This Task Assignment authorizes Geosyntec to conduct Site Assessment activities at Aurora Foundation Building in accordance with the Geosyntec scope of work, cost estimate, and schedule received on June 21, 2023.

Geosyntec confirmed on June 21, 2023, that Subtask 1 consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.

Subtask 2: This subtask includes cost site assessment activities, which include a private underground utility survey by Geotek, concrete coring up to 7 locations (SB004 through SB008, DPT005, and DPT006), one lithology boring (SB001), MAGS installation (MAGS001 – MAGS005) at locations (SB004 – SB008) as well testing will occur.

Geosyntec will also do soil sampling at up to 8 locations (SB001 – SB008) and samples will be analyzed for VOCS by synthetic precipitation leaching procedure (SPLP). Direct push technology (DPT) groundwater sampling will occur at up to 6 locations (DPT001 – DPT006).

Monitoring wells will be installed with up to 4 exterior wells with one vertical well.

Although PDS and Geoview were the lowest bidders, Action and GeoTek, the second lowest bidders, had the earliest availability.

Subtask 3: This subtask includes cost for a Florida professional land surveyor to survey the newly installed monitoring wells. Cost also include groundwater sampling from newly installed wells. The groundwater samples will be analyzed for VOCs by USEPA Method 8260 (full list), PAHs (partial list with acenaphthene, acenaphthylene, and naphthalene’s) by USEPA Method 8270-SIM, and TRPH by the FL-PRO method

Subtask 4: This subtask includes cost for IDW management. Subject: Scope of Work and Level of Effort – Site Assessment Aurora Foundation Building 1115 6th Avenue West Bradenton, Florida 34205 FDEP Site ID No. ERIC_4904

Dear Ms. Plyer, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work and Level of Effort (LOE) for site assessment activities at the Aurora Foundation Building (Site) located in Bradenton, Florida (ERIC_4904). Geosyntec prepared this LOE based on Geosyntec’s 13 March 2023 Site Assessment Work Plan (SAWP), approved by FDEP. A description of task breakdown for Site assessment field activities and report preparation is provided below.

SUBCONTRACTOR EVALUATION

Geosyntec obtained three bids from licensed and qualified utility locating, concrete coring, drilling, and IDW transportation and disposal firms; a cost comparison is provided in Attachment A. Geosyntec recommends the following subcontractors based on lowest cost bid (or earliest availability) among the qualified bidders:

• Underground utility locating – GeoTek Services, LLC

• Concrete Coring - Speedy Concrete Cutting, Inc (Speedy).

• Soil sampling, DPT groundwater sampling, and MW installation – Action Environmental (Action) (5 total field days)

• IDW transportation and disposal – Erwin Remediation

• SurvTech Solutions, Inc. is recommended for surveying the four (4) monitoring wells.

Copies of the bids from each subcontractor are included in Attachment A.

Ms. Brittany Plyer 21 June 2023

For ease of reference, Table 1 and Figures from the SAWP are attached as a summary of the field activities and samples to be collected.

Subtask 1 – Project Management

Fabio Fortes (Tampa Bay) will be the Geosyntec Project Manager. His labor hours are directly computed.

Subtask 2 – Multi-Media Sampling (Soil, DPT groundwater, soil vapor) and Monitoring Well Installation

Subtask 2 includes labor for field preparation, kick off meeting with FDEP, property owner, and construction contractor (because the building is under active construction), and preparation of a Site Health and Safety Plan. Geosyntec personnel will mobilize to the Site to observe the site assessment activities, which include a private underground utility survey, concrete coring, lithology boring, soil sampling, direct push technology (DPT) groundwater sampling, and monitoring well installation.

Utility locating: Geosyntec will observe utility locating at the Site performed by GeoTek.

Identified utilities will be mapped by Geosyntec staff using a Global Position System (GPS) unit following the private underground utility survey.

Concrete Coring: Geosyntec personnel will oversee coring activities within the Site building by a concrete coring contractor. Based on observations during the SAWP site visit, it is anticipated that the foundation inside the Site building may be up to 12 to 36 inches thick. The contractor should core the concrete up to 7 locations (SB004 through SB008, DPT005, and DPT006). Speedy will complete the boring holes with concrete at the end of the sampling.

Modified Active Gas Sampling (MAGS) installations and testing: Five, 2-inch (in.) diameter Schedule 40 polyvinyl chloride (PVC) MAGS wells (MAGS001 through MAGS005) will be installed (at location SB004 through SB008) by hand auger to a depth of approximately 4 ft BLS (or 1-ft above the static water level if the depth to groundwater is different than historically observed). Each MAGS well will be installed with 1-ft of riser and 10-ft of 0.01-in. slotted well screen extending from approximately 1 to 4 ft BLS. A 20/30 grade environmental sand filter pack will be placed from the bottom of the boring to the top of the well screen. The remaining annular space will be grouted to just below the top of casing.

