Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2 – SVE System Decommissioning This subtask includes labor and expenses to decommission the SVE system including the following:
• Remove approximately 700 feet of 2-inch diameter Schedule 40 polyvinyl chloride (PVC) conveyance piping in attic crawl space and on building exterior near SVE system compound;
• Remove all conveyance pipe metal hanging straps and screws from wooden trusses in attic crawl space;
• Remove all conveyance piping Unistrut on building exterior;
• Seal interior sheetrock firewall penetrations using sheetrock patch (2 walls to be patched);
• Seal exterior building penetrations by repairing stucco;
• Disconnect and remove underground, electrical wiring in conduit (approximately 40 feet) from SVE system to temporary power pole (overhead electrical power line disconnected from temporary power pole by Tampa Electric Company in June 2021). The underground conduit will be cut and capped below land surface;
• Dispose of all PVC pipe, metal hanging straps, Unistrut, electrical conduit, etc. from the decommissioning activities at proper off-Site landfill; and
• Properly prepare SVE system for long-term storage by sealing or capping all penetrations and closing all valves.
Geosyntec will keep the SVE system on site to be used later on at another site.
Geosyntec will prepare a brief letter report that documents the SVE system decommissioning activities and includes a photographic log of the activities.
MEMORANDUM

January 27, 2022 Page Two
Geosyntec proposes using one field staff to provide oversight throughout the duration of the decommissioning activities.
Geosyntec will prepare a brief letter report that documents the SVE system decommissioning activities and includes a photographic log of the activities.
Subtask 3 – Groundwater Monitoring Geosyntec will collect groundwater samples from two on-Site monitoring wells (i.e., MW002 and MW006) and six off-Site monitoring wells (i.e., MW010 to MW012 and MW016 to MW018). Prior to groundwater sampling, Geosyntec will gauge and record the depth-to-water (DTW) from 22 monitoring wells within a 24-hour period, including from 16 wells that are not anticipated to be sampled on the first day of field activities. Geosyntec will collect a total of eight groundwater samples using a peristaltic pump, and one field duplicate will be collected for quality assurance and quality control purposes. Field activities will be conducted in accordance with the FDEP Standard Operating Procedures. Groundwater samples will be submitted under chain of custody protocol for laboratory analysis of volatile organic halocarbons (VOHs) by Environmental Protection Agency Method 8260B. Based on previous sampling events, it is estimated that purge water generated during sampling activities will be containerized in one, 55-gallon drum. Geosyntec will collect one sample for VOHs to update the waste characterization analytical results for disposal (see Subtask 5).
This subtask also includes the preparation of this SOW and LOE.
Subtask 4 – Groundwater Monitoring Report Approximately 30 days after receipt of groundwater analytical results, Geosyntec will submit an Annual Groundwater Monitoring Report that will be signed and sealed by a licensed Florida engineer or geologist and include field forms and updated tables and figures.
Subtask 5 - IDW Management This subtask includes generation and management of groundwater investigation-derived waste (IDW), which is estimated to include one drum (see Subtask 3).
Subtask 6 – Two Quarters of Vegetation Maintenance This subtask includes labor and expenses to maintain vegetation within the SVE system compound for four quarters while the SVE system remains on-Site. Geosyntec will manually grub vegetation and/or utilize a weed trimmer to maintain vegetation within the SVE system compound. Additionally, Geosyntec will inspect the condition of the seals and/or caps installed on the SVE system as part of preparation for long-term storage during the vegetation maintenance visits. Geosyntec will photo document the SVE system compound before and after the vegetation maintenance activities.
Geosyntec will prepare quarterly email summaries of each events that include select photos of the vegetation maintenance activities.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $19,880.58 with 117 labor hours.
Geosyntec may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Mr. Evan Miller Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road Tallahassee, FL 32399-2400
Subject: SOW/LOE - SVE System Decommissioning, Annual Groundwater Monitoring, and 2 Quarters Vegetation Maintenance Walker Plaza (ERIC_4783) 603 – 617 North Maryland Avenue Plant City, Florida
Dear Mr. Cohen:
Geosyntec Consultants, Inc. (Geosyntec) has prepared the following scope of work (SOW) and proposed level of effort (LOE) to perform decommissioning of the soil vapor extraction (SVE) system, annual groundwater monitoring, and two quarters of vegetation maintenance within the SVE system compound at the Walker Plaza site (herein, the “Site”) located in Plant City, Florida.
The following scope of work was developed based on the meeting between Mubeen Darji with the Florida Department of Environmental Protection (FDEP) and Todd Kafka, Joseph Bartlett, and James Mills with Geosyntec conducted on 21 September 2021. During the meeting, the FDEP and Geosyntec agreed that SVE cleanup operations were complete based on confirmatory soil sampling results collected in July 2021 and that annual groundwater monitoring should continue for a subset of wells within the existing monitoring well network to evaluate stability of the groundwater plume over time. Lastly, the FDEP and Geosyntec agreed to stage the SVE system on-Site within the fenced SVE compound until the system can be reused at another drycleaner site with the need for a similar small SVE system; staging the SVE system will require quarterly vegetation maintenance of the compound.
