About this file

This document is a Task Assignment (TA) issued under Contract Number HW683 between the State of Florida Department of Environmental Protection (FDEP) and Geosyntec Consultants. The TA, designated DC014B, authorizes well abandonment activities at the Carriage Cleaners facility located at 3920 Confederate Point Road in Jacksonville, Duval County, Florida. The project is identified by FDEP Facility ID ERIC_4578. The FDEP Department Project Manager is Brittany Plyler (850-245-8851), and Geosyntec's Project Manager is Blaine Dawson (904-450-4271). Kelly Crain serves as the authorized signatory for FDEP, while Todd Kafka, P.G., serves as Contract Manager for Geosyntec.

The TA authorizes Geosyntec to conduct well abandonment activities in accordance with the contractor's scope of work, cost estimate, and schedule received on February 17, 2023. The contract is structured on a fixed unit rate basis with a total TA value of $9,417.90. Services are divided into two subtasks: Subtask 1 encompasses project management costs calculated at 13 percent of direct labor costs including fringe, overhead, and other direct costs; Subtask 2 includes costs for Geosyntec personnel to oversee the abandonment of seven monitoring wells (MW-1 through MW-7) and ten modified active gas sampling wells (MAGS001-MAGS008, VMP001, and VMP002) by a professional Florida-registered well driller. The single deliverable is the submittal and FDEP approval of the Well Abandonment Report due May 11, 2023, with invoice payment of $9,417.90 upon completion. The TA completion date is July 31, 2023. The project scope includes removal of well pads and manholes, grouting of wells from bottom to top using Portland cement and tremie pipes, site restoration to match pre-existing conditions (asphalt, concrete, or grass/dirt), and proper disposal of waste materials. Geosyntec solicited subcontractor quotes for drilling services, ultimately selecting Preferred Drilling Solutions at a cost of $4,906.00 for well abandonment services.

Subject: Well Abandonment Scope of Work/Level of Effort Carriage Cleaners 3920 Confederate Point Road Jacksonville, Duval, Florida FDEP Facility ID ERIC_4578

Dear Ms. Plyler, Geosyntec Consultants has prepared this scope of work (SOW) and proposed Level of Effort (LOE) for conducting well abandonment activities at the Carriage Cleaners (Site) (Facility ID ERIC_4578) located in Jacksonville, Florida. The monitoring well abandonments will be completed in accordance with the Site Rehabilitation Completion Order dated 22 December 2022.

A summary of the scope of work and associated costs is provided below.

The Site Access Agreement (SAA) was executed with Mr. Zuhair Shahly on November 1, 2019.

On January 18, 2023, Geosyntec reviewed tax records on the Duval County Property Appraiser website which indicates the Site owner is Shahly and Shahla, Inc. and that a change in Site ownership has not occurred since June 1, 2001. As a result, Geosyntec believes the current SAA is accurate.

Subtask 1 – Project Management

Subtask 2 – Monitoring Well Abandonment: Subtask 2 includes costs for Geosyntec personnel to oversee the abandonment of Site monitoring wells MW-1 through MW-7 and modified active gas sampling (MAGS) wells MAGS001 through MAGS008, VMP001, and VMP002 by a professional well driller registered in the state of Florida. The monitoring well pads and manholes will be removed before the well riser is cut approximately 6-inches below grade. Each monitoring well will be grouted with Portland neat cement from bottom to top using a tremie pipe. The area immediately surrounding each well will then be restored to match pre-existing conditions (i.e.

asphalt, concrete, dirt/grass). Waste associated with the former monitoring wells will be properly disposed of by the selected drilling subcontractor.

Ms. Brittany Plyler 17 February 2023

Following monitoring well abandonment activities, Geosyntec will prepare a Monitoring Well Abandonment Report documenting the abandoned monitoring well locations, construction details, and the State of Florida Well Completion Reports to be provided by the selected drilling subcontractor.

This is a cost estimate subject to our stand terms and conditions, not a fixed price bid. Due to the variable nature of subsurface conditions, site acess constraints, and/or site specific information not made known to JSA prior to mobilization, work will be billed on the above unit costs unless otherwise agreed in writing. The client will be responsible for loss or damage to any and all equipment and any associated extra costs, as a result of adverse, risky or unknown site conditions, by requesting field staff to attempt work that is normally beyond the capabilities of the equipment. The client is responsible for any disposal of Investigation Derived Waste (IDW) generated during services unless otherwise stated. At JSA's discretion, a restock fee may be imposed on the invoice for well materials not utilized due to changes in scope by client. Payment terms are net 30 days upon receipt of invoice unless otherwise stated in writing prior to initiation of work.

ATTORNEYS’ FEES & COSTS: The prevailing party shall be entitled to recover all of its costs, fees and expenses incurred in any dispute regarding collecting the balance due, enforcing the terms and conditions, or pertaining to JSA’s work. Such fees and expenses shall include attorneys’ fees, recording fees, and title report fees whether or not legal proceedings are instituted, and if instituted, shall include all other costs, fees expenses and, including without limitation, court costs and expert witness fees incurred in the trial/arbitration, appellate proceedings, and post-judgment.
