PROPOSED TASK ASSIGNMENT: HW685_DC023E

Dear Mr. McGrane:

WSP USA Inc. (WSP) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal for annual groundwater monitoring and confirmatory soil sampling at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with the DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Waste Site Cleanup Section requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.

SUBTASK 2 – ANNUAL GROUNDWATER MONITORING

Groundwater Monitoring Following authorization to proceed, WSP will use the Duval County Property Appraiser’s Office website to verify that the current property owner and the offsite property owners have an executed Permission to Enter Property agreement with the DEP. WSP will then coordinate with the real property owner, tenant, and offsite property owners for access to complete the scope of work.

Mr. Patrick McGrane Proposal No. 2025US483724

WSP developed the proposed groundwater monitoring plan following correspondence with the DEP after review of the 2025 Annual Groundwater Monitoring Report. The proposed plan includes collecting groundwater samples from select existing wells for analysis of chlorinated volatile organic compounds (VOCs), as well as total iron, as part of the continued Underground Injection Control monitoring following implementation of the interim source removal.

Figures 3 through 8 from the previous annual report show the well locations, groundwater flow directions, and chlorinated VOC results for the shallow, intermediate, and deep zones of the surficial aquifer.

Prior to groundwater sampling, depths-to-groundwater will be measured at accessible monitoring wells using an electronic water level indicator to determine groundwater elevations and flow directions. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.

Groundwater sampling activities will be completed in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective April 16, 2018). Prior to collecting the groundwater sample, each well will be purged and field parameters, including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.

The groundwater samples will be delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified, fixed-base laboratory along with the completed chain-of-custody documentation. Upon receipt of the laboratory analytical results, WSP will evaluate the constituent concentrations to determine if offsite notification is required.

SUBTASK 3 – CONFIRMATORY SOIL SAMPLING

Utility Location WSP will obtain the applicable local, county, and State-required permits prior to implementing the proposed scope of work. Before the initiation of fieldwork, WSP will contact Sunshine 811 for underground utility clearance and marking. Given that work will be conducted on private property, Sunshine State One Call might not be able to provide details regarding specific utilities in the area where soil sampling activities will be conducted. Therefore, WSP will subcontract with a private utility location company to locate and map shallow subsurface anomalies present within the proposed soil sampling areas. WSP will discuss any conflicts between suspected underground utilities and the proposed assessment locations with the DEP prior to any adjustments in the field.

Soil Sampling A Contamination Assessment Report (CAR), dated February 2000 and completed by Levine Fricke Inc. (LFR), indicated that several soil samples collected west, north, and south of the 2017 large diameter auger (LDA) soil remediation footprint had concentrations that exceeded the DEP Soil Cleanup Target Level (SCTL) for leachability, based on groundwater criteria, for tetrachloroethene (PCE) at depths between 2 and 6 feet below ground surface (bgs). In its August 2025 Annual Groundwater Monitoring Report, WSP recommended collecting confirmatory soil samples at these locations. The DEP agreed with this recommendation and provided additional soil sample locations via e-mail on September 17, 2025, to delineate the extent of the aforementioned soil contamination.

As such, WSP will collect soil samples from 13 soil borings. At each of the soil boring locations, WSP will use a decontaminated stainless-steel hand auger to collect discrete soil samples from the ground surface to the top of the water table. For cost estimating purposes, WSP has assumed the depth-to-groundwater is 7 to 8 feet bgs. The proposed soil sampling plan is included in Table 1. The proposed soil boring locations are shown on the attached Figure 5, which was taken from LFR’s February 2000 CAR.

Soil sampling activities will be completed in accordance with DEP SOPs for Field Activities (DEP SOP-001/01 [FS 3000]), dated January 2017 (effective April 16, 2018). WSP will collect a primary soil sample from the bottom of each of the proposed sampling depth intervals. WSP will also collect a split soil sample using a 25-gram EnCore sampler. The split samples will be placed on hold pending review of the primary sample results.

WSP will place the soil samples on ice and ship the coolers to a Florida Department of Health and National Environmental Laboratory Accreditation Program-certified fixed-based laboratory, along with the completed chain of custody documentation. The laboratory will analyze the primary samples for chlorinated VOCs using Environmental Protection Agency (EPA) Method 8260D. If the results exceed a DEP SCTL for Leachability, based on groundwater criteria, for any constituent, WSP will request DEP approval to analyze the split samples using the Synthetic Precipitation Leaching Procedure (SPLP), if necessary. For cost estimating purposes, WSP has assumed up to 13 split samples will be analyzed using the SPLP.

Following soil sample collection, WSP will backfill the boreholes with any excess soil and sand, as needed. If the boring was advanced into a concrete or asphalt surface, the borehole will be backfilled with sand to approximately 6 inches below grade, followed by a layer of concrete or asphalt patch (as appropriate) to match the existing surface.

IDW Transportation and Disposal WSP will containerize the investigation-derived waste (IDW) purge water and decontamination water in a 55-gallon drum. WSP will collect a waste characterization sample from the drum for analysis of total VOCs using EPA Method 8260D. The drum will be temporarily staged onsite, pending waste characterization. For cost estimating purposes, WSP has assumed that the IDW can be transported and disposed of as non-hazardous waste at an approved facility. The IDW transportation and disposal subcontractor will provide WSP with a non-hazardous waste manifest upon delivery to the disposal facility. WSP estimates that two drums of IDW purge water and equipment decontamination water will be generated during the field activities.

Interim Deliverables Upon completion of Subtasks 2 and 3, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical reports with chain-of-custody documentation, and a non-hazardous waste manifest.

SUBTASK 4 – REPORTING

WSP will prepare an Annual Groundwater Monitoring and Confirmatory Soil Sampling Report that will include a description of the sampling procedures, evaluation of the analytical results; comparison of current to historical data;

tabulated summary of the current and historical analytical data; site maps showing the groundwater and soil analytical results; water level elevation information in tables and figures; laboratory analytical data; and conclusions and recommendations.

HEALTH AND SAFETY

WSP will update the existing site-specific Health and Safety Plan for the fieldwork. The work will be completed in accordance with WSP’s safety policies and procedures. At a minimum, WSP will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities and anytime the scope of work or site conditions change.

COST AND SCHEDULE

The estimated cost to complete the proposed scope of work is $28,842.51. WSP anticipates the groundwater monitoring and soil sampling event will take place in June 2026. The level of effort sheets and cost estimate form are included in Attachment A. Subcontractor quotes are included in Attachment B. RE: Utility Locating Services – 5800 Fort Caroline Road in Jacksonville, Florida

Mr. McCann, GeoTek Services is pleased to respond to your request for Utility Locating services at 5800 Fort Caroline Road in Jacksonville, Florida. We will scan a (10’ x 10’) area around the thirteen proposed bore locations with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as vehicles, fencing, or buildings. We will scan as close as reasonably possible to fixed objects such as obstructions, landscaping, etc.

Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

SCOPE OF WORK

Soil Boring: We understand the scope of work on this project is to search for underground utilities in a 10’ radius around 13 soil bores. We will attempt to trace any utilities for which structures are visible from the work area. Utilities will be marked on the surface using paint, pin flags, or other appropriate means. The client will be responsible for providing drawings or notifying GPRS of any utilities entering the work area for which there are no apparent surface features or structures visible from the work area. To avoid additional charges, the areas should be laid out, marked, and cleared of obstructions before our arrival. Please visit www.simspec.org for an overview of our industry-leading best practices that will be applied to this project.

• GPRS is not responsible for housekeeping. Any debris, equipment, or other obstructions in the area at the time of scanning could potentially block out needed data.

• A thorough utility search can only be completed if GPRS is given access to all utility structures, interior and exterior. This service is never a replacement for the use of the state One Call system (811).

• All of our technicians have OSHA-10 safety training or greater. Site-specific safety training is not included in this quote. Please notify us if this project requires additional safety training.

EQUIPMENT

• Underground Scanning GPR Antenna. This GPR Antenna uses frequencies ranging from 250 MHz to 450 MHz and is mounted in a stroller frame that rolls over the surface. Data is displayed on a screen and marked in the field in real-time. The surface needs to be reasonably smooth and unobstructed to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the efficacy of GPR.

The total effective scan depth can be as much as 8’ or more with this antenna but can vary widely depending on the soil conditions and composition. Some soil types, such as clay, may limit maximum depths to 3’ or less. As depth increases, targets must be larger to be detected, and non-metallic targets can be challenging to locate. The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

• Electromagnetic Pipe Locator. This receiver can passively detect the signals from live AC power or radio signals traveling along some conductive utilities. Operators can connect a transmitter directly to accessible metallic pipes, risers, or tracer wires to generate a current at a specific frequency. The receiver can then detect the resulting signal along the pipe or tracer wire. Various factors may impact this device's effectiveness, including (but not limited to) access to the utility, conductivity, grounding, and interference from other utilities.

The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

• Traceable Rodder. The rodder consists of a copper wire encased in fiberglass. This device is pushed through a pipe with direct access, such as a sewer line at a cleanout or a storm drain catch basin. Operators then induce a current on the wire and trace the signal from the surface. The maximum traceable depth is 10’ depending on the soil conditions, and the maximum distance is 200’. Inserting the rodder into deeper pipes within manholes may not be feasible depending on site conditions. GPRS will not access electrical conduits. The signal is not detectable through metallic pipes. For more information, please visit: Link

• GPS. This handheld unit offers accuracy down to 4 inches; however, the accuracy achieved will depend on the satellite environment at the time of collection and is not considered survey-grade. Features can be collected as points, lines, or areas and then exported as a KML/ KMZ or overlaid on a CAD drawing. For more information, please visit: Link
