About this file
This is a Task Assignment Change Order (TACO #1) issued by the State of Florida Department of Environmental Protection (FDEP) under Contract HW685 with WSP USA Inc. The change order amends Task Assignment DC006B for environmental remediation and monitoring work at the Former Jay's Professional Dry Cleaners and Laundry located at 10025 San Jose Boulevard, Jacksonville, Duval County (Project ERIC_4521). The document was executed on March 6, 2024, by Matthew Crews (Environmental Engineer) for WSP USA Inc. and Jennifer Farrell (Secretary or Designee) for FDEP. The project manager for FDEP is Patrick McGrane, and the contractor's project manager is Matt Crews.
The change order authorizes a cost reduction and schedule extension due to changes in WSP labor assignments and subcontractor utilization related to the installation of monitoring well MW007R. The total task assignment value decreased from $23,802.55 to $22,103.91 (a reduction of $1,698.64). All deliverable due dates were extended by approximately 90 days, with the task completion date extended from October 31, 2024 to January 16, 2025 (77 additional days). The contract covers quarterly UIC (Underground Injection Control) and annual groundwater monitoring activities including well installation, groundwater sampling, laboratory analysis, field documentation, and comprehensive reporting. Payment is structured as a fixed unit rate with invoicing tied to specific deliverables across six subtasks: well installation and first quarter monitoring ($4,989.02), second quarter monitoring ($3,261.26), third quarter monitoring ($3,261.26), fourth quarter and annual monitoring ($4,966.19), and project management and reporting ($5,626.18).

SUBJECT: Negotiation Summary Task Assignment Change Order DC006B-1 (HW685)
DATE: March 5, 2024
This Change Order authorizes a total cost decrease, a 57-day TA completion date extension, and a deliverable due date extension due to changes in WSP Labor conducting Subtask 2 and in subcontractor utilization. Further information is found in the March 4, 2024, change order email with Rick McCann of
WSP.
The previous cost total was $23,802.55. Total costs were decreased by $1,698.64 resulting in a new final cost total of $22,103.91. All deliverable due dates were extended by ~90 days. The task assignment completion date is now 12/27/2024. WSP may invoice according to the included schedule.
Specific negotiated subtasks are as follows:
Subtask 1:
Project Management is automatically calculated as a percentage of total labor in accordance with
Contract HW685.
Subtask 2:
There were multiple changes in WSP labor conducting the field work in Subtask 2, reducing costs. Due to scheduling issues during the installation of MW007R, some subcontractors costs changed or were not utilized at all.
