Subject: Scope of Work and Level of Effort – Interior SVE Component Installation Art Craft Cleaners 1630 University Boulevard South
Jacksonville, Duval County, Florida 32216 FDEP Site ID No. ERIC_4584
Dear Ms. Plyler, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work and Level of Effort (LOE) for conducting interior soil vapor extraction (SVE) component installation activities in advance of the property owner’s plans to renovate the building associated with the Art Craft Cleaners site (Site) located in Jacksonville, Florida (ERIC_4584). Geosyntec prepared this LOE based on the discussions during the 30 January 2023 teleconference with FDEP. A description of task breakdown for interior SVE component installation activities is provided below.
The Site Access Agreement (SAA) was executed with Mr. Harikrushna Patel on February 2, 2023.
On March 2, 2023, Geosyntec reviewed tax records on the Duval County Property Appraiser website which indicates the Site owner is Shreeji 1630, LLC, and that a change in Site ownership has not occurred since December 19, 2022. As a result, Geosyntec believes the current SAA is accurate.
Subtask 1 – Project Management
Subtask 2 – Interior SVE Component Installation
The Technical Specifications Package and Construction Drawings, developed by Geosyntec, were submitted to three pre-qualified Contractors to implement SVE component installation activities within the interior of the existing building prior to renovations. The bid proposal documents submitted by the Contractors are attached and summarized in the following table.
Contractor Quote Timeframe (days)
Moran $11,600.73 3
Hepaco $8,125.00 3
Action $21,900.00 4
Based on the bids, Geosyntec recommends Hepaco to implement the interior SVE component installation activities. Hepaco was selected because their costs were the lowest among the bidders and they were able to meet the property owner’s expedited schedule. Hepaco has estimated the interior SVE component installation activities can be completed in 3 field days. As such, Geosyntec has assumed up to 3 days of Construction Quality Assurance (CQA) oversight by Geosyntec’s onsite CQA Manager. In addition, the Engineer of Record (EOR) will complete an inspection of the work.
Prior to interior SVE component installation activities, Geosyntec will observe the following activities completed by the Contractor: (i) private utility locate; and (ii) installation of security fencing. Geosyntec and the Contractor will document of utilities encountered during the private utility locate.
During interior SVE component installation activities, Geosyntec will provide daily oversight and document activities (including ambient vapor monitoring using a photoionization detector and visual dust monitoring).
The Project Manager will support the EOR and CQA Manager by managing communications with FDEP and the property owner. This communication will include a summary of construction activities and schedule during the event.
The EOR will maintain contact with the onsite CQA Manager throughout the field phase. The EOR will complete the following activities:
Provide technical support to onsite personnel daily, receive progress updates at the end of each day, and review the daily construction field report.
Review, log, and approve daily Contractor submittals.
Conduct a site inspection prior to Site restoration.
Conduct a final site inspection punch list with the onsite CQA Manager.
The Project Manager will maintain contact with the onsite CQA Manager throughout construction and will complete the following activities:
Review, log and approve daily onsite CQA Manager field reports, including the air monitoring results.
Review payment of bid line items associated with Technical Specifications Package.
Prior to complete demobilization by the Contractor, Geosyntec’s onsite CQA Manager and EOR will complete a final inspection with the Contractor’s representative. A punch list will be generated by the onsite CQA Manager and EOR identifying items that are required by the Contractor for completion of interior SVE component installation activities.
Geosyntec will submit copies of the Daily Field Construction Logs (with representative photographs) as the milestone for invoicing this SOW/LOE.
