Summary HW683
DC015A
This Task Assignment authorizes Geosyntec to conduct annual groundwater monitoring at the Former Penthouse Cleaners.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2 – Groundwater Sampling Geosyntec will mobilize to the Site in May or June 2022. As recommended in the 2021 Report, Geosyntec will collect a synoptic round of groundwater levels from 15 existing monitoring wells at the Site, collect groundwater samples from the seven PDBs (already deployed within the monitoring wells from the last groundwater sampling event), and redeploy new PDBs at MW001, MW004, MW005, MW006, MW008, MW013, and MW014 for analysis of volatile organic halocarbons (VOHs).
Subtask 3 – Annual Groundwater Monitoring Report Preparation Geosyntec will prepare a report summarizing sampling activities and results, and provide conclusions and recommendations based on data evaluation. The report will contain field forms, tables summarizing field and laboratory data, figures showing groundwater flow directions, and a textual summary of the groundwater monitoring event. The PCE groundwater isocontour map will be updated with new analytical results. The report will be signed and sealed by a Florida-licensed geologist and one electronic copy of the final report will be submitted to the FDEP via email and uploaded to FDEP’s file transfer protocol website.
This task also includes preparation of ADaPT and GIS data tables.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $7,395.80 with 54 labor hours.
Geosyntec may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Subject: Scope of Work and Level of Effort Annual Groundwater Monitoring
Former Penthouse Cleaners 2442 South Volusia Avenue Orange City, Volusia County, Florida FDEP Facility ID ERIC_5539
Dear Mr. Talley:
Geosyntec Consultants, Inc. (Geosyntec) has prepared this scope of work (SOW) and level of effort (LOE) for annual groundwater monitoring at the Former Penthouse Cleaners (the “Site”;
Facility ID ERIC_5539) located at 2442 South Volusia Avenue in Volusia County, Orange City, FL. This SOW/LOE incorporates comments provided by the Florida Department of Environmental Protection (FDEP) in review of the Annual Groundwater Monitoring Report - 2021 dated 30 June 2021 (Report). The SOW consists of three subtasks and is summarized below.
Subtask 1 – Project Management
Subtask 2 – Groundwater Sampling
Geosyntec will mobilize to the Site in May or June 2022. The 2022 sampling event will require one mobilization to the Site. The monitoring wells include four Upper Floridan aquifer wells (MW001, MW004, MW008, and MW014) and three lower surficial wells (MW005 MW006, and MW013). Water quality parameters will not be collected as part of PBD sampling. Groundwater samples will be submitted for laboratory analysis of VOHs by Environmental Protection Agency Method 8260B. IDW will not be collected or containerized during sampling activities. Water remaining in the PDBs after sampling will be returned to the monitoring well from which is was pulled.

Mr. Evan Miller 6 May 2022
FR0857F/PENTHOUSE CLEANERS SOW-NAM_MAY 2022
Subtask 3 – Annual Groundwater Monitoring Report Preparation
Geosyntec will prepare a report summarizing sampling activities and results, and provide conclusions and recommendations based on data evaluation.
Invoice and Deliverable Schedule:
Note that this schedule reflects two weeks FDEP review of the LOE. Geosyntec requests the following schedule of deliverables and invoices:
Subtask Deliverable Deliverable Date Invoice Amount
1, 2 & 3 Submittal of report, ADaPT and GIS data tables, project management 29 July 2022 $7,395.80
TOTAL COST $7,395.80
We appreciate this opportunity to work with FDEP on this project. Feel free to contact us if you have any questions or require any additional information.
Sincerely, Fabio Fortes, P.E.