Each MAGS well will be tested for a period calculated to remove one, 10 to 15-ft radius of soil vapor volume using a blower. The test duration is expected to be approximately 1 hour. The vacuum and flow rates used at each MAGS well will be based on the results of a step test at one interior location. The step test will include a minimum of three steps at increasing vacuums to evaluate optimum airflow rates. Vacuum will be monitored at adjacent MAGS wells at approximate 15-minute intervals, as practical, during MAGS testing. After the completion of the study, the MAGS wells will be left in place, capped, and covered with a flush-mount manhole so that they may be converted into soil vapor extraction wells and resampled as necessary in the future.

During testing, soil vapor will be monitored with an organic vapor analyzer equipped with a photoionization detector (PID) utilizing a 10.6 electronic volt lamp. Soil vapor samples will be collected at the end of each MAGS test using a batch-certified “clean” Summa canister provided by the analytical laboratory. The soil vapor samples will be sent to a fixed-based laboratory for analysis of VOCs by the TO-15 method.

It is possible that the vadose zone available for testing may be too thin due to the concrete thickness and water table depth. Geosyntec will evaluate in the field the ability to convert soil borings advanced (SB004 through SB008) in and around the suspected former suite into MAGS wells.

Geosyntec will notify FDEP for any change in the scope of work and prepare a WCD or TACO, if necessary.

Lithology boring: One soil boring (at SB001 location) will be advanced using a DPT drill rig to a depth of approximately 50-feet below land surface (ft BLS) or refusal (limestone is expected around 30 ft BLS), whichever is encountered first, to characterize the Site-specific lithology. This data will be used to assist in the selection of soil samples and DPT groundwater sampling intervals.

Soil Sampling: Soil sampling will be performed at up to 8 locations (SB001 through SB008) as presented on Figure 3 in the FDEP-approved SAWP. Based on an assumed depth-to-water (DTW) of 5 ft BLS, a minimum of three soil samples 0-0.5 ft BLS, 0.5-2 ft BLS, and 2-4 feet below land surface [ft BLS] or water table) will be collected from each of the 8 proposed soil sample locations.

Soil samples will be collected by hand auger. A concrete core drill will be used to access soil under the building slab (concrete) and driveway/parking lot (asphalt).

The soil samples will be analyzed for VOCs by USEPA Method 8260 (full list), PAHs (partial list with acenaphthene, acenaphthalene, and naphthalenes) by USEPA Method 8270-SIM, and TRPH by the FL-PRO method by a state of Florida certified, National Environmental Laboratory Accreditation Program (NELAP) accredited fixed-based laboratory. Soil samples will be screened with a photoionization detector (PID) organic vapor analyzer. Supplemental soil samples will be collected from each location and depth interval (via Encore samplers) for analysis of VOCs by the synthetic precipitation leaching procedure (SPLP) should VOC concentrations exceed the leachability-based soil cleanup target levels (SCTLs). PAH analysis by SPLP and TRPH speciation are also proposed for the soil analyses exceeding the applicable SCTLs. For budgetary purposes, it is assumed that approximately half of the proposed soil samples will be analyzed by

SPLP for VOCs and PAHs, and TRPH speciation (Eurofins quote for TRPH speciation is presented in Attachment A). Exterior soil samples will be surveyed with a GPS and interior sampling locations will be transcribed using as-builts based on Site features.

DPT groundwater sampling: Geosyntec personnel will observe the collection of DPT groundwater samples at up to 6 locations (DPT001 through DPT006) as shown on Figure 3 of the SAWP.

Groundwater samples will be collected at 4-ft intervals approximately every 10 feet (i.e., 5 to 9 ft BLS, 15 to 19 ft BLS, and 25 to 29 ft BLS assuming refusal at limestone is encountered at 30 ft BLS), resulting in 3 samples x 6 locations (18 samples). Sampling intervals may be adjusted based on observed depth to water and Site lithology. Groundwater samples submitted to a NELAP-approved fixed-based laboratory will be analyzed for the presence of VOCs by USEPA Method 8260 (full list), PAHs (partial list with acenaphthene, acenaphthalene, and naphthalenes) by USEPA Method 8270-SIM, and TRPH by the FL-PRO method. No field duplicates will be collected.

Monitoring well installations: In the same mobilization, Geosyntec personnel will observe the installation of up to 4 exterior monitoring wells to evaluate groundwater quality; the proposed locations are presented on Figure 3 of the SAWP. We have assumed the existing utility survey will be sufficient for siting the proposed monitoring well locations; however, should the spray paint be worn away or if there is any uncertainty in utility locations relative to drilling locations, Geosyntec will submit a WCD or TACO to FDEP for a focused utility locating at the drilling locations.

