HW683 DC014A
This Task Assignment authorizes Geosyntec to conduct supplemental site assessment activities at the Carriage Cleaners site
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2 – Modified Active Gas Sampling (MAGS) Subtask 2 includes costs for Geosyntec personnel to mobilize to the Site to re-test four MAGS wells (MAGS-004, -005, -006, and -008) and to remove one, 10 to 15-ft radius of soil vapor volume using an appropriately sized blower capable of generating up to 50 standard cubic feet per minute. As there was minimal change in the calculated ROI between 20 and 24 in-H2O during the October 2020 step tests, each MAGS test will be conducted with at least 24 in-H2O of applied vacuum with the goal of applying the maximum vacuum the system can produce. A soil vapor sample will be collected at each MAGS well after approximately one pore volume has been extracted and sent to a NELAP approved, fixed-based laboratory for analysis by EPA Method TO-15.
Subtask 3 – Groundwater Sampling Subtask 3 includes costs for Geosyntec personnel to mobilize to the Site to conduct groundwater sampling activities of Site monitoring wells. Prior to sampling, depth to water measurements will be collected from Site monitoring wells to evaluate groundwater flow direction. The monitoring wells will be sampled using a peristaltic pump in general accordance with FDEP Standard Operating Procedures (SOPs).
Subtask 4 – Supplemental Site Assessment Report Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a SSAR documenting the supplemental Site assessment activities and summarizing analytical results. As part of this subtask, Geosyntec will import and tabulate the GIS data in the select electronic data deliverable format.

Subtask 5 – IDW Management Subtask 5 includes costs for IDW transport and disposal.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $17,301.35 with 116 labor hours.
Geosyntec may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Mr. Evan Miller Florida Department of Environmental Protection 2600 Blair Stone Road Tallahassee, FL 32399-2400
Subject: Supplemental Site Assessment Scope of Work/Level of Effort Carriage Cleaners 3920 Confederate Point Road Jacksonville, Duval County, Florida FDEP Facility ID ERIC_4578
Dear Mr. Miller, Geosyntec has prepared this Scope of Work and Level of Effort (LOE) for supplemental site assessment activities at the Carriage Cleaners site (Site) located in Jacksonville, Florida (ERIC_4578). Geosyntec prepared this LOE based on FDEP’s 8 July 2021 Site Assessment Report (SAR) approval letter and subsequent discussions during the annual FDEP meeting on 26 April 2022. A description of task breakdown for Site assessment field activities and report preparation is provided below.
Subtask 1 – Project Management
Subtask 2 – Modified Active Gas Sampling (MAGS)
Subtask 2 includes costs for Geosyntec personnel to mobilize to the Site to re-test four MAGS wells (MAGS-004, -005, -006, and -008) to remove one, 10 to 15-ft radius of soil vapor volume using an appropriately sized blower capable of generating up to 50 standard cubic feet per minute.
The testing will be performed in accordance with the approach outlined in the April 2021 SAR.
The results of the MAGS study will be used to evaluate whether interim source removal efforts are warranted to address vadose zone impacts beneath the Site building.
Subtask 3 – Groundwater Sampling
Subtask 3 includes costs for Geosyntec personnel to mobilize to the Site to conduct groundwater sampling activities of Site monitoring wells. Prior to sampling, depth to water measurements will be collected from Site monitoring wells to evaluate groundwater flow direction. The monitoring wells will be sampled using a peristaltic pump in general accordance with FDEP Standard Operating Procedures (SOPs). The samples will be stored on ice and shipped to a NELAP approved, fixed-based analytical laboratory under chain of custody for analysis under standard turnaround times. The groundwater samples will be analyzed for VOHs by EPA Method 8260.
Subtask 4 – Supplemental Site Assessment Report
Following completion of field activities, Geosyntec will proceed with the preparation and submittal of a SSAR documenting the supplemental Site assessment activities and summarizing analytical results. As part of this subtask, Geosyntec will import and tabulate the GIS data in the select electronic data deliverable format.
Subtask 5 – IDW Management
Subtask 5 includes costs for IDW transport and disposal. Bid costs prepared by the IDW haulers are summarized below and attached. The current non-hazardous waste profile with Hulls Environmental is assumed to be sufficient for IDW management efforts.
Water (per 55-gal. drum) Hulls Environmental $175 Hepaco $191.27 + $500 Stoppage Fee Universal Environmental Solutions $215
Deliverable and Invoicing Schedule:
Subtask Deliverable Deliverable Due Date
Invoice Amount
2 Field deliverable for MAGS testing 7/8/22 $7,910.70
3 Field deliverable for groundwater sampling 8/12/22 $3,674.15
1, 4, 5 Submittal of SSAR, Final Waste Manifest, and GIS Data Tables, and ADaPT files 9/16/22 $5,716.50
Total $17,301.35
Geosyntec appreciates the opportunity to provide this proposal and cost estimate to you and the FDEP. Please contact us if you have any questions or require additional information.
Sincerely, Blaine Dawson, P.G.
