About this file

This document is a Task Assignment (TA) under Contract HW683 between the State of Florida Department of Environmental Protection (FDEP) and Geosyntec Consultants, Inc. The task assignment authorizes work at the Super Drycleaner site located at 5702 Lake Worth Road in Greenacres, Palm Beach County, Florida (FDEP ID: ERIC_5142). The TA is numbered DC003F with a completion date of September 30, 2026. Key personnel include Sarah Aduwa as the FDEP Project Manager and Nick Barnes as Geosyntec's Project Manager.

The scope of work consists of three subtasks: Project Management, Well Abandonment and SVE System Decommissioning Activities, and Report Preparation. Geosyntec will oversee the abandonment of seven groundwater monitoring wells, three vapor extraction wells, and four vacuum monitoring points by a state-licensed water well driller, as well as coordinate the decommissioning of the soil vapor extraction (SVE) system including removal of approximately 325 feet of PVC piping, electrical components, fencing, and related infrastructure. Upon completion, Geosyntec will prepare a letter report documenting these activities with photographic logs and disposal manifests. The total task assignment value is $22,131.09 based on a fixed unit rate, encompassing 67 labor hours. The single deliverable—the Well Abandonment and System Decommissioning Letter Report—is due August 18, 2026, with invoice payment of the full amount due upon FDEP approval of the report. The cost estimate includes labor, travel, subcontractor services (well abandonment, SVE decommissioning, and waste disposal), laboratory analysis, and field equipment expenses.

Subject: Scope of Work and CEF/LOE Well Abandonments and SVE System Decommissioning Super Drycleaner 5702 Lake Worth Road Greenacres, Palm Beach County, Florida

FDEP ID ERIC_5142

Dear Ms. Aduwa:

Geosyntec Consultants, Inc. (Geosyntec) has prepared the following scope of work (SOW) and level of effort (LOE) for completing soil vapor extraction (SVE) system decommissioning and well abandonment activities at the Super Drycleaner (herein, the “Site”) located in Greenacres, Florida. A Site Rehabilitation Completion Report (SRCR) was submitted on 10 November 2025 and a Site Rehabilitation Completion Order (SRCO) was issued on 17 February 2026.

On 17 April 2026, Geosyntec confirmed the Site Access Agreement (SAA) was executed with David Associates V, LLC, on 11 July 2017 as the real property owner of the Site located at the above address. The Palm Beach County Property Appraiser database indicates the property owner since 7 September 2000 is David Associates V, LLC and that the property has not changed ownership since then. As a result, Geosyntec believes the current SAA remains in place.

Subtask 1 – Project Management

Nick Barnes will serve as the Project Manager for the proposed SOW. Costs associated with this subtask are directly computed.

Subtask 2 – Well Abandonment and SVE System Decommissioning Activities

Geosyntec personnel will mobilize to the Site to observe the abandonment of seven groundwater monitoring wells, three vapor extraction wells (VEWs), and four vacuum monitoring points (VMPs) by a State licensed water well driller. Well abandonment permit applications for the monitoring wells will be completed and submitted to the South Florida Water Management District

(SFWMD).

Geosyntec requested quotes from licensed drillers to complete the well abandonment activities.

The quotes are provided in Attachment B of this proposal, and an evaluation of the quotes received is attached. The recommended driller (Action Environmental) will tremie grout the wells in place from the bottom of the well up to land surface. The driller will remove the concrete pads and well vaults. The ground surface will then be patched with neat cement, flush to surrounding grade. Concrete, asphalt, and other debris (e.g. well vault, flush-mount manhole covers) generated during the well abandonments will be disposed of by off-site by the Action Environmental.

Photographs of well abandonment activities will be collected in the field and provided as part of the field package for this effort. Action Environmental anticipates the well abandonment activities can be completed in one day. Geosyntec proposes using one field staff (S. Ableson) to provide oversight for the well abandonment activities for the one day.

This subtask also includes labor and expenses to decommission the SVE system and includes the following:

• Removal of approximately 325 feet of 2-inch diameter Schedule 40 polyvinyl chloride (PVC) conveyance piping and/or hose from above drop ceiling and on building exterior wall to SVE system compound area;

• Removal of conveyance pipe metal hanging straps or Unistrut;

• Sealing exterior walls penetrations by repairing stucco;

• Removal of underground, trenched electrical wiring from conduit (approximately 125 feet) from SVE system to transformer (if feasible). Geosyntec will coordinate with FP&L to disconnect wiring prior to decommissioning;

• Removal of electrical meter/disconnect stand and components;

• Removal and disposal of the chain‐link fencing with privacy slats surrounding SVE system and repairing/patching any holes in concrete or asphalt from fence posts;

• Disposal of all removed PVC pipe, metal hanging straps, Unistrut, electrical components, etc. from the decommissioning activities at a proper disposal facility; and

• Properly preparing the SVE system for long-term storage or transport by sealing or capping all penetrations and closing all valves. It is our understanding that Terracon will utilize this system at another location and will be responsible for transporting the system to that location.