Water-table monitoring wells MW001 through MW003 will be installed via DPT drilling. The wells will be constructed of 1-in. diameter Schedule 40 PVC pipe with 10 ft of 0.010-in. slotted well screen, filter pack consisting of 20/30 silica sand, and at least a 0.5-ft thick, fine sand (30/65 silica sand) seal. The remaining annular space will be grouted to land surface with Portland Cement Type I/II and monitoring wells will be completed at the surface with a flush-mount 8-in. manhole with a locking monitoring well cap.

Geosyntec will install one vertical extent monitoring well (MW004) to a depth of 30 feet, if DPT groundwater sampling results warrant vertical delineation. The well will also be constructed with a 5-ft screen depending upon the lithologic information from SB001.

The monitoring wells will be developed until the discharge is relatively sediment-free and sampled no sooner than 24 hours after development.

Field GPS and Interior Mapping: Exterior sampling locations (DPT groundwater and soil boring) will be surveyed throughout the week with a field GPS by Geosyntec personnel. Interior sampling locations will be transcribed using as-builts based on Site features.

Subtask 2 activities will be performed by Geosyntec staff members assuming 1 day for utility locating and marking the boring locations (1 Geosyntec staff), 3 days for concrete coring (1 Geosyntec staff), 2 days for MAGS well installation and testing (2 Geosyntec staffs), and 5 days for drilling oversight and close out tasks at the Site (2 Geosyntec staffs). We have assumed 10-hours per day.

Subtask 3 – Groundwater Sampling and Surveying

Monitoring Well Survey: This subtask includes costs for a Florida professional land surveyor to survey the four (4) newly installed monitoring wells. Cost has been included for Geosyntec to oversee the survey.

Groundwater sampling: Geosyntec personnel will collect groundwater samples from the four (4) Site monitoring wells. Depth to water measurements (DTW) will be collected from the monitoring wells to evaluate groundwater flow direction. The monitoring wells will be sampled using a peristaltic pump in general accordance with FDEP Standard Operating Procedures (SOPs), assuming the DTW and drawdown do not exceed the suction limit of the peristaltic pump. The samples will be stored on ice and shipped to a NELAP approved, fixed-based analytical laboratory under chain of custody for analysis under standard turnaround times. The groundwater samples will be analyzed for VOCs by USEPA Method 8260 (full list), PAHs (partial list with acenaphthene, acenaphthalene, and naphthalenes) by USEPA Method 8270-SIM, and TRPH by the FL-PRO method.

Field Deliverable and Draft Summary Figure: Geosyntec has included costs to generate the following to facilitate the discussion with FDEP:

• One figure and table to summarize laboratory analytical results from the DPT groundwater samples;

• One figure and table to summarize laboratory analytical results from soil samples; and

• One figure and table to summarize laboratory analytical results from the groundwater monitoring well samples.

These data will be provided to FDEP for evaluation. The deliverable will include the final surveyor report, field forms for soil and groundwater assessment, and groundwater sampling forms. If fixed-based laboratory analytical results of soil and groundwater samples indicate that additional sampling will be required to complete delineation, Geosyntec will propose additional sampling locations and corresponding depths.

Subtask 3 activities will be performed by 1 Geosyntec staff member assuming 1 day for monitoring well survey oversight, and 1 day for monitoring well sampling, IDW sampling, and close out tasks at the Site. We have assumed 10-hour per day.

Subtask 4 – IDW Management

IDW Sampling: The IDW will be sampled prior to profiling. One composite liquid and one composite soil sample will be collected and analyzed for VOCs and metals using the Toxic Characteristic Leaching Procedure. Geosyntec will review the generation of a waste characterization profile as well as the preparation of IDW manifests. Geosyntec will coordinate disposal arrangements for up to 5 drums of non-hazardous IDW. Analytical costs are included to establish the soil and groundwater profiles required by the selected waste hauler. We have assumed field oversight of the drum pickup is not necessary.

For this cost estimation, we assumed the IDW will not be impacted by (screening to be performed under HW028A) and Geosyntec will submit a WCD or TACO to FDEP for any change of scope.

Subtask 5 – Site Assessment Report

Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a Site Assessment Report (SAR) documenting assessment activities and summarizing analytical results. This subtask includes costs to generate a SAR that summarizes the analytical results of the assessment activities outlined above and complies with the requirements of Chapter 62-780.600, FAC. The costing included in the CEF/LOE includes the following figures:

• USGS Site Topographic Map

• Potable Well Survey

• Site Layout

• Site Utility Map

• Lithologic Cross Section (1 transect)

• Summary of Soil Analytical Results from 0-0.5, 0.5-2, and 2-4 ft BLS (3 figures)

Hi Matt, we only need the service inside the building (up to 7 locations) and let’s think in the worst scenario (3 ft of concrete). Can you send me the cost to fill the concrete back after the concrete coring work?

Thank you, Professional Engineer

I believe you had asphalt also.
