About this file
This document is a Task Assignment Change Order (TACO #1) issued by the State of Florida Department of Environmental Protection (DEP) to Geosyntec Consultants, Inc. under Contract HW683 for environmental assessment services at the City of Marianna-Jackson Square site (ERIC_18453) located at 4151 Lafayette Street, Marianna, Florida. The original Task Assignment (BF02A) was executed on February 14, 2024, with an effective completion date of September 16, 2024, later modified to November 1, 2024. The Department's Project Manager is Megan Johnson, and the Contractor's Project Manager is Kevin Warner.
The TACO reduces the scope and associated costs of the site assessment work by decreasing the number of DPT groundwater samples analyzed from 21 to 11 and reducing IDW (Investigative Derived Waste) drums for disposal from 37 to 25. These reductions resulted in a total cost decrease of $2,182.00, lowering the Task Assignment value from $147,850.02 to $145,668.02. The amended compensation remains on a fixed unit rate basis. The deliverables include field forms and laboratory results (due 06/19/2024), IDW manifest summary and disposal certification (due 07/22/2024), and a site assessment report with project management documentation (due 10/01/2024). Invoice amounts are segmented as follows: $115,061.93 for field forms and laboratory results, $3,557.52 for IDW disposal, and $27,048.57 for the site assessment report, totaling $145,668.02. The TACO was executed on August 7 and 8, 2024, extending the task completion date by 46 days.