Geosyntec submitted bid requests to three qualified Contractors and received two bids for the decommissioning scope of work. Action Environmental was the lowest responsive bidder for decommissioning the SVE system and is recommended to complete the SOW. The bids are provided in Attachment B. Action Environmental anticipates the decommissioning activities can be completed in two days concurrently with well abandonment and will use two field crews.

Geosyntec proposes using one field staff (R. Temple) to provide oversight for the decommissioning activities over two days.

A sample from the vapor phase granular activated carbon (GAC) drum will be collected by Geosyntec staff, with laboratory analysis for volatile organic compounds (VOCs) and metals via the Toxicity Characteristic Leaching Procedure (TCLP) method. Subcontractor quotes for disposal are provided in Attachment B, with Erwin Remediation, Inc. providing the lowest cost and selected for GAC disposal. It is assumed that no on-Site IDW removal oversight is necessary.

Subtask 3 – Reporting

Upon completion of the activities described above, Geosyntec will prepare a brief letter report that documents the well abandonment and SVE system decommissioning activities and includes a photographic log of the activities and the GAC disposal manifest.

Invoice and Deliverable Schedule

Geosyntec proposes the following deliverables and Period of Performance schedule below.

Subtask Deliverable Deliverable Date Invoice Amount

1, 2, 3 Submittal of Well Abandonment and System Decommissioning Letter Report 08/18/2026 $22,131.09

Total $22,131.09 The above schedule assumes that Geosyntec is tasked for this SOW by May 20, 2026.

2.2 EnviroServe shall have the right to subcontract any or all of the work covered by the Agreement. Any assignment of this Agreement or any rights hereunder by Client without written consent of EnviroServe shall be void.

2.3 EnviroServe will perform the services as an independent contractor. For the purposes of its performance hereunder, EnviroServe is not a servant, employee or other agent of Client. EnviroServe shall have the exclusive right to determine how its work or services will be accomplished and Client shall have no right to control and shall not control or direct the details, manner or means by which EnviroServe obtains or provides services to Client.

EnviroServe may continue to offer its services to the general public. Except as may be provided for herein, neither party may enter into agreements, make commitments or incur liabilities or obligations on behalf of the other party. Nothing in this Agreement will be deemed to create an agency relationship, partnership or joint venture between EnviroServe and Client, each of which is expressly denied.

2.4 Client warrants to EnviroServe that Client has procured the requisite legal right, title, and interest necessary to provide EnviroServe legal access to the job site ("Site"). EnviroServe will take reasonable precautions to minimize any damage to the Site and adjoining properties. Client shall identify and locate the area to be serviced at the Site and shall identify and accurately mark all pipelines, utilities, and other facilities at or near the work area.

EnviroServe is not charged with knowledge of any latent, concealed, or subsurface site conditions, and shall not be liable for: (i) damage or injury to any subsurface structures (including, but not limited to, utilities, mains, pipes, tanks, and telephone cables, human remains, burial markers, archeological sites, wetlands or hazardous materials or any wastes not identified in the Agreement) or any existing subsurface conditions, or the consequences of such damage or injury, if such structures or conditions were unknown and were not identified or shown, or were incorrectly shown, in information or on plans furnished to or obtained by EnviroServe in connection with the services; (ii) concealed conditions encountered in the performance of the services; (iii) concealed or unknown conditions in an existing structure at variance with the conditions indicated by the Agreement or information furnished to or obtained by EnviroServe; or (iv) unknown subsurface physical conditions that differ materially from those ordinarily encountered. Should EnviroServe encounter such conditions, the Parties shall agree on a revised Scope of Work providing for an equitable adjustment of the price and/or time of performance to account for such unknown or changed conditions. EnviroServe’s duty to defend, indemnify and hold harmless, as provided in this Agreement and otherwise, shall not extend to any losses arising out of, resulting from, or in any way connected with, any such latent, concealed, or subsurface conditions.

2.5 Client agrees to de-energize, lock out, tag out, and ground any electrical equipment consistent with EnviroServe's schedule for services, provide utility services required for the performance of services; provide ventilation capacity, as specified, provide access to workrooms, toilet facilities, first-aid and medical facilities as available, for EnviroServe personnel; re-energize the equipment after servicing by EnviroServe. Client shall be responsible for all loss, damage, or injury to equipment that is de-energized including fuses, cutouts and switches, and shall operate its equipment before, during and after servicing in accordance with state, local and federal laws. Client acknowledges that all equipment may be affected by operating conditions other than those for which the equipment was designed. Client recognizes that equipment is subject to failure at any point and failures may be brought about prematurely by excessive heating, overloading, shock loading, transient voltage surges, among other causes, all of which are within the exclusive control of Client. Client is aware that operating conditions, including examples stated above which contribute to the failure of equipment may exist prior, during and subsequent to the performance of the services.
