From: Sarah Aduwa, Project Manager Date: February 20, 2024 Subject: Negotiation Summary

HW685 DC030B

This Task Assignment authorizes Geosyntec to conduct Supplemental Site Assessment at the A1A Cleaners site in accordance with Geosyntec scope of work, cost estimate and schedule received on February 19, 2024. The assessment is consistent with the path forward discussions held during the Geosyntec’s annual site review meeting with FDEP held on September 19, 2023.

Subtask 2: Monitoring Well Installations Geosyntec proposes the installation of five (5) water table monitoring wells (MW022-MW023) to define the extent of the current groundwater impacts. The wells will be installed using the hollow stem auger drilling method and constructed of 2-inch (in) diameter Schedule-40 polyvinyl chloride (PVC) pipe with 10-feet (ft) of 0.010-in standard factory-slotted screen. Each monitoring well will be developed until the discharge is relatively sediment-free. The monitoring well locations and top of casing elevations will be surveyed by a licensed Florida surveyor. Drill cuttings, decontamination water, and development water generated during well installation activities will be placed in 55-gallon drums and manifested for off-site disposal.

Subtask 3: Groundwater Monitoring Subtask 3 includes costs associated with the collection of groundwater samples from 29 monitoring wells including the five (5) additional monitoring wells installed as part of Subtask 2.

Performance groundwater samples (29) will be collected from MW001 through MW019, MW021 through MW026, EW001, and PZ001 through PZ003 and analyzed for VOHs by EPA Method 8260B. The monitoring wells will be sampled using a peristaltic pump in general accordance with FDEP Standard Operating Procedures (SOPs). Purge water generated during sampling activities will be placed in a 55-gallon drum and manifested for off-site disposal.

Subtask 4 – Supplemental Site Assessment Report Geosyntec will prepare a Supplemental Site Assessment Report documenting the results of the monitoring well installation, depth to groundwater measurements, and groundwater sampling

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $45,539.46 with 154 labor hour.

On 31 January 2024, Geosyntec confirmed that a Site Access Agreement (SAA) was executed with Cook Family Shopping Center, LLC, on 18 April 2023 as the real property owner of the A1A Cleaners Site (ERIC_4937) located at the above address. The Nassau County property appraiser database indicates the property owner since 3 December 2015 is “Cook Family Shopping Center, LLC,” and that the property has not changed ownership since then. As a result, Geosyntec believes the current SAA remains in force.

FDEP’s Site Assessment Report (SAR) comment memorandum dated 3 March 2015, indicates concurrence with Geosyntec’s conclusion that the contamination has been adequately delineated.

The SAR concluded there were no observed soil impacts and groundwater impacts remained undelineated upgradient and downgradient of the source area. The effort presented within this SOW/LOE is to pursue completion of Site Assessment according to Chapter 62-780, Florida Administrative Code.

Subtask 1 – Project Management

Subtask 2 – Monitoring Well Installations

Prior to well installation activities, Geosyntec will mobilize to the Site to mark/stake the well locations and observe a private utility locate. This mobilization will occur separately from the well installation mobilization as a means to evaluate access for the drill rig and support vehicles to the well locations in the wooded areas of the park. Should any vegetation clearing efforts be required, Geosyntec will submit a work change directive for any additional costs in support of completing well installation activities.

Geosyntec proposes the installation of five (5) water-table monitoring wells (MW022-MW026) to refine the extents of current groundwater impacts. The approximate locations of proposed monitoring wells (screened 10 to 20 feet below land surface [ft BLS]) are presented on Figure 1.

The monitoring wells will be installed using the hollow-stem auger drilling method and constructed of 2-inch (in) diameter Schedule-40 polyvinyl chloride (PVC) pipe with 10-feet (ft) of 0.010-in standard factory-slotted screen. The filter pack will consist of ten (10) ft of 20/30 silica sand sealed with a minimum of 0.5-ft of 30/65 sand. The remaining borehole will be grouted to land surface. The monitoring wells will be completed at land surface with an 8-in diameter steel manhole set in a 2-ft by 2-ft by 4-in thick concrete pad.

Investigation derived waste (IDW) will be staged in the rear of the facility. Historical analytical results indicate IDW has been non-hazardous. New soil and groundwater profiles will be setup with our selected IDW waste hauler WRI.

Subtask 3 – Groundwater Monitoring

Subtask 3 includes costs associated with the collection of groundwater samples from 29 monitoring wells in accordance with Table 1 of the 2023 Groundwater Monitoring Report and including the five (5) additional monitoring wells installed as part of Subtask 2. Below is a summary of the sampling locations and analyte list.

UIC groundwater samples (3) will be collected from MW002, MW019, and PZ003. The UIC groundwater samples will be submitted to a fixed-base laboratory and analyzed for TRPH by FL-PRO.

Performance groundwater samples (29) will be collected from MW001 through MW019, MW021 through MW026, EW001, and PZ001 through PZ003 and analyzed for VOHs by EPA Method 8260B.

Consistent with previous sampling events, groundwater level measurements will also be collected from the monitoring well network within a 24-hour period on the first day of field activities. Wells located on adjacent parcels and within the wooded terrain must be accessed on foot (see attached figure).

