HW683_DC017B

This Task Assignment authorizes Geosyntec to conduct remediation activities at Maitland Plaza in accordance with the Geosyntec scope of work, cost estimate, and schedule received on May 26, 2023.

Geosyntec confirmed on May 26, 2023, that Subtask 1 consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.

Subtask 2: This subtask includes costs associated with the preparation of an ISR Work Plan.

The ISR Work Plan will recap the results of the soil assessment activities completed at the Site and propose the SVE well layout, as well also include an engineering cost estimate for implementation and operation and maintenance.

Subtask 3: Geosyntec will develop and prepare Bid Documents necessary to obtain subcontractor costs to implement the ISR at the Site. The Bid Documents will include the specifications necessary for potential subcontractors to provide cost for site restoration activities.

Subtask 4: Geosyntec will develop a list of up to three qualified SVE Contractors for invitation to the Site to participate in a bid walk. Geosyntec will send interested Contractors copies of the ISR Work Plan, Bid Documents, and Technical Specifications.

The FDEP project manager confirmed site access on June 5, 2023.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost is $30,592.22 with 202 labor hours.

May 26, 2023

Ms. Brittany Plyer Florida Department of Environmental Protection 2600 Blair Stone Road Tallahassee, FL 32399-2400

Subject: Scope of Work and CEF/LOE – Interim Source Removal Work Plan

Maitland Plaza 110 North Orlando Avenue Maitland, FL 32751 FDEP Site ID No. ERIC_5057

Dear Ms. Plyer:

Geosyntec Consultants, Inc. (Geosyntec) has prepared this proposed Scope of Work (SOW), Level of Effort (LOE), and Cost Estimating Form (CEF) for remediation activities at the Maitland Plaza/Utopia Cleaners Site located in Maitland, Florida (ERIC_5057). This SOW includes (i) preparing an Interim Source Removal (ISR) Work Plan, (ii) preparing Bid Documents, and (iii) soliciting bids from qualified subcontractors and evaluating the subcontractor bids for selection.

A description of task breakdown for field activities and report preparation is provided below. A CEF/LOE is provided as Attachment A.

Subtask 1 – Project Management

Costs associated with this subtask are directly computed.

Subtask 2 – ISR Work Plan

This subtask includes costs associated with the preparation of an ISR Work Plan pursuant to Chapter 62-780.525, Florida Administrative Code to address vadose zone soil impacts at the Site.

Specifically, Geosyntec anticipates that the ISR Work Plan will detail a soil vapor extraction (SVE) system to treat soils from 0 to 19 feet (ft) below land surface (BLS), as identified in the January 2023 Supplemental Site Assessment Report.

The ISR Work Plan will recap the results of the soil assessment activities completed at the Site and propose the SVE well layout (including use of existing modified active gas sampling [MAGS] wells), piping layout, construction, operation, and maintenance details (including a process and instrumentation diagram, well details, electrical connection to system, and trenching cross section details). The ISRWP will also include an engineering cost estimate for implementation and operation and maintenance (O&M).

To support the preparation of the ISR Work Plan, Geosyntec will communicate with the Site owner to discuss the design plan and coordinate logistics for a Site visit with the owner (or local representative, if present) and existing tenants. The objectives of the Site visit are to evaluate interior access for construction activities, such as trenching for subgrade piping runs or the use of above grade piping along building columns into overhead crawlspaces (or both); document current Site conditions; discuss access and logistics with the Site owner and building tenants; determine a location for the treatment trailer; and evaluate the presence of overhead and underground utilities.

Due to the complexity of the Site footprint and potential challenges with implementation of the system, Olivia Cain (Engineer) and Rachel Klinger (Engineer of Record) will conduct the Site visit. Effort has been included for Geosyntec to converse with the property owner and tenants before and after the Site walk to ensure the proposed design is appropriate (assumed two, 1-hour teleconferences with Olivia Cain and Rachel Klinger).

Prior to submittal of the ISR Work Plan, Geosyntec will provide FDEP with a photo log documenting existing Site conditions and a copy of notes and/or meeting minutes from discussions with the Site owner and building tenants that culminates in an acceptable conceptual design. The conceptual design will be discussed with FDEP during a 1-hour teleconference to facilitate the SVE design and minimize FDEP comments.

Subtask 3 – Bid Package Development

After FDEP’s approval of the ISRWP, Geosyntec will develop and prepare Bid Documents necessary to obtain subcontractor costs to implement the ISR at the Site. The Bid Documents will include the specifications necessary for potential subcontractors (i.e., bidders) to provide a cost to permit, mobilize, and prepare the Site for construction; complete trenching, backfilling, and install the treatment system; and perform Site restoration activities. Geosyntec will submit the Bid Documents or Technical Specifications to complete this Subtask and recognizes the FDEP Engineer will not review them.

The Bid Documents will include:

Instructions for Bidders;

Request for Bids Scope of Work;

Construction Drawings;

Technical Specifications; and Bid Proposal Form.

The Bid Documents and Technical Specifications section will include, but not be limited to, the following elements:

Summary of work;

Description of site conditions, access constraints, overhead and subsurface utilities, and historical soil data, etc.;

Permitting (local, state, federal);

Safety, health, and emergency response requirements;

Site preparation and pre-mobilization activities (e.g., underground utility locating);

Requirements for protection of existing structures, as applicable;

Trenching locations and specifications;

Conveyance piping and connection specifications;

Backfill specifications;

Electrical connection requirements and electrical subcontractor costs;

Telemetry system connection and operational requirements;

Waste profiling, manifesting, transportation, and disposal requirements; and Site restoration and demobilization requirements.

Subtask 4 – Bid Walk and Evaluation

Following FDEP’s review and approval of the Bid Documents and Technical Specifications, Geosyntec will develop a list of up to three qualified SVE Contractors for invitation to the Site to participate in a bid walk. Prior to scheduling the Site visit, Geosyntec will send interested Contractors copies of the ISR Work Plan, Bid Documents, and Technical Specifications approved by FDEP. Geosyntec will coordinate the visit with the Site owner and current tenants and will conduct a 1-hour pre-call with both to provide an overview of the ISR Work Plan and allow the owner and tenant to provide input on logistics, layout changes (if Site conditions differ from those at the time of the ISR Work Plan), and how to minimize disruptions to facility operations. This information will allow for the Site visit to run smoothly and provide the best opportunity for setting clear expectations for the contractors. No additional labor has been included to address requests or changes from the discussions with the owners and tenants during the ISR Work Plan preparation.

Olivia Cain (Engineer) and Maria Lemes (Professional) will attend the bid walk, with support from Rachel Klinger, as needed (Engineer of Record).

Geosyntec will respond to requests for additional information (RAIs) submitted by contractors following the Site visit. Geosyntec will review contractor bids and evaluate costs and approaches to identify the best value to FDEP. Geosyntec will submit the Contractor bids, RAIs, bid evaluation, and our recommendation for Contactor selection to FDEP to serve as the deliverable for this Subtask. We anticipate implementation of the ISR will be performed under a separate Task Assignment.
