Subject: 6 Months O&M – Year 2 - Soil Vapor Extraction System Colonial/Touch of Class 4552 Lake Worth Road Greenacres, Palm Beach County, Florida 33463 FDEP Facility ID NO. ERIC_5222 FDEP Task Assignment No. DC038F

Dear Evan:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this Statement of Work (SOW) and Level of Effort (LOE) for six months of operations and maintenance (O&M) in the second operation year of the soil vapor extraction (SVE) system at Colonial/Touch of Class (Site) (Facility ID - ERIC 5222) located in Greenacres, Florida. The SVE system startup occurred 30 November 2020 through 2 December 2020. Monthly O&M has been conducted at the Site since startup, and air and groundwater sampling has been performed in general accordance with Table 6 of the FDEP-approved May 2020 Interim Source Removal (ISR) Work Plan. Note that some additional air and groundwater samples have been approved by FDEP and collected during Year 1.

This LOE is subdivided into subtasks by quarter. For ease of reference, the major scope elements are outlined below as they will pertain to multiple subtasks in some cases. Each subtask will be outlined separately thereafter.

SCOPE OF WORK ELEMENTS

Monthly SVE System O&M Tasks

Geosyntec personnel will conduct 6 monthly O&M visits to the Site beginning in February 2021 (Months 14 to 19 of system operation). Note that the January 2022 O&M (Month 13) is included

Colonial/Touch of Class – Year 2 O&M LOE

FR3169/Colonial TOC_LOE_Year 2 OM_DC038F_revised under TA DC038E, which has been approved by FDEP and is in progress. During each O&M visit, Geosyntec personnel will perform the following tasks:

• record vacuum, pressure, and temperature readings at the SVE blower;

• record air flow rates at the blower and five vapor extraction wells (VEWs);

• record vacuum readings at the five VEWs (VEW-1 through VEW-5), MW001 and

VMP01 through VMP05;

• drain fluids accumulated in the moisture separator and disposing of fluids in a 55-gallon drum as investigation derived waste (IDW; water from the moisture separator at the Site has been sampled, analyzed for volatile organic halocarbons [VOHs] and characterized as non-hazardous);

• collect groundwater level measurements from monitoring wells MW001 through

MW007;

• perform required air sampling;

• inspect and clean the equipment compound; and

• perform periodic equipment maintenance activities recommended by equipment manufacturers.

The results of the monthly system O&M activities will be documented in a site-specific O&M log, which will be submitted to the FDEP with the monthly O&M summary emails.

Groundwater sampling

In accordance with Table 6 of the approved May 2020 ISR Work Plan, MW001 and MW002 will be sampled semi-annually (June 2022). During Year 1, MW003 was added to the semi-annual sampling event due to an increase in VOH concentrations observed in November 2020; however, no GCTL exceedances were reported in MW003 during 2021 semi-annual or annual sampling event. Therefore, for Year 2, groundwater sampling in accordance with Table 6 of the May 2020 ISR Work Plan is proposed.

Prior to collecting groundwater samples, depth to water measurements will be collected from monitoring wells MW001 through MW007. The groundwater samples will be analyzed by a FDOH NELAC certified laboratory for VOHs using EPA Method 8260. Results of the groundwater sampling events will be documented in the O&M report for the same reporting period.

IDW Generation and Management

Investigative Derived Waste (IDW) is not assumed to be generated during the M14 – M19 O&M activities, therefore costs for IDW are not included in this LOE. If IDW is generated during this period, Geosyntec will submit a work change directive (WCD).

Air sampling

In accordance with Table 6 of the approved May 2020 ISR Work Plan, air emissions will be monitored by sampling combined influent air from the VEWs on a quarterly basis at the Site (one air sample per quarter). Combined effluent will not be sampled as the granular activated carbon (GAC) was removed from the SVE system on 11 May 2021 following the 7 April 2021 sampling event that continued to show that VOHs were below the Hazardous Air Pollutant (HAP) criteria requiring GAC treatment. Air samples will be collected with a summa canister and analyzed at a fixed-base laboratory for VOHs by EPA Method TO-15.

Reporting

A Year 2 Semi-Annual O&M Report (following Month 18 [June 2022]) will be prepared to document the results of O&M activities. The O&M reports will include, at a minimum, the following:

• results of laboratory analyses of air samples (tabular and graphical form);

• O&M activities performed during the reporting period;

• the calculated mass of PCE (and daughter products, if detected) removed by the SVE system during the reporting period, and cumulative total removed since startup;

• the SVE system hours of operation during the reporting period;

• information regarding the depth to groundwater;

• information regarding any repairs and/or modifications made to the SVE system;

• vacuum ROI data (tabular and illustrated); and

• recommendations and conclusions pertaining to the continued operation and/or optimization of the SVE system.

Utilities

Monthly electrical utility bills have been estimated at $300 based on the previous year’s average monthly invoices from Florida Power & Light. An invoice from Product Recovery Management, Inc. (PRM) for annual telemetry services from 1 January to 31 December 2022 was received.

Telemetry is currently provided by PRM and is available through 31 December 2021. The cost of telemetry is $76.00 per month with taxes for a total of $975.84 and is levied as an annual fee (Attachment A). The utilities also include a $20 monthly processing fee for electrical service and we have also applied $20 processing fee for one-time payment of the PRM telemetry invoice.
