Subtask 3 – Excavation and Field Oversight:

Based on Hepaco’s schedule, Geosyntec has assumed up to 15 days (including 3 contingency days) of Construction Quality Assurance (CQA) oversight by Geosyntec’s onsite CQA Manager. In addition, the local Design Engineer or EOR will complete weekly inspections of the work; their inspections will alternate weeks. It has been assumed that two weekly inspections will be performed by the local Design Engineer and one weekly inspection will be performed by the EOR prior to the final site inspection [three total inspections]. Prior to earthwork activities, Geosyntec will observe the following activities completed by the Contractor: (i) private utility locate; and (ii) installation of security fencing and establishment of erosion control measures. Geosyntec and the Contractor will document of utilities encountered during the private utility locate. During earthwork activities, Geosyntec will provide daily oversight and document excavation-related activities (including ambient vapor monitoring using a photoionization detector and visual dust monitoring). Geosyntec estimates that approximately 20 samples will be collected during excavation activities and submitted to Pace Analytical Laboratories for analysis on a maximum 5-day turnaround time.

The EOR will maintain contact with the onsite CQA Manager and Construction Manager throughout the field phase. The EOR will complete the following activities:

• Provide technical support to onsite personnel daily, receive progress updates at the end of each day, and review the daily construction field report.

• Review, log, and approve daily Contractor submittals.

• Prepare a summary of construction activities twice a week via email.

• Review the waste tracking spreadsheet prepared by the Construction Manager.

• Conduct biweekly site inspections. It has been assumed that an onsite meeting with FDEP personnel can be completed at the same time.

o Contractor is responsible for providing interim and final weigh tickets to Geosyntec twice a week.

• Sign non-hazardous manifests on behalf of FDEP.

• Review payment of ISR bid line items associated with waste transportation and disposal per the measurement and payment section of the Bid Specification Package.

The Design Engineer will maintain contact with the EOR throughout construction and will complete the following activities:

o Provide technical support during the completion of excavation, as needed.

o Conduct biweekly site inspections (alternating EOR visits).

o Conduct the final site inspection following Contractor’s completion of items identified on the demobilization punch list.

Prior to complete demobilization by the Contractor, Geosyntec’s onsite CQA Manager and EOR will complete a final inspection with the Contractor’s representative. A punch list will be generated by the onsite CQA Manager and EOR identifying items that are required by the Contractor for completion of ISR activities.

Following completion of field activities, the EOR will review the following items to be submitted by the Contractor:

• Interim and final weigh tickets;

• Backfill delivery tickets;

• Final survey reports;

• Geotechnical reports showing the backfill soil meets the compaction requirements; and

• Final waste manifests with disposal facility signatures.

Geosyntec will collect waste characterization samples from stockpiled excavated soils for total and TCLP VOH analysis via EPA Methods 8260B and 1311, respectively. Geosyntec estimates that approximately 20 samples will be collected during excavation activities and submitted to Pace Analytical Laboratories for analysis on a maximum 5-day turnaround time.

The Project Manager will support the EOR and Construction Manager by managing communications with FDEP and the property owner. This communication will include a summary of construction activities and schedule at least twice weekly via email.

The EOR will maintain contact with the onsite CQA Manager and Construction Manager throughout the field phase. The EOR will complete the following activities:

• Provide technical support to onsite personnel daily, receive progress updates at the end of each day, and review the daily construction field report.

• Review and approve the density testing results during backfilling.

• Conduct a final site inspection punch list with the onsite CQA Manager.

• Approve payment of ISR bid line items per the measurement and payment section of the Bid Specification Package.

The Construction Manager will maintain contact with the onsite CQA Manager throughout construction and will complete the following activities:

• Review, log and approve daily onsite CQA Manager field reports, including the air monitoring results.

• Review waste characterization data and draft waste manifests.

• Maintain a comprehensive list of each waste stream and the associated tonnage and unit cost for transportation and disposal. This waste tracking spreadsheet will be provided to the EOR weekly during construction.

o Contractor is responsible for providing daily estimates to Geosyntec on the quantity of excavated and disposed material.

o Contractor is responsible for providing interim and final weigh tickets to Geosyntec twice a week.

• Interim and final weigh tickets;

• Backfill delivery tickets;

• Final survey reports;

• Geotechnical reports showing the backfill soil meets the compaction requirements; and

• Final waste manifests with disposal facility signatures.

Geosyntec will submit copies of the Daily Field Construction Logs (with representative photographs), weight tickets (interim and final), and waste disposal manifests (generator and transporter signatures only) as the milestone for invoicing this SOW/LOE.

Subtask 4 – Interim Source Removal Completion Report: Geosyntec will prepare a Construction Completion Report (CCR) to summarize the ISR construction-related activities. The CCR will include tables and figures summarizing the extents and volume of excavated soil, the volumes and weights of each waste stream produced and methods of treatment and disposal, as applicable, weight tickets for disposed soil and backfilled material, photo documentation, final waste manifests (signed by landfill(s)), and required permits. The Report will be signed and sealed by the EOR.
