Ms. Sarah Aduwa Florida Department of Environmental Protection Waste Site Cleanup Section 2600 Blair Stone Road Tallahassee, FL 32399-2400
Subject: Scope of Work and Level of Effort Monitoring Well Installations, Annual Groundwater Monitoring, and Preliminary PFAS Screening Vick’s Cleaners, Inc.
302 Gulf Breeze Parkway Gulf Breeze, Santa Rosa County, Florida
FDEP ID ERIC_5421
Dear Ms. Aduwa, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW), Cost Estimating Form (CEF), and Level of Effort (LOE) for monitoring well installations and annual groundwater monitoring activities at the former Vick’s Cleaners Site (Site) located in Gulf Breeze, Florida.
Supplemental assessment activities proposed in this SOW are generally consistent with recommendations provided in the 2023 Annual Groundwater Monitoring Report submitted in July 2023 and subsequent FDEP correpondence. A copy of the CEF/LOE is included in Attachment A.
Subtask 1 – Project Management
Costs associated with this subtask are directly computed.
Subtask 2 – Offsite Access Procurement
This subtask includes costs related to procurement of access to offsite locations of two of the three proposed monitoring wells. Based on assumed difficulties in obtaining access to private offsite properties, procurement of Right of Way Permits from the City of Gulf Breeze will be conducted for work in the Rights of Way of Live Oak Street and Fairpoint Drive. The most recent groundwater analytical results are illustrated in Figure 1 and the proposed locations of the wells are provided in Figure 2 of Attachment B.
Ms. Sarah Aduwa April 5, 2024
Vicks Cleaners (ERIC_5421)_HW683 MW Installs-Annual GW Monitoring LOE-CEF_r1
Subtask 3 – Monitoring Well Installation
This Subtask includes costs associated with the installation of three downgradient monitoring wells (Figure 2). The monitoring wells (MW013, MW014, and MW015) will be installed using direct-push technology (DPT) to approximately 45 feet (ft) below land surface (BLS). The monitoring wells will be constructed of 10 ft of one-inch diameter Schedule 40 polyvinyl chloride (PVC) pre-packed well screen slotted at 0.010-inches and blank Schedule 40 PVC riser. The filter pack will consist of 20/30 silica sand placed around the well screen to approximately 0.5-ft above the top of the well screen, followed by a 0.5-ft thick 30/65 fine sand seal and Portland Cement grout to the surface. The monitoring wells will be competed flush-to-grade in 8-inch diameter manholes embedded in 2-ft by 2-ft concrete pads. The monitoring wells will be developed until visually clear. Soil cuttings and development water will be containerized in drums and staged on-site.
Geosyntec obtained cost estimates from four drilling subcontractors - Preferred Drilling Solutions (PDS), Walker Hill Environmental, Groundwater Protection (GWP), and Environmental Drilling Service, Inc (EDS). The quotes are provided in Attachment C. PDS provided the most competitive bid and has been selected for this work.
Prior to conducting monitoring well installation activities, Geosyntec will observe a subsurface survey to identify potential subsurface utilities or obstructions. Subcontractor quotes are provided in Attachment D. GPRS provided the most competitive bid and has been selected for this work.
If well installations within a public ROW are necessary, costs for Maintenance of Traffic (MOT) have been provided. Acme Barricades provided the only responsive bid and has been selected for this work, if necessary. The bid from Acme Barricades and several no-bid responses are provided in Attachment E. The bid assumes that MOT is conducted in accordance with FDOT Index 602.
Subtask 4 – Annual Groundwater Monitoring and Preliminary PFAS Screening
This Subtask includes costs associated with annual groundwater monitoring activities. At least one week following the installation of the additional monitoring wells in Subtask 3, Geosyntec will mobilize to the Site to collect depth-to-water measurements from 15 monitoring wells in the Site-wide monitoring well network (MW001 through MW012, and new monitoring wells MW013, MW014, and MW015). Measurements will be collected over a 24-hour period to generate groundwater elevation contour maps to evaluate groundwater flow directions.
Groundwater monitoring well samples will be collected from the 15 monitoring wells.
Groundwater samples will be collected in accordance with FDEP Standard Operating Procedures and submitted for laboratory analysis of volatile organic halocarbons by Environmental Protection Agency Method 8260.
Vicks Cleaners (ERIC_5421)_HW683 MW Installs-Annual GW Monitoring LOE-CEF_r1
Geosyntec will also sample four (4) Site monitoring wells for PFAS to screen for its presence or absence. The proposed monitoring wells for PFAS screening are outlined in the table below.
Well ID Screened Interval
(ft BLS) Rationale
MW001 8.3 – 18.3
Water table well in front and downgradient of former drycleaner
MW002 35 - 45
Area with highest VOH impacts near sanitary sewer lift station
MW003 8 - 18
Water table well at rear of former drycleaner
MW004 40 - 50
Intermediate well at rear of former drycleaner ft BLS indicates feet below land surface.
Samples will be collected using a peristaltic pump in accordance with the draft SOPs for PFAS sampling developed by FDEP, and the SOPs for PFAS sampling prepared by Geosyntec for use at FDEP’s Site Investigation Section fire training facilities. PFAS-free water will be needed to decontaminate the water level meter between sampling each monitoring well. Geosyntec will purchase PFAS-free water from a grocery store approved by FDEP.
Geosyntec understands that the FDEP laboratory will analyze up to four (4) groundwater samples for PFAS, provide 1 liter of PFAS-free water for an equipment blank, and ship the sampling kits to Geosyntec. Based on the location of the Site, Geosyntec anticipates shipping the samples to the FDEP laboratory. Geosyntec assumes that the FDEP laboratory will provide return shipping labels with the sampling kits.
Following the collection of the monitoring well samples, purge water will be stored in 55-gallon drums on Site. To meet waste profiling requirements, a composite sample will be collected for PFAS; benzene, toluene, ethylbenzene, and total xylenes; VOHs, and the 8 Resource Conservation and Recovery Act metals analyses. If PFAS are not detected in the investigation derived waste (IDW) characterization sample, then a Task Assignment Change Order will be prepared to reduce the disposal costs to reflect VOH pricing rather than PFAS-impacted IDW pricing.
Subtask 5 - Monitoring Well Survey and IDW Disposal
Following the installation of the proposed additional monitoring wells and groundwater sampling activities, transportation and disposal of the IDW drums will be conducted. Bid costs prepared by the waste disposal companies are included in Attachment F. Based on the bids, Geosyntec recommends selecting Erwin Remediation to haul and dispose of IDW generated. We have
Vicks Cleaners (ERIC_5421)_HW683 MW Installs-Annual GW Monitoring LOE-CEF_r1 assumed no oversight is required for drum pickup and that IDW will be disposed of as non-hazardous waste.
This subtask also includes costs for a Florida professional land surveyor to survey the horizontal coordinates, top of casing elevations, and ground surface elevations of the three (3) newly installed monitoring wells. A copy of the surveyor’s quote is included in Attachment G.
Subtask 6 – Annual Groundwater Monitoring Report
Geosyntec will prepare a Groundwater Monitoring Report documenting the VOH results of the groundwater monitoring event. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the field activities completed. A final signed waste manifest for IDW transport and disposal will also be included in the Groundwater Monitoring Report. The report will be signed and sealed by a Florida-licensed professional geologist or engineer.
Subtask 7 – PFAS Screening Deliverable
Geosyntec will prepare an email deliverable with attachments including one figure with groundwater analytical results for PFAS (showing PFOA, PFOS, and the summation of PFOA and PFOS concentrations for each well sampled and no isoconcentration lines), certified analytical laboratory report, and field forms including equipment calibration logs, groundwater sampling forms, and an IDW drum log. The email deliverable will be submitted to the FDEP within 2 working days of receipt of the final certified analytical laboratory report from the FDEP laboratory.
Geosyntec assumes the final analytical laboratory report will be emailed by the FDEP laboratory after the report is finalized.
Deliverable and Invoicing Schedule:
Geosyntec proposes the following deliverables and Period of Performance schedule below.
Subtask Deliverable Deliverable Date Invoice Amount
2, 3 Well Installation Field Deliverable July 8, 2024 $16,306.59

