halocarbons (VOHs) by Environmental Protection Agency (EPA) Method 8260.

Purge water generated during the groundwater monitoring activities will be stored in a 55-gallon drum on-Site prior to off-Site disposal.

Subtask 3 – Annual Groundwater Monitoring Report (VOHs)

Geosyntec will prepare a Groundwater Monitoring Report documenting the VOH results of the groundwater monitoring event. The report will contain a summary of the field activities completed, tables summarizing field and laboratory groundwater data, figures showing groundwater flow direction(s) and groundwater analytical results, conclusions, and recommendations. The report will be signed and sealed by a Florida-licensed professional geologist or engineer.

Subtask 4 –PFAS Screening Activities

Subtask 4 includes costs associated with PFAS groundwater screening. During the Subtask 2 sampling activities, groundwater samples are proposed to be collected from three surficial aquifer monitoring wells including MW001, MW002, and MW004 based on the VOH analytical results and groundwater flow direction generally to the south (see attached Figure) for PFAS screening.

Please see the table below for additional details on the proposed wells.

Well ID Diameter (inches)

Screen Interval (ft BLS) Rationale/Location

MW001 1 7-12 Interior MW, likely closest to former drycleaning machine

MW002 1 5-15 North of building, outside rear door of former drycleaner, in VOH groundwater plume

MW004 1 5-15 South of building, in VOH groundwater plume

Groundwater samples will be collected in accordance with draft Standard Operating Procedures (SOPs) for PFAS sampling that were developed by FDEP and the SOPs for PFAS sampling that were prepared by Geosyntec for use at FDEP’s Site Investigation Section firefighting training facilities.

Geosyntec assumes that the PFAS samples will be analyzed by the FDEP Central Laboratory.

Geosyntec understands that the FDEP laboratory will analyze up to three groundwater samples for PFAS, provide 1 liter of PFAS-free water for an equipment blank, and ship the sampling kits to Geosyntec. Geosyntec assumes that the FDEP laboratory will provide return shipping labels with the sampling kits.

Upon receipt and review of the analytical data, Geosyntec will prepare a PFAS Screening Deliverable documenting the results of the groundwater monitoring event. The deliverable will include the information below and will be submitted via electronic mail to you, Breck Dalton, Jennifer Farrell, Mubeen Darij, Safa Shariat, and Doris Brown:

An email summary of the field activities and results;

Groundwater sampling field forms;

A figure illustrating the analytical results for PFOA, PFOS, and the sum of PFOA &

PFOS;

The FDEP analytical laboratory report.

IDW disposal quotes prepared by waste disposal companies are included in Attachment A. Based on the quotes, Geosyntec recommends selecting Erwin Remediation to haul and dispose of IDW generated during the groundwater monitoring activities. For planning purposes, it is expected that the IDW will need to be disposed of as PFAS-containing waste. Bid costs prepared by waste disposal companies are included in Attachment A. Based on the bids, Geosyntec recommends selecting Erwin Remediation to haul and dispose of IDW generated during the groundwater monitoring activities.

For planning purposes, it is expected that the IDW will need to be disposed of as PFAS-containing waste. As indicated above, Geosyntec recommends selecting Erwin Remediation to haul and dispose of IDW generated during the groundwater monitoring activities. The subcontractor costs for IDW disposal are included within Subtask 4.

Deliverable and Invoicing Schedule:

Subtask Deliverable Due Date Invoice Amount

2 Field Deliverable – Groundwater Monitoring

January 26, 2026 $5,295.01

3 Groundwater Monitoring Report February 26, 2026 $4,189.17

1, 4 PFAS Screening Deliverable and IDW Manifest

March 16, 2026 $3,315.79

Total $12,799.97 The above schedule assumes the Task Assignment is received in November 2025.

Geosyntec appreciates the opportunity to provide this proposed SOW/LOE. Please contact us if you have any questions or require additional information.

Sincerely, Nicholas S. Barnes P.E. (FL) Todd K. Kafka, P.G. (FL, TN)
