Subject: Scope of Work and CEF/LOE – Monitoring Well Installation and Annual Groundwater Monitoring Maitland Plaza 110 North Orlando Avenue Maitland, FL 32751 FDEP Site ID No. ERIC_5057

Dear Ms. Aduwa:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this proposed Scope of Work (SOW), Level of Effort (LOE), and Cost Estimating Form (CEF) to conduct monitoring well installation activities and annual groundwater monitoring for volatile organic halogens (VOHs) at the Maitland Plaza Site located in Maitland, Florida (ERIC_5057). A description of task breakdown for field activities and report preparation is provided below. A CEF/LOE is provided as Attachment A.

Subtask 1 – Project Management

Costs associated with this subtask are directly computed.

Subtask 2 – Monitoring Well Installation

This Subtask includes costs associated with the installation of one downgradient and offsite monitoring well (Figure 1) as recommended in the February 2024 Annual Groundwater Monitoring Report in an effort to complete delineation of VOHs. An existing access agreement with this offsite property owner is in place.

Prior to conducting monitoring well installation activities, Geosyntec will observe a subsurface survey to identify potential subsurface utilities or obstructions. Subcontractor quotes are provided in Attachment B. GPRS provided the most competitive bid and has been selected for this work. The proposed monitoring well will be installed using direct-push technology (DPT) to approximately 50 feet (ft) below land surface (BLS). The monitoring well will be constructed with 10 ft of one-inch diameter Schedule 40 polyvinyl chloride (PVC) pre-packed well screen slotted at 0.010-inches and blank Schedule 40 PVC riser. The filter pack will consist of 20/30 silica sand placed

Maitland Plaza (ERIC_5057)_HW683 SOW_GMR_2025_01 around the well screen to approximately 0.5-ft above the top of the well screen, followed by a 0.5-ft thick 30/65 fine sand seal and Portland Cement grout to the surface. The monitoring well will be competed flush-to-grade in 8-inch diameter manhole embedded in a 2-ft by 2-ft concrete pad.

The monitoring well will be developed until visually clear. Soil cuttings and development water will be containerized in drums prior to offsite disposal.

Geosyntec obtained cost estimates from three drilling subcontractors. The quotes are provided in Attachment B. EDS provided the most competitive bid and has been selected for this work.

Subtask 3 – Annual Groundwater Monitoring

This subtask includes the labor and expenses for the completion of a groundwater monitoring event. Geosyntec will mobilize to the Site to collect depth-to-water (DTW) measurements and groundwater samples from the Site-wide monitoring well network. Geosyntec will measure DTW from all 34 wells within a 24-hour period. Each well is a flush mount that may require bailing of the manholes. Geosyntec staff will survey the top of casing (TOC) of the newly installed monitoring well and collect GPS coordinates of the well during this Subtask. Geosyntec will collect laboratory analytical samples from 17 wells in accordance with FDEP Standard Operating Procedures (SOPs) and submitted for laboratory analysis of volatile organic halogens (VOHs) by Environmental Protection Agency Method 8260. A summary of the proposed wells for sampling are listed below; please refer to the figures in Attachment C for the prior groundwater quality results.

GMW-006

Based on the historic DTW at the Site, it is assumed that a peristaltic pump will be used to purge and sample each monitoring well. Purge water will be stored in 55-gallon drums. Based on per-and polyfluoroalkyl substances (PFAS) screening previously completed at the Site, it is assumed that the IDW drums will need to be disposed of as PFAS-impacted waste. Costs prepared by the waste disposal companies are included in Attachment B. Based on the bids, Geosyntec

Maitland Plaza (ERIC_5057)_HW683 SOW_GMR_2025_01 recommends selecting Erwin Remediation to haul and dispose of investigation derived waste (IDW) generated during monitoring well installation and sampling activities. It is assumed that oversight of the IDW pickup will not be required.

Subtask 4 – Groundwater Monitoring Report

Geosyntec will prepare a Groundwater Monitoring Report documenting the VOH results of the groundwater monitoring event. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the field activities completed. The report will be signed and sealed by a Florida-licensed professional geologist or engineer.

SCOPE OF WORK

Soil Boring: We understand the scope of work on this project is to search for underground utilities in a 10’ radius around 1 soil bores. We will attempt to trace any utilities for which structures are visible from the work area. Utilities will be marked on the surface using paint, pin flags, or other appropriate means. The client will be responsible for providing drawings or notifying GPRS of any utilities entering the work area for which there are no apparent surface features or structures visible from the work area. To avoid additional charges, the areas should be laid out, marked, and cleared of obstructions before our arrival. Please visit www.simspec.org for an overview of our industry-leading best practices that will be applied to this project.

GPRS is not responsible for housekeeping. Any debris, equipment, or other obstructions in the area at the time of scanning could potentially block out needed data.• A thorough utility search can only be completed if GPRS is given access to all utility structures, interior and exterior. This service is never a replacement for the use of the state One Call system (811).

All of our technicians have OSHA-10 safety training or greater. Site-specific safety training is not included in this quote. Please notify us if this project requires additional safety training.

EQUIPMENT

Underground Scanning GPR Antenna. This GPR Antenna uses frequencies ranging from 250 MHz to 450 MHz and is mounted in a stroller frame that rolls over the surface. Data is displayed on a screen and marked in the field in real-time. The surface needs to be reasonably smooth and unobstructed to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the efficacy of GPR.

The total effective scan depth can be as much as 8’ or more with this antenna but can vary widely depending on the soil conditions and composition. Some soil types, such as clay, may limit maximum depths to 3’ or less. As depth increases, targets must be larger to be detected, and non-metallic targets can be challenging to locate. The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

Electromagnetic Pipe Locator. This receiver can passively detect the signals from live AC power or radio signals traveling along some conductive utilities. Operators can connect a transmitter directly to accessible metallic pipes, risers, or tracer wires to generate a current at a specific frequency. The receiver can then detect the resulting signal along the pipe or tracer wire. Various factors may impact this device's effectiveness, including (but not limited to) access to the utility, conductivity, grounding, and interference from other utilities.

The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

Traceable Rodder. The rodder consists of a copper wire encased in fiberglass. This device is pushed through a pipe with direct access, such as a sewer line at a cleanout or a storm drain catch basin. Operators then induce a current on the wire and trace the signal from the surface. The maximum traceable depth is 10’ depending on the soil conditions, and the maximum distance is 200’. Inserting the rodder into deeper pipes within manholes may not be feasible depending on site conditions. GPRS will not access electrical conduits. The signal is not detectable through metallic pipes. For more information, please visit: Link

GPS. This handheld unit offers accuracy down to 4 inches; however, the accuracy achieved will depend on the satellite environment at the time of collection and is not considered survey-grade. Features can be collected as points, lines, or areas and then exported as a KML/ KMZ or overlaid on a CAD drawing. For more information, please visit: Link