ProCheck Inspections appreciates the opportunity to provide this proposal. Feel free to contact me at any time if you have any questions, or if you need additional information.
SCOPE OF WORK
We understand the scope to be to search for underground utilities as identified by the customer prior to clearing for soil boring and monitoring well locations. We will attempt to trace any utilities for which there are structures visible from the work area, and mark said utilities with paint and flags where possible. The client will be responsible for providing drawings or notify ProCheck Inspections of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival. Considerations were given to the fact that there are 6 different locations that vary in size and potential complexity of the associated locate for each. It is our assessment that two to three of the areas have a much higher potential for being more congested thus taking longer to conduct the locates. We’re estimating that the amount of time needed to complete the desired tasks will range from 4-5 hours.
EQUIPMENT
350 MHz Hyperstacking GPR Antenna. The antenna is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, standing water, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. GPR works by sending pulses of energy into a material and recording the strength and the time required for the return of the reflected signal. Reflections are produced when the energy pulses enter a material with different electrical properties from the material it left. The strength of the reflection is determined by the contrast in signal speed between the two materials. The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the conductivity of the materials. The patented hyperstacking antenna can improve depth penetration and resolution when compared with a traditional GPR antenna.
Electromagnetic Pipe Locator. The EM locator can detect the electromagnetic fields from live power or radio frequency signals. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting EM field can then be detected by the receiver. The receiver is moved over the surface without contacting the ground, so it is not affected by terrain. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. Depths achieved can be as much as 20’ depending on the type of signal being traced or methods used.

TERMS & CONDITIONS
1. ProCheck Inspections provides Ground Penetrating Radar (GPR) services in underground applications, including assisting in location services in accordance with ASCE Standard 38-02, Quality Level “B”. GPR is a great tool for detecting objects underground, however, site restrictions and environmental conditions can limit the accuracy and effective depth of GPR. For underground scanning, depth is completely dependent on the composition of soils in the area being surveyed. Customer fully understands that for every foot in depth penetration with the GPR equipment, the pipe/utility must be at least 1” in diameter to be located. For example, at 4 feet in depth, the pipe/utility must be 4” or larger to be detected. Some types of pipes are very difficult to locate, such as clay or concrete pipes, transite piping, and empty PVC type pipes. Given these factors, ProCheck Inspections CANNOT guarantee it will be able to locate ALL utilities on site.
2. Our goal is to provide you with the best answers possible to your questions regarding what lies below the surface, and where it is located. Customer acknowledges that they understand the fact that our answers are based upon the quality of the data retrieved and what ProCheck Inspections believes lies below the surface. The decision to proceed with boring or excavations of any manner, type, or method, it under the sole discretion of the customer and customer assumes full liability for aforementioned processes.
3. ProCheck Inspections will not accept liability for an inaccurate interpretation or any other reason, and customer agrees to release and indemnify ProCheck Inspections and its owners and agents from all losses and damages from all alleged negligence and/or contract claims by customer or any third party. These terms and conditions supersede any other previous terms and conditions either oral or written.
4. Payment is due upon receipt of invoice unless otherwise specified in writing.
5. If for some reason ProCheck Inspections arrives on site and the work is cancelled without prior notification, there will be a charge of
$300.00.

Thanks Nick. With the mobilization it would be $2,750 for the clearance of the three holes. Sorry for the high bid, but Pensacola is a haul for us and would take somewhat out for 2 days, along with the overnight associated costs.

Hello, I am interested in getting a quote for Maintenance of Traffic rental and setup/takedown for work we are planning to do in Gulf Breeze, FL. Our project name is Vick’s Cleaners and our work is near the intersection of Gulf Breeze Parkway and Northcliff Drive/Fairpoint Drive as shown on the attached figure.
I believe our work will be over 2 days, with MOT setup/takedown in one location on the first day and then setup/takedown in a location across the street on the second day. Our work includes installation of groundwater monitoring wells as shown in blue boxes on the attached figure. I am thinking that we can do this work under FDOT Index 602.
Please let me know if you any questions or need any more information.
Thank you, Nick

Nick, As we just discussed, AWP Safety no longer has an office in the Pensacola, FL area.
Therefore, we would not be able to service your project in Santa Rosa County at this time.
We will be re-opening in the Florida panhandle within the next few months.