The monitoring wells will be sampled using a peristaltic pump in general accordance with FDEP Standard Operating Procedures (SOPs). The samples will be stored on ice and shipped to the analytical laboratory under chain of custody for analysis under standard turnaround times.

Purge water generated during sampling activities will be placed in a 55-gallon drum and manifested for off-site disposal. IDW will be staged in the rear of the facility. Historical analytical results indicate IDW has been non-hazardous. New soil and groundwater profiles will be setup with our selected IDW waste hauler WRI. Historical PFAS profiling analytical results have indicated disposal as non-hazardous waste. Bid costs prepared by the waste disposal companies are included in Attachment A.

Subtask 4 – Supplemental Site Assessment Report

Geosyntec will prepare a Supplemental Site Assessment Report documenting the results of the monitoring well installation, depth to groundwater measurements, and groundwater sampling. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the groundwater monitoring event. The report will be signed and sealed by a Florida-licensed professional geologist. A final signed waste manifest for IDW transport and disposal will also be included in the report.

From: Blaine Dawson <BCDawson@Geosyntec.com> Sent: Friday, February 2, 2024 1:54 PM Subject: RFQ - Utility Locate Services

Good afternoon, I’m interested in procuring an estimate to locate (EM and GPR) at the A1A Cleaners site located at 450026 State Road 200, Callahan, Florida. The attached figure illustrate the five (5) monitoring well locations for utility locating. We will need a maximum of a 20’x20’ grid around each location to make sure we have enough room to install the wells. The wooded areas have numerous cleared hiking trails that will be used to access the well locations. If possible, I’d appreciate your bids back by COB Tuesday, 6 February 2024. Let me know if you have any questions.

Thank you, Blaine Dawson, PG (FL,TN) Senior Geologist

RE: Utility Locating Services – 5 Bore Locations A1A Cleaners Site, Callahan, FL

Mr. Dawson, GeoTek Services is pleased to respond to your request for Utility Locating services at the A1A Cleaners site located in Callahan, Florida. We will scan up to a 20’ x 20’ area around the five proposed bore locations with the goal of locating underground utilities.

We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings, or vehicles. We will scan as close as reasonably possible to fixed objects such as landscaping, fences, etc.

Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Date: 02/06/2024 To: Geosyntec Attn: Blaine Dawson Project: Callahan, FL – Utility Locate/Soil Borings

ProCheck Inspections appreciates the opportunity to provide this proposal. Feel free to contact me at any time if you have any questions, or if you need additional information.

SCOPE OF WORK

We understand the scope to be to search for underground utilities as identified by the customer prior to clearing for soil boring locations. We will attempt to trace any utilities for which there are structures visible from the work area, and mark said utilities with paint and flags where possible. The client will be responsible for providing drawings or notify ProCheck Inspections of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival.

EQUIPMENT

• 350 MHz Hyperstacking GPR Antenna. The antenna is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, standing water, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. GPR works by sending pulses of energy into a material and recording the strength and the time required for the return of the reflected signal. Reflections are produced when the energy pulses enter a material with different electrical properties from the material it left. The strength of the reflection is determined by the contrast in signal speed between the two materials. The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the conductivity of the materials. The patented hyperstacking antenna can improve depth penetration and resolution when compared with a traditional GPR antenna.

• Electromagnetic Pipe Locator. The EM locator can detect the electromagnetic fields from live power or radio frequency signals. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting EM field can then be detected by the receiver. The receiver is moved over the surface without contacting the ground, so it is not affected by terrain. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. Depths achieved can be as much as 20’ depending on the type of signal being traced or methods used.

TERMS & CONDITIONS

1. ProCheck Inspections provides Ground Penetrating Radar (GPR) services in underground applications, including assisting in location services in accordance with ASCE Standard 38-02, Quality Level “B”. GPR is a great tool for detecting objects underground, however, site restrictions and environmental conditions can limit the accuracy and effective depth of GPR. For underground scanning, depth is completely dependent on the composition of soils in the area being surveyed. Customer fully understands that for every foot in depth penetration with the GPR equipment, the pipe/utility must be at least 1” in diameter to be located. For example, at 4 feet in depth, the pipe/utility must be 4” or larger to be detected. Some types of pipes are very difficult to locate, such as clay or concrete pipes, transite piping, and empty PVC type pipes. Given these factors, ProCheck Inspections CANNOT guarantee it will be able to locate ALL utilities on site.

2. Our goal is to provide you with the best answers possible to your questions regarding what lies below the surface, and where it is located. Customer acknowledges that they understand the fact that our answers are based upon the quality of the data retrieved and what ProCheck Inspections believes lies below the surface. The decision to proceed with boring or excavations of any manner, type, or method, it under the sole discretion of the customer and customer assumes full liability for aforementioned processes.

3. ProCheck Inspections will not accept liability for an inaccurate interpretation or any other reason, and customer agrees to release and indemnify ProCheck Inspections and its owners and agents from all losses and damages from all alleged negligence and/or contract claims by customer or any third party. These terms and conditions supersede any other previous terms and conditions either oral or written.

4. Payment is due upon receipt of invoice unless otherwise specified in writing.

5. If for some reason ProCheck Inspections arrives on site and the work is cancelled without prior notification, there will be a charge of

$300.00.
