Task Assignment Change Order Summary
This document is a Task Assignment Change Order (TACO #2) issued by the State of Florida Department of Environmental Protection (FDEP) for Contract HW685, Project ERIC_4521. The parties involved are the State of Florida Department of Environmental Protection and WSP USA Inc., a private environmental consulting contractor. The project concerns groundwater monitoring and assessment activities at the Former Jay's Professional Dry Cleaners and Laundry facility located in Jacksonville, Duval County, Florida.
The work encompasses quarterly groundwater sampling, monitoring well installation and maintenance, laboratory analysis, field documentation, and comprehensive reporting related to Underground Injection Control (UIC) parameters and chlorinated volatile organic compounds (CVOCs) monitoring. This change order authorizes a cost reduction from $22,103.91 to $21,963.51, reflecting a decrease of $140.40 due to changes in WSP labor assignments for Subtask 3 field work. The original task assignment completion date was extended from October 31, 2024, to January 16, 2025 (77 days) under TACO #1. The deliverables include quarterly monitoring reports with FDEP approval, due on specified dates through November 22, 2024, with corresponding invoice amounts ranging from $3,261.26 to $5,626.18 for individual subtasks. Project management costs are automatically calculated as a percentage of total labor in accordance with Contract HW685.
