Mr. Evan Miller Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road Tallahassee, FL 32399-2400
Subject: SOW/LOE – Post Active Remediation Monitoring Walker Plaza (ERIC_4783) 603 – 617 North Maryland Avenue Plant City, Florida
Dear Mr. Miller:
Geosyntec Consultants, Inc. (Geosyntec) has prepared the following scope of work (SOW) and proposed level of effort (LOE) to perform post active remediation monitoring (PARM) at the Walker Plaza site (herein, the “Site”) located in Plant City, Florida. The objective of this SOW is to collect three quarters of groundwater samples from seven existing monitoring wells to evaluate the effectiveness of soil vapor extraction (SVE) remediation activities conducted on-Site pursuant to Chapter 62-780.750, Florida Administrative Code on groundwater concentrations of volatile organic halocarbons (VOHs). Based on comments from the Florida Department of Environmental Protection (FDEP) memorandum, dated 20 April 2022 and addressing the Annual Groundwater Monitoring Report, it is assumed that the groundwater sampling event conducted in March 2022 will serve as the first of four quarters (i.e., Quarter 1) of PARM.
SOW ELEMENTS
Geosyntec will collect three quarters (groundwater sampling event from March 2022 will serve as Quarter 1) of groundwater samples from two on-Site monitoring wells (i.e., MW002 and MW006) and five off-Site monitoring wells (i.e., MW010 to MW012, MW013, and MW016). The rationale for the locations (see Figure 2 and Figure 3) is as follows:
MW002 (2.8-12.8 ft) Source area on-Site, PCE above GCTL MW012 (65-75 ft) cDCE detection, off-Site
MW006 (0.7-10.7ft) Source area on-Site, historical PCE impacts MW013 (3-13 ft) No VOH impacts, off-Site downgradient property line
MW010 (3-13 ft) PCE above GCTL, off-Site MW016 (3-13 ft) TCE above GCTL, off-Site downgradient property line
MW011 (27-32 ft) PCE and TCE above GCTLs, off-Site
Notes: PCE indicates tetrachloroethene; TCE indicates trichloroethene; cDCE indicates cis-1,2-dichloroethene;
GCTL indicates Groundwater Cleanup Target Level.

Prior to each quarterly groundwater sampling event, Geosyntec will gauge and record the depth-to-water (DTW) from 19 monitoring wells (this excludes MW008, MW009D, and UMW001) within a 24-hour period, including from 12 wells that are not anticipated to be sampled on the first day of field activities. Geosyntec will collect a total of 7 groundwater samples per quarterly event (28 samples total) using a peristaltic pump. Field activities will be conducted in accordance with the FDEP Standard Operating Procedures. Groundwater samples will be submitted under chain of custody protocol for laboratory analysis of VOHs by Environmental Protection Agency Method 8260B.
Action levels for the wells listed above will be as follows:
• The FDEP Groundwater Cleanup Target Level (GCTL) for tetrachloroethene (PCE) or 3 micrograms per liter (µg/L), where current PCE concentrations are less than its GCTL (i.e., MW006, MW012, MW013, and MW016)
• The FDEP GCTL for trichloroethene (TCE) or 3 µg/L, where TCE concentrations are less than its GCTL (i.e., MW002, MW006, MW010, MW012, and MW013).
• The FDEP Natural Attenuation Default Concentration (NADC) for PCE or 300 µg/L, where current PCE concentrations are greater than its GCTL but less than its NADC (i.e., MW002, MW010, and MW011)
• The FDEP NADC for TCE or 300 µg/L, where current TCE concentrations are greater than its GCTL but less than its NADC (i.e., MW016).
• The TCE action level at MW011 will be 1.5 times the 95% upper confidence level for historical TCE results (742 µg/L) at MW011 or 1,113 µg/L.
If analyses of groundwater samples indicate that concentrations of PCE or TCE exceed the action levels above, then a change order will be submitted and the well or wells exceeding the action levels will be resampled no later than 30 days after receipt of the analytical results for the corresponding quarter.
Based on previous sampling events, it is estimated that purge water generated during sampling activities will be containerized in one, 55-gallon drum. Geosyntec will collect one sample for VOHs during the last quarterly sampling event to update the waste characterization analytical results for disposal (see Subtask 6).
LOE SUBTASKS
Subtask 1 – Project Management
James Mills will serve as the Project Manager for the proposed SOW. Costs associated with this subtask are directly computed.
