Subject: Scope of Work and Level of Effort Groundwater Monitoring and Site Rehabilitation Completion Report Super Drycleaner 5702 Lake Worth Road Greenacres, Palm Beach County, Florida
FDEP ID ERIC_5142
Dear Ms. Aduwa, Geosyntec Consultants, Inc. (Geosyntec) has prepared this Scope of Work (SOW), Cost Estimating Form (CEF), and Level of Effort (LOE) for groundwater monitoring activities and reporting activities at the Super Drycleaner Site (Site) located in Greenacres, Florida. On 3 June 2025, Geosyntec confirmed that a Site Access Agreement (SAA) was executed with David Associates V, LLC, on 11 July 2017 as the real property owner of the Site located at the above address. The Palm Beach County property appraiser database indicates the property ownership has not changed. As a result, Geosyntec believes the current SAA remains valid. The path forward and SOW provided below was recently discussed during the Annual Site Review meeting on 29 April 2025. A copy of the CEF/LOE is provided.
Subtask 1 – Project Management
Costs associated with this subtask are directly computed.
Subtask 2 –Groundwater Monitoring
This Subtask includes costs associated with groundwater monitoring activities. Geosyntec will mobilize to the Site to collect depth-to-water measurements from the 7 Site monitoring wells in the monitoring well network (MW001 through MW007). Measurements will be collected over a 24-hour period to generate a groundwater elevation contour map to evaluate groundwater flow direction.
Groundwater monitoring well samples will be collected from monitoring wells MW004, MW005, and MW006 in accordance with the approved Interim Source Removal Work Plan (ISRWP). The samples will be collected in accordance with FDEP Standard Operating Procedures and submitted for laboratory analysis of volatile organic halocarbons (VOHs) by EPA Method 8260. Purge water
Ms. Sarah Aduwa June 3, 2025
Super Drycleaner (ERIC_5142)_HW683 GW Monitoring-SRCR SOW generated during the monitoring well sampling activities will be stored in a 55-gallon drum on- Site prior to off-Site disposal.
Subtask 3 - IDW Disposal
Following groundwater sampling activities, transportation and disposal of the IDW drum will be conducted. Bid costs prepared by the waste disposal companies are included in Attachment A.
Based on the bids, Geosyntec recommends selecting Erwin Remediation to haul and dispose of IDW generated. We have assumed no oversight is required for drum pickup and that IDW will be disposed of as non-hazardous waste based on prior analytical results (including PFAS screening in May 2023 indicating that PFAS constituents are not present in groundwater at concentrations above provisional GCTLs).
Subtask 4 – Site Rehabilitation Completion Report Preparation
Provided that groundwater concentrations are below applicable Groundwater Cleanup Target Levels (GCTLs) per Chapter 62-777, Florida Administrative Code (FAC), Geosyntec will prepare a Site Rehabilitation Completion Report (SRCR) to advance the Site to closure. The report will contain tables summarizing field and laboratory data, figures showing groundwater flow direction(s) and concentrations, and a summary of the remedial activities completed. A signed waste manifest for IDW disposal will be included in the report. The report will be signed and sealed by a Florida-licensed professional geologist or engineer.
If groundwater concentrations indicate that criteria for Site Closure are not met, the results will be discussed with FDEP, and a Groundwater Monitoring Report may only be prepared instead.
Deliverable and Invoicing Schedule:
Geosyntec proposes the following deliverables and Period of Performance schedule below.
Subtask Deliverable Deliverable Date Invoice Amount
Groundwater Sampling Field Deliverable
September 25, 2025 $2,389.65
1,3,4 Site Rehabilitation Completion Report and Final IDW Manifests
November 10, 2025 $6,935.21
Total $9,324.86
The above schedule assumes the TA is executed in June 2025
