DEP FACILITY ID NUMBER: ERIC_4531
PROPOSED TASK ASSIGNMENT: HW685_DC027A
Dear Mr. Grubbs:
Golder Associates USA Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal for biennial groundwater monitoring at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The scope of work is based on the recommendations accepted by the DEP in the June 25, 2020 electronic mail correspondence to Golder. The proposed scope of services is described below.
SUBTASK 1 – PROJECT MANAGEMENT
To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.
SUBTASK 2 – SOIL SAMPLING
Golder will contact Sunshine 811 to initiate a utility locate for underground utilities and use a private utility locator to complete an underground utility locate where the proposed 28 soil borings are located on the attached Figure 1A.
The soil borings located around the 2005 and 2016 source removals where chlorinated volatile organic compound
Mr. Ben Grubbs Proposal No. 202208346
Florida Department of Environmental Protection May 23, 2022
(VOC) concentrations exceeded the soil cleanup target levels (SCTL) in several samples collected in the top 2 feet.
Soil samples will be collected at depths of 0.5 and 2.0 feet below ground surface at each boring location and submitted to a laboratory under proper chain-of-custody procedures for VOC analysis by EPA Method 8260. A total of 56 soil samples will be collected for chlorinated VOC analysis.
Soil samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective April 16, 2018). The soil will be collected and placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program (NELAP)-certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260.
For cost estimating purposes, Golder plans to return the soil from each boring back into the hole on site. Golder estimates two days for a two-person field crew to complete soil sampling activities.
SUBTASK 3 – ANNUAL GROUNDWATER MONITORING
Golder will collect groundwater samples from 22 wells: six shallow zone monitoring wells (MW001, MW016, MW028, MW031, MW034, and MW038); seven intermediate zone monitoring wells (MW018, MW019, MW029, MW032, MW035, MW037, and MW039); six injection wells (IW010, IW017, IW018, IW021, IW022, and IW023), and three deep zone monitoring wells (MW024, MW030, and MW036). The attached figures (Figures 5, 6, and 7) from the 2021 Annual Groundwater Monitoring Report show the site layout and chlorinated VOC results for the three surficial zones of the surficial aquifer.
Prior to groundwater sampling, depth-to-water measurements will be collected using an electronic water level indicator to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.
Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation reduction potential (ORP) will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.
Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program-certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B. Groundwater samples from three of the monitoring wells (MW029, MW034, and MW035) will also be analyzed for total organic carbon using EPA Method 9060 and CHITOREM® Underground Injection Control parameters including total dissolved solids using EPA Method 160.1 and color. Upon receipt of the laboratory analytical results, Golder will evaluate the groundwater concentrations to determine if off-site notification is required.
Golder will temporarily store the investigation derived waste (IDW) purge water generated from the groundwater sampling event in a 55-gallon steel drum(s). A waste characterization sample will be collected from the IDW drum(s) for analysis of total VOCs using EPA Method 8260. If the waste characterization sample is acceptable to the disposal subcontractor, the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.
Upon completion of Subtask 3, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, and laboratory chain-of-custody documentation.
SUBTASK 4 – REPORTING
The soil and annual groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will also update the DEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the DEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.

RE: Utility Locating Services – 1931 Blanding Boulevard, Jacksonville, Florida
Mr. Brown, GeoTek Services is pleased to respond to your request for Utility Locating services at 1931 Blanding Boulevard located in Jacksonville, Florida. We will scan a 10’ x 10’ area around the twenty-eight bore locations shown in your PDF drawing with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings. We will scan as close as reasonably possible to fixed objects such as walls, landscaping, fences, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

ProCheck Inspections appreciates the opportunity to provide this proposal. Feel free to contact me at any time if you have any questions, or if you need additional information.
SCOPE OF WORK
We understand the scope to be to search for underground utilities as identified by the customer within (x28) 10’x10’ areas prior to conducting soil borings. We will attempt to trace any utilities for which there are structures visible from the work area, and mark said utilities with paint and flags where possible. The client will be responsible to provide drawings or notify ProCheck Inspections of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival.
EQUIPMENT
• 350 MHz Hyperstacking GPR Antenna. The antenna is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, standing water, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. GPR works by sending pulses of energy into a material and recording the strength and the time required for the return of the reflected signal. Reflections are produced when the energy pulses enter a material with different electrical properties from the material it left. The strength of the reflection is determined by the contrast in signal speed between the two materials. The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the conductivity of the materials. The patented hyperstacking antenna can improve depth penetration and resolution when compared with a traditional GPR antenna.
• Electromagnetic Pipe Locator. The EM locator can detect the electromagnetic fields from live power or radio frequency signals. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting EM field can then be detected by the receiver. The receiver is moved over the surface without contacting the ground, so it is not affected by terrain. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. Depths achieved can be as much as 20’ depending on the type of signal being traced or methods used.

TERMS & CONDITIONS
1. ProCheck Inspections provides Ground Penetrating Radar (GPR) services in underground applications, including assisting in location services in accordance with ASCE Standard 38-02, Quality Level “B”. GPR is a great tool for detecting objects underground, however, site restrictions and environmental conditions can limit the accuracy and effective depth of GPR. For underground scanning, depth is completely dependent on the composition of soils in the area being surveyed. Customer fully understands that for every foot in depth penetration with the GPR equipment, the pipe/utility must be at least 1” in diameter to be located. For example, at 4 feet in depth, the pipe/utility must be 4” or larger to be detected. Some types of pipes are very difficult to locate, such as clay or concrete pipes, transite piping, and empty PVC type pipes. Given these factors, ProCheck Inspections CANNOT guarantee it will be able to locate ALL utilities on site.
2. Our goal is to provide you with the best answers possible to your questions regarding what lies below the surface, and where it is located. Customer acknowledges that they understand the fact that our answers are based upon the quality of the data retrieved and what ProCheck Inspections believes lies below the surface. The decision to proceed with boring or excavations of any manner, type, or method, it under the sole discretion of the customer and customer assumes full liability for aforementioned processes.
3. ProCheck Inspections will not accept liability for an inaccurate interpretation or any other reason, and customer agrees to release and indemnify ProCheck Inspections and its owners and agents from all losses and damages from all alleged negligence and/or contract claims by customer or any third party. These terms and conditions supersede any other previous terms and conditions either oral or written.
4. Payment is due upon receipt of invoice unless otherwise specified in writing.
5. If for some reason ProCheck Inspections arrives on site and the work is cancelled without prior notification, there will be a charge of
$300.00.

SCOPE OF WORK
Soil Borings We understand the scope to be to search for underground utilities within a radius of approximately 10’ around 28 soil borings. We will attempt to trace any utilities for which there are structures visible from the work area. Utilities will be marked on the surface using paint, flags, or other appropriate means. The client will be responsible to provide drawings or notify GPRS of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival in order to avoid additional charges. Please visit www.simspec.org for an overview of our industry-leading best practices that will be applied to this project.
EQUIPMENT
Underground Scanning GPR Antenna. The antenna frequencies range from 250 MHz-450 MHz is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time.
The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the types of materials being scanned through. Some soil types such as clay may limit maximum depths to 3’ or less. As depth increases, targets must be larger in order to be detected and non-metallic targets can be especially difficult to locate. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link
Electromagnetic Pipe Locator. The EM locator can passively detect the signals from live AC power or radio signals travelling along some conductive utilities. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting signal can then be detected by the receiver. A utility’s ability to be located depends on a variety of factors including access to the utility, conductivity, grounding, interference from other utilities, and many others. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link
Traceable Rodder. The rodder has a copper wire encased in fiberglass. The line is pushed through accessible pipes before placing a current on the wire and the signal is then traced from the surface. The maximum traceable depth is 10’ depending on the soil conditions and the maximum distance is 200’. The line can be pushed through a pipe with direct access such as a sewer line at a cleanout or a storm drain catch basin. It may not be able to be pushed through deeper pipes within manholes. Electrical conduits will not be accessed by GPRS. The signal cannot be located through metallic pipes. For more information, please visit: Link
GPS. This handheld GPS unit offers accuracy down to 4 inches, however, the accuracy achieved will depend on the satellite environment at the time of collection and should not be considered to be survey-grade. Features can be collected as points, lines, or areas and then exported as a KML/KMZ or overlaid on a CAD drawing.