Subtask 2 – Quarter 2 PARM Sampling (June – August 2022)
Geosyntec will conduct one quarterly groundwater sampling event as outlined above, prepare and update the FDEP GIS Data Tables/ADaPT files electronically, and submit one field deliverable to the FDEP with groundwater sampling forms, laboratory analytical results, updated groundwater analytical results and DTW tables, and a Site Map figure.
Subtask 3 – Quarter 3 PARM Sampling (September – November 2022)
Geosyntec will conduct one quarterly groundwater sampling event as outlined above, prepare and update the FDEP GIS Data Tables/ADaPT files electronically, and submit one field deliverable to the FDEP with groundwater sampling forms, laboratory analytical results, updated groundwater analytical results and DTW tables, and a Site Map figure.
Subtask 4 – Quarter 4 PARM Sampling (December 2022 – February 2023)
Geosyntec will conduct one quarterly groundwater sampling event as outlined above, collect one IDW sample, prepare and update the FDEP GIS Data Tables/ADaPT files electronically, and submit one field deliverable to the FDEP with groundwater sampling forms, laboratory analytical results, updated groundwater analytical results and DTW tables, and a Site Map figure.
Subtask 5 – PARM Report
Approximately 60 days after receipt of the final quarter (i.e., Quarter 4) of groundwater analytical results, Geosyntec will submit one PARM Report that will be signed and sealed by a licensed Florida engineer or geologist and include analytical results, field forms, and updated tables and figures summarizing the four quarterly sampling events. The field forms will consist of groundwater sampling, calibration, DTW, and drum logs generated during quarterly sampling activities. The tables will include (i) DTW measurements, (ii) groundwater analytical results for VOHs, and (iii) geochemical field parameters. The figures will include (i) Site location Plan, (ii) Site map, (iii) groundwater elevation contours in the surficial aquifer, (iv) groundwater elevation contours in the intermediate aquifer, (v) groundwater analytical results in the surficial aquifer, and
(vi) groundwater analytical results in the intermediate aquifer.
Subtask 6 - IDW Management
This subtask includes generation and management of groundwater investigation-derived waste (IDW), which is estimated to include one drum. IDW will be staged inside the gated enclosure containing the SVE skid at the Site. Universal Environmental Solutions, LLC. is the lowest cost provider of transportation and disposal services per drum. Geosyntec will review the generation of a waste characterization profile as well as prepare the IDW manifest. IDW transportation and disposal quotes are provided in Attachment A. Field oversight of the final IDW pickup is not required.
Invoice and Deliverable Schedule
Please note the following schedule is contingent on FDEP review and approval of this SOW/LOE within two weeks of submittal. We have tentatively scheduled groundwater sampling for the week of 6/13/2022.

BID PROPOSAL FORM 2022
FDEP Drycleaner and Hazardous Waste Sites
Investigation Derived Waste (IDW) Transportation and Disposal
Contractor Company Name: HEPACO, LLC
Contact: Linda Basinger, 813-240-8020, lbasinger@hepaco.com
Region T&D of 1 Non-Hazardous IDW Drum (solid or liquid), Process Code STAB01
T&D of 1 Hazardous IDW Drum (solid or liquid), Process Code STAB02
Northwest $ 191.27 $ 367.36
Northeast $ 191.27 $ 367.36
Central $ 170.00 $ 346.10
Southwest $ 170.00 $ 346.10
Southeast* $ 170.00 $ 346.10
Assumptions and Clarifications:
• Unless otherwise specified within this proposal all terms and conditions will be governed by the most recent Master Services
Agreement between GEOSYNTEC and HEPACO, LLC. In the event that a current agreement is not in place all terms and conditions will be governed by HEPACO’s standard service agreement.
• Price is contingent upon profile approval and process code approval into HEPACO’s preferred disposal facilities.
• A minimum stop fee of $500.00 will apply to all shipments.
• The above rates are for less than truck load quantities (LTL). In the event that a higher volume of disposal is needed at any particular facility HEPACO will provide a case by case pricing scenario to help reduce overall cost.
• Above rates include a standard energy and insurance fee. In the event that market changes dramatically HEPACO reserves the right to increase or decrease these fees based on the appropriate energy index. In the event that this is required HEPACO will provide a
30 day written notice of the change.
* Excludes Monroe County (Case by case basis determined by volume of IDW)
• Bid costs above should be inclusive of transportation fees, fuel fees, environmental recovery fees, etc.
• No standby time should be included in the above bid costs.
• Assume that the drums are accessible.
• It is assumed that VOH totals results will be provided for profiling. If other analytical results are required, please indicated here:
