Ms. Claire Field Proposal No. 2025US423223

Florida Department of Environmental Protection June 18, 2025

SUBTASK 2 FIELD SAMPLING PLAN/QUALITY ASSURANCE PROJECT PLAN AND

HEALTH AND SAFETY PLAN

This subtask will include a review of historical site files; a site visit to assess current site conditions, including evaluating the integrity of the existing site monitoring wells; and participation in teleconferences with DEP to discuss the site and scope of the PAWS activities. WSP will then prepare and submit a Field Sampling Plan/Quality

Assurance Project Plan (QAPP) and Health and Safety Plan (HASP), which will include the following:

Site location and project planning elements

Project personnel

Brief site background and history

Project description

Proposed groundwater, soil, sediment, and surface water locations, depths, and analyses

Proposed sample location figures

Data quality objectives

Data generation and acquisition methods

Per OSHA 1910.120 requirements, WSP will also prepare a site-specific HASP for the fieldwork. The work will be high visibility safety vests, and steel-toed boots.

SUBTASK 3 PRELIMINARY ASSESSMENT ACTIVITIES

Site Access and Utility Locate

Following authorization to proceed, WSP will use the Duval current property owner and relevant offsite property owners have an executed Permission to Enter Property agreement with the DEP. WSP will then coordinate with the real property owner, tenant (if applicable), and offsite property owners for access to complete the scope of work. WSP or the DEP will attempt to obtain new or updated

Permission to Enter Property forms from relevant property owners prior to and/or during mobilization to the site, as needed.

WSP will obtain the applicable local, county, and State-required permits prior to implementing the proposed scope of work. Before the initiation of fieldwork, WSP will contact Sunshine 811 for underground utility clearance and marking. Given that work will be conducted on private property, Sunshine State One Call might not be able to provide details regarding specific utilities in the area where site restoration activities will be conducted. Therefore, WSP will use a private utility location company to locate and map shallow subsurface anomalies present within the proposed site restoration areas.

Soil Sampling

Soil sampling activities will be completed in accordance with DEP SOPs for Field Activities (DEP SOP-001/01

[FS 3000]), dated January 2017 (effective April 16, 2018). WSP will advance a total of five soil borings to determine potential soil impacts and collect representative soil samples for laboratory analysis. At each of the soil boring locations, WSP will use a decontaminated stainless-steel hand auger to collect discrete soil samples from the ground surface to a total depth of 2 feet below ground surface (bgs). WSP will collect soil samples for laboratory analysis from the bottom of the following depth intervals: 0 to 1.0 feet bgs and 1.0 to 2.0 feet bgs. The proposed soil boring locations are shown on the figure included in Attachment A.

The samples will be placed in the laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of 4° Celsius, and delivered to the DEP Central Laboratory in Tallahassee, Florida, along with the completed chain-of-custody documentation. The coolers and sample containers (including any pre-preserved sample containers) will be provided by the DEP Central Laboratory. The soil samples will be analyzed for the presence of the eight Resource Conservation and Recovery Act (RCRA) metals (arsenic, barium, cadmium, chromium, lead, mercury, selenium, and silver) using EPA Method 6010/7470; organochlorine pesticides (OCPs) using EPA Method 8270E; and chlordane and toxaphene using EPA Method 8276.

Groundwater Sampling

WSP will collect groundwater samples from the following five monitoring wells: MEDW-4, MW-12, MW-14, MW-22, and MW-10D. The attached Figure 1 shows the monitoring well locations.

Prior to groundwater sampling, the depth-to-groundwater will be measured at each of the five well locations using an electronic water level indicator to determine groundwater elevations. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced as necessary.

Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures

(SOPs) for Field Activities (DEP SOP-001/01 [FS 2200]), dated January 2017 (effective April 16, 2018). Prior to collecting the groundwater samples, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be

Groundwater samples will be collected using low-flow sampling methods.

WSP will submit the samples to the DEP Central Laboratory, along with the completed chain-of-custody documentation. The groundwater samples will be analyzed for the presence of the eight RCRA metals (arsenic, barium, cadmium, chromium, lead, mercury, selenium, and silver) using EPA Method 6010/7470; OCPs using EPA

Method 8270E; and chlordane and toxaphene using EPA Method 8276.

Sediment and Surface Water Sampling

, which borders the site. The proposed sediment and surface water sampling locations are shown on the figure included in

Attachment A.

WSP assumes that the sediment sample can be collected with a decontaminated stainless-steel hand auger within the top 6 inches of sediment. Before collecting the surface water sample, WSP will purge approximately three equipment volumes with a peristaltic pump and record field parameters, including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the

Quality Assurance/Quality Control

WSP will submit the following quality assurance/quality control (QA/QC) samples during the sampling event:

Duplicate Samples (Soil): WSP will collect two field duplicate samples immediately after the collection of the primary field sample. The label for the duplicate sample will be consistent with the primary sample and include historical soil analytical results, WSP proposes to collect the duplicate soil samples from the easternmost proposed sample location

(see figure included in Attachment A).

Duplicate Samples (Groundwater): WSP will collect one field duplicate sample immediately after the collection of the primary field sample. The label for the duplicate sample will be consistent with the primary sample and ed on the historical groundwater analytical results, WSP proposes to collect the duplicate groundwater sample from well MW-12.

Equipment Blanks (Soil): WSP will collect one equipment blank for soil samples. WSP will collect the equipment blank by pouring distilled water over the decontaminated soil sampling equipment (hand auger extension and bucket) and collecting the water into a sample container.

Equipment Blanks (Groundwater): WSP will collect one equipment blank for groundwater samples. WSP will collect the equipment blank by pumping distilled water through silicon and/or HDPE tubing and collecting the water into a sample container.

Matrix Spike and Matrix Spike Duplicate (MS/MSD) Samples (Soil): WSP will collect one MS/MSD sample in the same manner and at the same time and location as a primary sample (i.e., additional sample volume), preferably in an area known to have little to no contamination. If the samples do not appear visually similar, WSP will choose another location for the collection of the MS/MSD sample. The MS/MSD sample will be labeled with the same sample name and time as the primary sample and denoted as an MS/MSD sample on the chain-of-custody and sample label. Based on the historical soil analytical results, WSP proposes to collect the

MS/MSD sample from the southernmost proposed sample location, adjacent to monitoring well MW-22 (see figure included in Attachment A).

MS/MSD Samples (Groundwater): WSP will collect one MS/MSD sample in the same manner and at the same time and location as a primary sample (i.e., additional sample volume), preferably in an area known to have little to no contamination. If the samples do not appear visually similar , WSP will choose another location for the collection of the MS/MSD sample. The MS/MSD sample will be labeled with the same sample name and time as the primary sample and denoted as an MS/MSD sample on the chain-of-custody and sample label. Based on the historical groundwater analytical results, WSP proposes to collect the MS/MSD sample from well MW-22.

Each QA/QC sample will be analyzed for the presence of the eight RCRA metals (arsenic, barium, cadmium, chromium, lead, mercury, selenium, and silver) using EPA Method 6010/7470; OCPs using EPA Method 8270E;

and chlordane and toxaphene using EPA Method 8276.

Investigation-Derived Waste

WSP will containerize the investigation-derived waste (IDW) purge water and decontamination water in a 55-gallon drum. WSP will collect waste characterization samples from the drum for laboratory analysis using the Toxicity

Characteristic Leaching Procedure for the eight RCRA metals using Method 1311/6010/7470; OCPs using

Method 1311/8270E; and chlordane and toxaphene using Method 1311/8276. The drum will be temporarily staged onsite, in a secure location, pending waste characterization. For cost-estimating purposes, WSP has assumed that the IDW can be transported and disposed of as non-hazardous waste at an approved facility. The IDW transportation and disposal subcontractor will provide WSP with a non-hazardous waste manifest upon delivery to the disposal facility. WSP estimates one drum of IDW purge water will be generated during the field activities.

Interim Deliverable

Upon completion of Subtask 3, WSP will submit an e-mail deliverable that will include field documentation (field data sheets, equipment calibration forms, and field notes), laboratory analytical reports, the non-hazardous waste manifest, and site photographs.

SUBTASK 4 REPORTING

WSP will prepare a Preliminary Assessment With Sampling Activities (PAWS) Report deliverable that will include a description of the site history, pathway targets, field activities and sampling procedures, evaluation of the analytical results; comparison of the results to State Chapter 62-777, FAC Cleanup Target Level (CTLs); a tabulated summary of the analytical data; a site map showing the soil, groundwater, sediment, and surface water analytical results exceeding CTLs; water level elevation information in tables and figures; laboratory analytical data; and conclusions and recommendations.

COST AND SCHEDULE

The estimated cost to complete the proposed scope of work is $15,318.93. WSP anticipates the sampling activities will take place in July or August 2025. The level of effort sheets and cost estimate form are included in Attachment B.

RE: Utility Locating Services Former Clark Pest Control, Jacksonville, Florida

Mr. Whipple, GeoTek Services is pleased to respond to your request for Utility Locating services at the Former Clark Pest Control site located in Jacksonville, Florida. We will scan a

5 proposed bore locations with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings. We will scan as close as reasonably possible to fixed objects such as walls, landscaping, fences, etc.

Our deliverable will be marking paint and/or flags indicating the location of potential

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.