Subtask 1 – Project Management
James Mills will serve as the Project Manager for the proposed SOW. Costs associated with this subtask are directly computed.
Subtask 2 – SVE System Decommissioning
This subtask includes labor and expenses to decommission the SVE system including the following:
• Remove approximately 700 feet of 2-inch diameter Schedule 40 polyvinyl chloride (PVC) conveyance piping in attic crawl space and on building exterior near SVE system compound;
• Remove all conveyance pipe metal hanging straps and screws from wooden trusses in attic crawl space;
• Remove all conveyance piping Unistrut on building exterior;
• Seal interior sheetrock firewall penetrations using sheetrock patch (2 walls to be patched);
• Seal exterior building penetrations by repairing stucco;
• Disconnect and remove underground, electrical wiring in conduit (approximately 40 feet) from SVE system to temporary power pole (overhead electrical power line disconnected from temporary power pole by Tampa Electric Company in June 2021). The underground conduit will be cut and capped below land surface;
• Dispose of all PVC pipe, metal hanging straps, Unistrut, electrical conduit, etc. from the decommissioning activities at proper off-Site landfill; and
• Properly prepare SVE system for long-term storage by sealing or capping all penetrations and closing all valves.
The following components of the SVE system will remain after the decommissioning:
• SVE system compound including SVE system with exhaust stack and four anchors, geotextile fabric liner, and chain-link fencing with privacy slats;
• Temporary power pole;
• Metal flashing covering the former penetrations of the conveyance piping on the building exterior; and
• Four interior vapor extraction wells (i.e., VEW01 to VEW04) and associated 2-inch PVC conveyance piping that runs vertically from each vapor extraction well through the drop tile ceiling and into the attic crawl space (in the event the extraction wells are necessary for future remediation efforts at the Site); the conveyance piping will be cut approximately 1-foot above the attic floor and sealed with a 2-inch diameter PVC end cap.
Action Environmental was the lowest bidder for decommissioning the SVE system and is the recommended contractor. The decommissioning bids are provided in Attachment A. Action Environmental anticipates the decommissioning activities can be completed in 2 days. Geosyntec proposes using one field staff to provide oversight throughout the duration of the decommissioning activities.
Geosyntec will prepare a brief letter report that documents the SVE system decommissioning activities and includes a photographic log of the activities.
Subtask 3 – Groundwater Monitoring
Geosyntec last conducted groundwater sampling at off-Site monitoring well locations in December 2020 and at two locations on-Site in July 2021. The purpose of following groundwater sampling plan is to monitor wells with exceedances on- and off-Site to evaluate the extent of groundwater contamination on an annual basis.
Geosyntec will collect groundwater samples from two on-Site monitoring wells (i.e., MW002 and MW006) and six off-Site monitoring wells (i.e., MW010 to MW012 and MW016 to MW018).
Subtask 4 – Groundwater Monitoring Report
Approximately 30 days after receipt of groundwater analytical results, Geosyntec will submit an Annual Groundwater Monitoring Report that will be signed and sealed by a licensed Florida engineer or geologist and include field forms and updated tables and figures. The field forms will consist of groundwater sampling, calibration, DTW, and drum logs generated during sampling activities. The tables will include (i) DTW measurements, (ii) groundwater analytical results for VOHs, and (iii) geochemical field parameters. The figures will include (i) Site location Plan, (ii)
Site map, (iii) groundwater elevation contours in the surficial aquifer, (iv) groundwater elevation contours in the intermediate aquifer, (v) groundwater analytical results in the surficial aquifer, and
(vi) groundwater analytical results in the intermediate aquifer.
This subtask also includes time to updates the FDEP ADaPT files and associated GIS data tables for this Site after receipt of the analytical data.
Subtask 5 - IDW Management
This subtask includes generation and management of groundwater investigation-derived waste (IDW), which is estimated to include one drum (see Subtask 3). Universal Environmental Solutions, LLC. is the lowest cost provider of transportation and disposal services (per drum).
Geosyntec will review the generation of a waste characterization profile as well as prepare the IDW manifest. The IDW transportation and disposal quotes are provided in Attachment B.
Subtask 6 – Two Quarters of Vegetation Maintenance
This subtask includes labor and expenses to maintain vegetation within the SVE system compound for two quarters while the SVE system remains on-Site (February through June). Geosyntec will manually grub vegetation and/or utilize a weed trimmer to maintain vegetation within the SVE system compound. Geosyntec assumes any vegetation or trash that is removed from the SVE system can be disposed in the dumpster on-Site. Additionally, Geosyntec will inspect the condition of the seals and/or caps installed on the SVE system as part of preparation for long-term storage during the vegetation maintenance visits. Geosyntec will photo document the SVE system compound before and after the vegetation maintenance activities.
Geosyntec will prepare quarterly email summaries of each event that include select photos of the vegetation maintenance activities.
Invoice and Deliverable Schedule
Please note the following schedule is contingent on FDEP review and approval of this SOW/LOE within two weeks of submittal. We have tentatively scheduled groundwater sampling for the week of 2/28/2022.
